KNOWLEDGEABLE INNOVATIVE ESTABLISHED HSA Online Enrollment Instructions With a Health Savings Account (HSA), you ll pay less in taxes and increase your take-home pay. Take control over your health care dollars and enroll in an HSA today through Flex-Plan Services. A Health Savings Account (HSA) works with a High Deductible Heath Plan (HDHP), and lets you set aside a portion of your paycheck before taxes into an account to pay for medical expenses that aren t covered by your plan. It can also help you plan for future medical expenses. You can deposit money into your HSA account up to an annual per person or family limit set by the IRS. When you enroll, an account will be created for you at a sponsor bank and you ll have access to a secure, easy-to-use web portal where you can track your account balance, view your investment accounts and submit requests for reimbursements. You ll get a convenient Benny Card that makes it easy to access your funds. In order to enroll, you will need to register for your online account at www.flex-plan.com. If you ve already registered an account with us, you can skip the registration step and log directly into your account. Step 1: To register for your online account access, select any of the links labeled Participant to direct you to the participant information page. From here, you can access your account by selecting Manage My Account. First time users will select Register with Flex-Plan.com. You will need the following information to register your account: Last Name, First Initial Company E-mail Address (this can be changed after registration) Company Code: (ex. XXX) Choose a User Name Date of Birth Step 2: Once you ve logged into your account on the Participant Portal, click on the link Health Savings Account (HSA) Step 3: Begin online enrollment by clicking the HSA Enroll link in the HSA Enrollment Section.

Step 4: Review the Summary of Accounts. Click Next to continue. Step 5: Check that you have read and agree with the Designated Representative Agreement, the HSA Custodial Agreement and Disclosure Statement and the Patriot Act Requirement. Click Next to continue. You must read and agree to each section to continue

Step 6: Verify and enter missing Consumer Profile information. Click Next to continue. Fields with a red asterisk (*) are required. Your social security number is required to set up your HSA account. Your email address allows you to receive notifications and other important information quickly.

Step 7 (if applicable): Enter Dependent information and click Add Dependent. The added dependent displays under the My Dependents list. Click Next to continue. Step 8: Review the HSA Qualifications and then check that you certify that you are eligible for a Health Savings Account. The Qualifying Health Plan Coverage options on this screen vary depending on your plan. Click Next to continue.

Step 9: Enter your election amount in the My Election field. Click Next to continue. Step 10: Select your Payment Methods. Click Next to continue.

If selecting Direct Deposit enter your bank account information. The Bank Information may pre-populate based on the routing number entered. If not, enter the contact information for your bank. A message will alert you about the micro deposit process to activate the account. Click Add Bank Account to continue.

Step 11 (Optional): Select beneficiaries from the Dependents box to pre-fill the information. Click Add Beneficiary. Add a contingent beneficiary by choosing them from the Dependents box or complete the fields, and click Add Beneficiary. Beneficiaries and Share Percentages display and you may Update or Remove as necessary. Click Next to continue. Note: During Enrollment, you must name your spouse as a primary beneficiary if you are married.

Step 12: Verify your Enrollment Summary information carefully. Click Edit Information in any of the sections to make corrections. Click the Print link for a copy or your Enrollment Verification. Click Submit. The final step is to complete the HSA Account Creation Authorization requesting that an HSA be opened in your name. Click Submit Enrollment.

Step 12: Congratulations! You have now successfully completed your HSA enrollment! Please contact Flex-Plan Services if you have questions. Once your enrollment is completed, you can update your enrollment at any time during the open enrollment period from the home page, by clicking Update.
