Croydon Healthy Living Pharmacies Quality Criteria - Service Requirements and Quality Criteria The Healthy Living Pharmacy (HLP) framework is a way of bringing together the many services that can enhance public health and reduce inequalities, within the community pharmacy setting. Becoming a Healthy Living Pharmacy is about adopting a philosophy and a new working culture within your team where the public s health and well-being is at the centre of what you do. In a Healthy Living Pharmacy, the whole team works together and recognises that every interaction with a customer is a potential opportunity for a health intervention. The aim of this document is to set out the steps needed to achieve HLP status and to be awarded the quality mark that indicates they are a Healthy Living Pharmacy. In setting out on the journey to become a HLP, pharmacies are reminded of the key aim of the programme: that a customer in a Healthy Living Pharmacy can expect a professional service at all times from a knowledgeable pharmacy team who put healthy living at the heart of their business approach. To that end, potential applicants to the programme are reminded that HLP is a quality framework for service delivery. Becoming a Healthy Living Pharmacy requires more than just having services commissioned. It requires demonstration of consistent, high quality service delivery, premises fit for purpose, trained and skilled staff and engagement with other healthcare professionals and the public. The pharmacy team will pro-actively provide good quality advice on healthy lifestyles. Further details on the overall approach of the HLP programme are set out in the Prospectus for the programme and are not repeated here. Croydon Healthy Living Pharmacies Quality Criteria - Achieving Healthy Living Pharmacy Status Achieving HLP Status To ensure that Healthy Living Pharmacies can deliver a range of services consistently to a high standard, a set of quality criteria and service requirements are shown in this document to support the award of the HLP Quality Mark. This document will help pharmacy teams decide whether it s time to apply for the Healthy Living Pharmacy Quality Mark and take you, step by step, through what you need to do to achieve the award. In the first instance, we are looking to commission 18 Community Pharmacies to become HLPs. This equates to 3 in each of the Primary Care Networks in the borough. The Networks include: East Croydon Thornton Heath Selsdon and New Addington Purley Woodside and Shirley Mayday HLP Application Process Each pharmacy wishing to become a Healthy Living Pharmacy may submit an application as soon as the pharmacy team consider that all the quality criteria and service requirements of a HLP can be met. The application process will run until Monday 13 April at 12:00. Self-Assessment This self-assessment is designed to help you and the body awarding the Healthy Living Pharmacy quality mark to understand whether you have met the Healthy Living Pharmacy service requirements and quality criteria. It is in two parts: a) Service requirements b) Quality Criteria Going through the self-assessment process will help you develop your action plan and engage with your team around developing the ethos of Healthy Living Pharmacy. Activities undertaken to become a Healthy Living Pharmacy should also benefit other aspects of your practice and business. Achieving HLP Status 4

Croydon Healthy Living Pharmacies Quality Criteria - How to Assess Yourself How to assess yourself This assessment is for an individual pharmacy. Each pharmacy in a group/chain will complete its own assessment. Read the service requirements and quality criteria and, together with members of your team, decide which criteria you are achieving and those for which you are Getting There. Identify where you have already met criteria and service requirements; think about how you will demonstrate this. Decide how you will provide information in each of the categories. In some cases, e.g. the number of four week smoking quits claimed, the Working Group will be able to verify your submission directly. In other cases you may be asked to provide supporting information yourself think about how you can do this and what information you will need to gather. You may feel that you are on the way to meeting requirements for Healthy Living Pharmacy Getting There - but need to do some more work in your team: think about what you need to do to Get There. You are encouraged to ensure that you meet the HLP standard before submitting your application to save time in the approval process. Supporting information should be provided in a portfolio; this is not intended to be burdensome. The portfolio should include information about all the quality criteria and service requirements in one place so that the panel has the information it needs to review your application. The panel s intention is that wherever possible, data collected for another purpose will be used so that you do not have to duplicate effort, although it may be appropriate to include a note to reference other information, for example, having signed up for locally commissioned services. You will need to demonstrate that you have met the criteria but you do not need to write detailed or complex reports. For example, for the HLC training requirements, copies of the RSPH awards received by your HLCs with a note about how they cover the opening hours of the pharmacy would be sufficient. Other examples could include dates and notes of meetings where training is discussed, training certificates, written records of signposting and interventions, policy How to Assess Yourself 5

