RISK ASSESSMENTS When you arrive at a worksite, you need to talk to the people in charge and undertake an induction to allow you onsite. This is all necessary to make you aware of the correct practices and procedures of that site. Once you have accessed the worksite you need to plan your work and inspect your working area for hazards. PLAN YOUR WORK AND RISK ASSESSMENT Before doing any work, you need to plan for it. For example, what have you got to lift? What equipment is available for you to use? Do you need an attachment for the machine? Do you know where you have to move it to? Who do you need to communicate with to get this done? Have you got the paperwork you need? Do you need any additional permits to do this? HAZARD IDENTIFICATION You must then decide what to do with the identified hazards. The best option is to try and get rid of the hazard or eliminate it. If not possible, use the hierarchy of control. (You should memorise this) Elimination of the Hazard Substitution of the Hazard Swap the hazard for something safer Isolation Restrict access to the hazard Engineering Control Use equipment to minimise the risk of the hazard, redesign or enclose etc.. PERSONAL PROTECTIVE EQUIPMENT PPE should be selected before you start work. This will normally be safety boots and high vis. Some sites require goggles, gloves, ear plugs, hard hats, etc. So now you have your PPE on you should decide what safety measures you are going to use. These are methods of controlling the hazards or limiting the risk. REMEMBER PEOPLE AND MACHINERY DO NOT MIX Examples of hazards control measures are: Barriers Signs Spotters Designated walk ways Speed limits PPE These control measures should always be applied prior to starting work. If you did have an accident and someone got hurt, how did you try and prevent it? You would need to communicate quickly to supervisors, co-workers and emergency personnel. ELECTRICITY If you do have overhead power lines on site they can be very dangerous, you need to keep well away as electricity can arc (jump). You have exclusion zones around power lines which can also be found in the Electrical Code of Practice. If you wanted or need to get closer than this, your best option would be to isolate the power. (Turn it off) If this is not possible you can get an approved electrical spotter or call Energex. It is not a bad idea to fit torapoli pipes (tiger tails). If you do accidently hit power lines you should remain calm and make sensible decisions. You may risk your life if you panic. The safest option may be to stay in the machine and wait for the supplier to turn the power off. If the machine catches fire you may have to jump clear, don t earth yourself. You may need to shuffle or hope away for at least 8 metres. Report the incident to management and the power authority and always have the machine checked over prior to further use. TRAVEL PATH When deciding on your travel path, be aware of where you are going - through doorways, one ways and high pedestrian traffic areas. If you do need to drive in a cool room or in an area with restricted ventilation you should use a battery operated machine or a hand pallet jack as they don t emit carbon monoxide (CO). Carbon Monoxide is a poisonous and odourless gas and can kill a person in around 20 minutes. Be aware of the hazards associated with battery electric machines. The battery is normally a lead acid type and should be treated with caution. It is full of hydrogen gas and sulphuric acid. DATA PLATES AND RATINGS You obviously need to know the products you are moving, how heavy they are, how long they are and what is in them. You will also need to know the height this load is to be lifted to. If the load is not marked with a weight, calculate it, ask your supervisors or you could call the manufacturer. Don t do trial lifts. Also remember to add the weight of the pallet. The Rated Capacity on the data plate tells you how high you can safely lift a load of a certain weight at a certain load centre. PRE START CHECKS These should be done daily or every 8-10 hours and records must be kept. A quick example could be depending on the machine; If there are any faults you need to tag it out, remove the key and report to your supervisor. Tags need to be kept close at hand, usually in the back of the seat. When checking your tyres there are generally 2 types fitted to forklifts, either solid or pneumatic. Pneumatic can be a better ride and quality but if punctured can roll the machine. Fitted to the forklift are several protective guards. These can include overhead guards, foot guards and the back rest / or load guard. POST START CHECKS Once you are happy with your pre-start checks you should now do your post start checks. This normally would be all your hydraulic functions to their limits, lift, tilt, side shift and any attachments. Then you would check your brakes, including inching and hand brake and all of your electrics including lights, horns and beepers and strobe lights, etc. Document Number: TOS-RES-015-03 Date Printed: 12 September 2016 Page 7 of 12
If they are fitted they must work. If anything is wrong during the post start check you need to take the key out, tag it and report it to your supervisor. If it is a minor fault and you feel confident you can fix it, you must first get permission from your employer / supervisor. Electric functions (Lights, Gauges) Audible functions (Horn, Reverse beep) Hydraulic functions (Mast and Steering) Steering functions Braking functions GENERAL OPERATION Remembering people and machinery do not mix. Ensure that no one is next to you in your exclusion zone (3m or more if higher lifts) Ensure travel beepers work and look in direction of travel. When reversing check over both shoulders, monitor the mirrors, you may need a spotter. When travelling up ramps, the load must face upwards to prevent the load sliding off. If you re unloaded you may need to travel with the forks facing downhill to keep weight on the braking wheels. If you decide to turn on a slope this could roll or tip the machine and needs to be avoided and if you need greater lifting capacity you must use manufacturer approved counterweights. No one should be in your exclusion zone as hydraulics can fail or loads can fall. Unsafe loads or dangerous travel can have catastrophic results. Travel heights should be level with the front axle or 150mm (coke can) unless the situation requires otherwise (don t hit floor). 150mm If you do get in rollover situation, stay calm, brace yourself and lean the opposite way. Ensure seatbelts are worn. They stop you jumping out, head butting the overhead guard or from being crushed if you fall out. Stack loads to maintain stability and avoid crushing products. The loads should always be carried against the heel of the fork where possible. The further the load is out, the less the machine can lift. SHUT DOWN Park in a designated area or safe place, lower the forks, apply the hand brake, turn off the machine and isolate the fuel, remove the key. Forklifts should never be parked on hills or slopes. They are very heavy and if they start to roll they can be lethal. You will have to get the wheels chocked before you can exit the machine. Only do this in an emergency or break down situation. Keys should always be removed to restrict access to un-licenced / unauthorised people. This can be site specific. BEING COMPLIANT If you have decided to be trained in your own workplace, to do this legally, you must be enrolled into a course (on a training plan) with an RTO and be under the direct supervision of an operator that holds a current valid licence whenever you are operating the forklift. Direct supervision means being close enough to be able to react in the case of an emergency and prevent an incident happening. Once you have completed your course and received your paper work stating that you are competent, you will have 60 days to apply for your high risk work Licence for the forklift on line with Worksafe Qld. You are automatically covered under Legislation to operate a forklift, for up to 60 days, to give you time to apply. Your High Risk Work licence (HRW) is photographic, nationally recognised and will be renewable every 5 years. You have to keep WHSQ up to date with your current address as renewal notices will be sent out to you every 5 years. Failure to renew this licence within 12 months of expiry can result in you not being able to renew your licence or WH&S may force you to complete another training course which may see you working under the supervision of another licence holder whilst on that course. You should not operate a forklift unless you feel competent, ie: if you have not for a few years and are asked to unload a truck you must be confident and competent. If you don t feel competent you must either: seek further training or hand in your licence. Your employer cannot force you to operate a forklift if you do not feel competent. As a high risk work Licence holder, you have a duty of care to ensure the safety of yourself and others (due diligence). If you are found to be negligent and don t practice your duty of care, you may have your licence cancelled or suspended or you may be prosecuted in a court of law.
