Continue paying employee p r e m i u m share to former Agency. Agency must manually enroll employee in Benefits Billing Module. Agency HR or Benefits Yes Up to 30 Months under COBRA Employee must elect continuation coverage w/in 60 days from date coverage would end or date of COBRA notice, whichever is later. Employee pays 102% of the full premium (employee portion plus state portion). Employee must pay full premium w/in 45 days of election. Tuition Reimbursement Yes An employee who completes an approved course prior to the effective date of layoff remains eligible for reimbursement. Submit any unpaid bills or medical reports to Gallagher Bassett. Tier IIA and Tier III employees may receive a refund of retirement contributions if they are not vested and eligible for any immediate or future retirement benefits. Hybrid refund of retirement contributions Hybrid employees may elect a withdrawal of retirement contributions. 5