Croydon Healthy Living Pharmacies Quality Criteria - How to Assess Yourself documents, or similar. Photographs with brief explanations may be useful, for example, to show the health promotion zone in the pharmacy and health promotion materials used by staff with customers. You will need to provide supporting information for your achievement of some criteria (guidance is given in each section as to the types of supporting information that could be supplied). Some criteria can be self-reported via a web-based reporting tool. Assessment Your application will be assessed by the local Healthy Living Pharmacy Panel. Its membership consists of: Sharon Boakye Healthy Living Pharmacy Project Manager, Public Health Croydon. Ellen Schwartz, Consultant in Public Health, Croydon Public Health Barbara Jesson, Community Pharmacy Advisor / Medicines Optimisation Lead, Croydon CCG Chris Hartfield, Project Manager, South West London, Academic and Social Care System Matt Phelan, Public Health Principal, Croydon Public Health The panel has also invited Andrew McCoig, Chief Executive, Croydon LPC as an advisor throughout the Healthy Living Pharmacy process. The pharmacy will be visited by panel members in order to verify some of the information you provide. An independent assessment will also be conducted before HLP status is awarded. Review The Healthy Living Pharmacy status of each pharmacy will be reviewed at least every two years. If a pharmacy team fails to uphold the quality criteria and service requirements of Healthy Living Pharmacy, the team will have their Healthy Living Pharmacy status removed in order to maintain confidence in those teams that are delivering excellent care through Healthy Living Pharmacy. Evaluation An extensive evaluation of the programme will be conducted to assess viability for further role out across the Borough in the coming years. Consideration will be given to the roll of mystery shoppers and we may wish to conduct qualitative research through interviews and photographs, both at the beginning and throughout the Programme. 6

Croydon Healthy Living Pharmacies Quality Criteria - Service Requirements Service Requirement 1: HLP Leadership-trained pharmacist The lead pharmacist must have undergone leadership training. The pharmacist will lead the pharmacy team in delivering the Healthy Living Pharmacy. The leadership programme for Pharmacists is being delivered by HESL. Please contact your LPC / HLP Project Manager who will signpost you to the appropriate contact in HESL. Suitable leadership training is available from CPPE. Certificate from an approved organisation. Service Requirement 2: Trained Healthy Living Champion(s) At least one non-pharmacist member of staff is trained and appointed a Healthy Living Champion. The Healthy Living Champion(s) will actively promote healthy living among other staff and patients. At least one Healthy Living Champion is trained to a minimum of Royal Society of Public Health Level 2 Understanding Health Improvement. Certificate from an approved organisation. Demonstrating how Champions knowledge will be covered throughout Pharmacy opening hours. Service Requirement 3: Essential Services Delivery of all essential services as detailed in the current contractual framework. The pharmacy will meet the contractual monitoring requirements for all Essential Services: Dispensing Repeat Dispensing Supply of Appliances Waste Management Public Health campaigns Signposting Support for self-care Clinical Governance Self-declaration. Service Requirements 7

Croydon Healthy Living Pharmacies Quality Criteria - Service Requirements Service Requirement 4: Advanced Services Delivery of specified advanced services as detailed in the current contractual framework. Medicines Use Review activity New Medicines Service activity Medicine Use Reviews The pharmacy will be accredited to provide MURs. Pharmacists will be trained and able to deliver MURs. The pharmacy will have a consultation room that meets the specification for MURs. The pharmacy should be actively delivering MURs. The pharmacy must perform at least 150 MURs between 12 months of being awarded HLP Status. Five anonymised targeted respiratory MURs must be submitted for evaluation at the time of application for the award of the Healthy Living Pharmacy Quality Mark. New Medicine Service The pharmacy will be delivering the NMS. Pharmacists to have completed the self-assessment and be able to deliver the NMS. The pharmacy should be actively delivering the NMS service every month. Service Recommendation 5: Locally Commissioned Services Delivery of key commissioned services. The pharmacy must be delivering activity in the following Commissioned Services: Smoking Cessation Minor Ailments 1 Medicines Optimisation Service 2 Flu vaccination service The Pharmacy should be able to demonstrate active engagement in delivery of these services. 1 The Pharmacy First minor ailment scheme is a service commissioned by Croydon CCG to enable patients easy access to NHS treatments for minor ailments. The consultation, at a pharmacy, follows agreed protocols and supply of medicine, if deemed necessary, from a formulary. The service aims to free up GP time and reduce demand on Urgent Care Centres 2 The Medicines Optimisation Service is a focus on supporting patients to get the best use from their prescribed medicines e.g. New Medicines Service, Medicine Use Reviews, and Domiciliary Medicine Reviews. 8

