St. John Fisher College APPLICATION FOR JUNIOR CLINICAL YEAR SPRING 2016 Students apply for the nursing junior clinical year during their second semester sophomore year. Transfer students should apply 1 2 semesters prior to the nursing junior clinical year. Transfer students must be accepted to St. John Fisher College before their application for the clinical year will be reviewed. Prerequisite courses may be in-progress when the application is submitted but must be completed with a C or higher before starting the clinical program. At the time of application for the clinical year, students should have completed at least 55-60 liberal arts and science credits of the college core. Student academic progress is monitored and updated until the student starts the clinical year. Only students who were enrolled as SJFC freshman nursing majors are given preferred placement in the junior clinical year. For all others, admission to the junior clinical year is competitive. Acceptance to SJFC does not mean you have been accepted into the clinical portion of the nursing program. Students must meet the minimum requirements below to be considered for admission to the junior clinical year. The Admissions Committee will identify the most qualified eligible candidates. Admission to the program is competitive and preference may be given to applicants who exceed minimum requirements. Minimum requirements for admission to the junior clinical year are: Cumulative GPA of 2.75 or higher from all previously attended colleges. Prerequisite GPA of 2.40 or higher that includes all attempts at anatomy, physiology, microbiology, chemistry, nutrition, statistics and lifespan development. A minimum grade of C is required for all prerequisite courses. Score 84% or higher on the SJFC Math Assessment Level A exam. Full criteria and application materials are attached. Procedures to Apply for the Spring 2016 Clinical Year Please read the following information carefully. Applications are due on or before September 1, 2015. Application for the Clinical Year: Complete the application form carefully. Fill in all information and sign the application. Scholastic Ability: Selection to enter the junior clinical year is competitive. Minimum cumulative Grade Point Average (from all previously attended colleges) is 2.75. A grade of C or better must be achieved in each of the prerequisite courses: anatomy, physiology, microbiology, chemistry, statistics, nutrition, and lifespan development. The minimum prerequisite GPA is 2.40, including all attempts at the seven sciences. Mathematics Screening Test: For ALL Applicants: To meet the math competency, students must get a grade of 84% or above on the Math Assessment Level A. Although students have a total of four chances to meet this competency, failure to score 84% or above after the second attempt requires the student to complete the Math Remediation Contract (available on BlackBoard or the SJFC WSON website). Students must complete the terms of the contract for remediation before taking the test for the third time. If a student does not meet the math competency after four tests, the student will not be eligible for the nursing clinical program at Fisher. The math competency must be met for the clinical application to be complete. Testing date information will be available through the Math Center and on the WSON website. If you are currently a sophomore SJFC Nursing Major: You should have access to the BlackBoard (BB) site for the WSON Math Screening Test. If you cannot access this BB site, call Judy Martz in the WSON office at 385-8241 to have your name added. Incoming transfer students do not have access to BlackBoard and will sign in as a transfer student when taking the math exam. This is a paper test and must be taken at SJFC. Personal Statement: Please submit a short (1-2 page) well-developed essay, which speaks to your specific professional plans and career objectives. The quality of your essay is an indication of your ability to constructively express your thoughts in a cogent manner. Please view this essay as an opportunity to analyze your past professional development (if applicable) and consider future contributions as a result of professional nursing education. Submit your essay on a separate sheet of paper, typed, double-spaced, and clearly identified with your name and student identification number (if available). References: Two references are required for all applications to the clinical year. The appropriate forms are included in this packet. At least one of the references must be from a college-level professor from one of your seven prerequisite science classes. The other may be from an employer or person who can comment on personal characteristics. Family members or personal friends are NOT appropriate references. Forward a reference request to each individual you select to serve as a reference for you and indicate that the form is to be returned directly to St. John Fisher College,, Attn: Judy Martz, in a sealed envelope. The following requirements are not due with the application but must be submitted after you have been accepted into the program -- about six to eight weeks prior to starting clinical: CPR Certification: o Contact either the American Heart Association (course: BLS for Health Care Providers) or the American Red Cross (course: CPR/AED for Professional Rescuers and Health Care Providers) for this requirement. Students with Advanced Cardiac Life Support (ACLS) must still complete Basic Life Support (BLS) to meet this requirement. Your CPR is good for one or two years depending on location. Submit a copy of your card and keep the original. o Send a copy of your CPR card to the. Students are required to have current CPR certification for each semester they are in clinical. Physical Examination and Immunizations: Students are expected to have a current physical examination or health update, PPD, and immunizations on file each semester/year they are in clinical. o The College s Health History Form must be submitted to the Wegmans School of Nursing. ALL students must have a history and physical completed by a health care provider dated no more than 6 months before beginning the first semester of the clinical year clearing them for full participation for nursing student clinical training. After the junior year, an annual health update is required. The form is available on the nursing website, or you may also use a health update form to indicate you continue to meet health requirements for clinical work. o ALL students must submit an immunization history or proof of immunity against measles, mumps, rubella, meningitis, polio, tetanus, chicken pox, hepatitis B, as well as proof of an annual PPD. o Accepted students need the seasonal flu shot each year in order to participate in clinical. Plan on getting this when the new season s flu vaccine is available, usually after the middle of August. o For students with a positive PPD, a chest x-ray report must be submitted. NOTE: Nursing students are NOT exempt from immunizations against measles, mumps, and rubella due to age when entering the clinical component of the program. Your application to begin the clinical year is considered to be complete ONLY when: You have passed the WSON mathematics screening exam The application is submitted The personal statement is submitted Both references are submitted. A transfer student has been accepted to St. John Fisher College No student will be considered for the clinical program until ALL these items have been received.