Croydon Healthy Living Pharmacies Quality Criteria - Service Requirements Service Recommendation 6: Additional Commissioned Services HLPs offer a wide range of health and health improvement services direct to the local community. Delivery of at least one further commissioned services from the list below: Sexual health services NHS Health Checks Needle and Syringe Exchange Service Supervised Self Administration of Methadone/Buprenorphine Chlamydia screening Flu vaccination CCG waste initiative scheme 3 HLPs will be asked to agree to deliver another service upon being awarded HLP accreditation. The Pharmacy should be able to demonstrate active engagement in delivery of these services. Service Recommendation 7: Standards, Ethos and Environment The pharmacy will actively promote free health, healthy living and lifestyle services as commissioned by the NHS or local authority. Pharmacies are expected to ensure the content and style of advertising material for products and services are consistent with Healthy Living Pharmacy. The Pharmacy must not stock, supply, sell or otherwise trade in nicotine delivery devices or products that are not licensed by the MHRA as part of a stop smoking programme. This applies to all products commonly known as e-cigarettes or shisha pipes which are at present unlicensed for smoking cessation. Licensed products will be available from May 2016. We will allow a discretionary period of 3 months for any stock to be used up that has already been purchased from the point of awarding HLP status. There is no barrier to a Healthy Living Pharmacy providing private services, for example weight management. However, Healthy Living Pharmacies are expected to ensure that all members of the public who wish to use such private services are aware of free local services commissioned by Local Authorities, CCGs or other commissioners. All pharmacy staff, including any locum or temporary staff, will be aware of all the services that the pharmacy is contracted to provide as well as other local healthy living services. All such services will be actively promoted. All staff will be aware of the nature of the service and who is able to provide it. If services are not available at any time pharmacies will signpost the customer to another local provider or invite the customer to return to the pharmacy on another occasion depending on customer preference. 3 This waste initiative scheme compensates community pharmacies for not dispensing a valid prescription, where that prescription is for medicines that are not required e.g. have been stopped or the patient did not order. 9

Croydon Healthy Living Pharmacies Quality Criteria - Service Requirements Service Recommendation 8: Environment and Professional Standards The pharmacy and associated staff should uphold the standards as set out by the GPhC for pharmacy professionals and premises. Any regulatory investigations will trigger a review of the Healthy Living Pharmacy status. We expect to be informed within one month of being informed of any investigations processes. Service Recommendation 9: Stakeholder Engage The pharmacy needs to be proactive about informing GP practices about the services it offers and how this can support their patients. Pharmacy will provide supporting information of how it has engaged with GP practices regarding provision of services. Service Recommendation 10: Community Engagement The pharmacy needs to be proactive about reaching out into the community about the services it offers and how this can support the local population. Pharmacy will provide supporting information of how it has engaged with the local community to improve uptake of services offered both within the HLP and elsewhere. Suitable supporting information might be records of meetings or examples of targeting patients e.g. for MURs in line with GP agenda. 10

Croydon Healthy Living Pharmacies Quality Criteria - Quality Criteria This self-assessment is designed to help you and the body awarding Healthy Living Pharmacy (HLP) status understands whether you have met the HLP quality criteria for the environment you have created. This, together with the service requirements will help the panel decide whether your pharmacy can be a Healthy Living Pharmacy. The evidence you put together will help towards you receiving your Healthy Living Pharmacy quality mark. These quality criteria include the environment, staff attitudes and training, information provision and engagement with others through joined up working. These are not listed in any priority order; all are equally important. The General Pharmaceutical Council sets standards for the safe and effective practice of pharmacy from pharmacy premises. These are the core standards that all retail pharmacies must meet and would be a minimum requirement for all. These HLP quality criteria support a pharmacy in meeting their professional requirements when delivering healthy living services. Appendix 1 outlines the HLP National Framework. It is envisaged for the initial stages of the scheme pharmacies will be accredited at one level with no distinctions between the levels of provision. The HLP quality criteria may, in time, incorporate additional criteria where it is relevant for HLP levels 2 and 3. Quality Criteria 11

Croydon Healthy Living Pharmacies Quality Criteria - Self-Assessment Healthy Living Pharmacy: Quality Criteria Self Assessment Questionnaire Introduction This self-assessment is designed to help you and the body awarding Healthy Living Pharmacy (HLP) status understand whether you have met the HLP quality criteria for the environment you have created. This, together with other service specific criteria will help commissioners decide whether your pharmacy can be a Healthy Living Pharmacy. The evidence you put together will help towards you receiving your Healthy Living Pharmacy quality mark. These quality criteria include the environment, staff attitudes and training, information provision and engagement with others through joined up working. These are not listed in any priority order; all are equally important. The General Pharmaceutical Council sets standards for the safe and effective practice of pharmacy from pharmacy premises. These are the core standards that all retail pharmacies must meet and would be a minimum requirement for all. These HLP quality criteria support a pharmacy in meeting their professional requirements when delivering healthy living services. The HLP quality criteria may, in time, incorporate additional criteria where it is relevant for HLP levels 2 and 3. How to assess yourself This assessment is for an individual pharmacy. It is not appropriate to complete a single form for a number of pharmacies within a group, as the status may be very different in each location. Read the quality criteria and, together with members of your team, decide which level you are achieving. You may be required to provide supporting information in each of the categories. Where you do not meet the requirements for HLP Level 1, think about what you need to do to achieve the criteria. The criteria for staff relate to those individuals working within the scope of the pharmacy business, they do not apply to staff working in larger stores who do not interface in the health aspects of the business. Quality Criteria The following describes the different levels of service delivery that form the self-assessment Getting there: Some arrangements are in place and the pharmacy is moving towards achieving the criteria. If arrangements are not in place, there is a robust action plan to achieve the criteria. Meets HLP: Arrangements are in place that meets all criteria and overall the service is working at that level of provision. Self-Assessment 12

Croydon Healthy Living Pharmacies Quality Criteria - Self-Assessment Workforce Development The aim is to ensure that, irrespective of premises, the pharmacy staff demonstrates that they embrace the healthy living ethos. 1. Staff attitude, skills, values and training. All pharmacy staff understand the basic principles of health and wellbeing. All pharmacy staff have some understanding of the public health needs in their area. Members of staff receive training on issues of confidentiality and consent relevant to the member of the public receiving the service. The team are friendly, welcoming and sensitive to the need for privacy for different individuals seeking advice and health services. Members of the pharmacy team are able to communicate effectively and sensitively when recruiting members of the public to health and wellbeing services or providing them with advice. The team recognise the need for equality and diversity; providing a person-centred approach. The team understand that every interaction is an opportunity for a health intervention. There is a clear leader within the team who is responsible for achieving the vision and creating an ethos of proactive health and wellbeing within the pharmacy. 13

Professional environment The professional environment reflects the impression and ethos of a Healthy Living Pharmacy, pro-actively promoting health and wellbeing, to the public e.g. the appearance and dress of the staff and premises. A healthy living pharmacy takes its responsibility to contribute to a sustainable environment seriously and this is reflected in the way it operates its business. It is clear to the public that free, confidential advice on public health and wellbeing can be accessed. There are appropriate materials readily available for members of the public on health and wellbeing. This information is refreshed and checked regularly to ensure that it is current and relevant. The information available appeals to a wide range of the public including men and women, young people, smokers, people with long term conditions, learning difficulties and older people. Where the community includes a significant ethnic minority group, then their needs should as far as possible be accommodated. There is a clearly marked and accessible health promotion zone. Written and verbal information is readily available for proactive health and wellbeing interventions by the member of the pharmacy team. Croydon Healthy Living Pharmacies Quality Criteria - The Healthy Living Pharmacy Framework The Healthy Living Pharmacy Framework A Healthy Living Pharmacy National Framework has been developed based on the Portsmouth and early adoption sites learning. We recognise that pharmacies are currently at different levels, therefore while the programme in Croydon aspires to use this framework in future to support improvement in HLP locally, it is, at this stage provided for reference only. For the initial stages of the scheme pharmacies will be accredited at one level with no distinctions between the levels of provision. The framework is intended to act as a guide to inform and create appropriate pathways that improve access and outcomes for patients specific to their locality and needs. The services listed are exemplars from across the country where community pharmacy has demonstrated the potential for positive outcomes. The bottom part of the framework details the enablers (quality criteria) that will support pharmacies to make this transition. There are three levels (detailed in Table 1) at which Healthy Living Pharmacies could operate which build on the core Essential and Advanced services provided by community pharmacies as part of the national contractual framework.
