FINANCIAL CONTRACT AND ENROLLMENT AGREEMENT 2016 2017 SEASON Name of Singer: 2016-2017 Choir: Melodia Harmonia Dolce Bella Cantabile Please read and check each section. I/We understand that enrollment is for July 2016 through June 2017, plus tours and summer performances if applicable. I/We are responsible for the entirety of the financial contract and chosen payment plan with the PGC. I/We understand a $25 fee will be assessed for returned checks and NSF charges. I/We understand payments are due the 1 st of the month unless otherwise stated. If paid after the 15 th of the month, a $25 late fee will be assessed. I/We understand that we are responsible for the following additional costs: $50 Registration Fee One-Time $35 Music and Theory Supply Fee Formal and informal uniform costs. Music folder for Bella and Cantabile choirs. Tour assessments (first payment is a non-refundable deposit). Concert tickets to attend performances. Music checked out and not returned, or returned damaged. I/We are responsible for a $325.00 fundraising minimum through the annual PGC authorized fundraiser, understanding any funds not raised through this fundraiser will be our responsibility to pay. I/We are responsible for providing volunteer services for a minimum of 10 hours per family (beginning July 1, 2016 and ending June 30, 2017) I/We are responsible for our daughter s financial commitment and attendance for rehearsals, camps, and PGC public Concert Series. We understand that our singer missing two consecutive summer camps without prior approval by the artistic director is subject to dismissal from the Chorus. I/We are responsible for our daughter s financial commitment and attendance for rehearsals and the PGC public Concert Series. I/We are responsible for our daughter s commitment and attendance for community performances and other performance opportunities, in addition to our public Concert Series, when scheduled. These are important aspects of singer education, community outreach, and visibility for the Chorus. I/We are responsible for our daughter s financial commitment and attendance for tours with her choir (if in Dolce, Bella or Cantabile) starting with a deposit due as early as July and no later than October 1 st. Touring is required. We understand that a request for our daughter to miss a tour has to be submitted in writing to the artistic director. It will be due on or before the annual date announced by the artistic director and no later than October 1 st. We also understand that our daughter missing two consecutive tours is subject to dismissal from the Chorus. I/We understand that PGC family emails, phone numbers, addresses, and communication folders are intended for singers to receive information regarding PGC business only. Unauthorized communications coming from anyone other than the PGC Staff to singers and their families is strictly prohibited. I/We understand that the Permission and Release, the Behavior Policy, the Cell Phone Policy, and the Uniform Policy we acknowledged and signed when our daughter initially joined, continue to apply for the entirety of her time with PGC. We have read and accept the financial information for the 2016-2017 Season. We understand we are responsible for the entirety of this financial contract, even if our daughter does not complete the season. Phoenix Girls Chorus Permission and Release The undersigned states and affirms that I am (we are) either the parent(s) or the legally authorized guardian(s) of the above named girl. I (we) give permission for her to participate with the Phoenix Girls Chorus (PGC) in all events scheduled by the PGC, including traveling on concert tours to various locations within and outside the United States. I (we) hereby give the adult staff and chaperones of the PGC permission to authorize medical treatment for my (our) daughter named above, as they deem necessary or reasonable, without additional permission from me. I (we) agree to assume responsibility for all costs in order to obtain medical care for my (our) daughter. In the event that the adult representatives of the PGC decide that my (our) daughter must leave an event for medical or other unforeseen reasons, I (we) agree to be responsible for any costs related to her return trip home separate from any tour. I (we) understand such a decision is at the discretion of the adult representatives of the PGC, but whom will consult me (us). In return for the PGC allowing my (our) daughter to participate in concerts, practices, rehearsals, tours and other events involving the PGC, I (we) agree to release the PGC, its Board of Directors, officers, employees and agents from all actions, causes of action, damages, claims or demands, which the undersigned or our daughter or any successor may have against the PGC, or such other parties, for all personal injuries, property damage or other loss or damage of any kind, whether or not presently known or contemplated or which may arise out if or relate in any way to a concert, practice, rehearsal, tour or other event of the PGC. This permission and release form will remain in effect until the registered child no longer participates in the PGC or I (we) notify the PGC in writing of the withdrawal of my (our) permission and release. For publicity purposes, media resources may request specific names of singers to list in their articles. They will also request photo opportunities of the singers. By showing photos and listing PGC singers that live in their circulation, they are able to promote community connections and help promote the Phoenix Girls Chorus organization. NO address or contact information will be given. The only information they will receive will be the first & last name of the singer and the city you live in. Singers are also listed in PGC concert programs and in festival programs for the touring choirs. I CONSENT to allow the Phoenix Girls Chorus to release my daughter s name and city of residence to requesting media resources for release in their publications. I give permission for my daughter to be in photo opportunities. I understand that NONE of their personal information will be given and this is voluntary. I DO NOT CONSENT to allow the Phoenix Girls Chorus to release my daughter s name and city of residence to requesting media resources for release in their publications. I understand that my daughter s name will NOT appear in media opportunities promoting the Phoenix Girls Chorus, PGC concert programs, festival programs while touring, and her photo will not be used. I understand my daughter may be kept out of performances for this reason. By checking either box, the guardian/parent acknowledges having read this release themselves and has related the understanding all of its terms and their significance to his/her singer, and states that this release is signed and delivered voluntarily.

Behavior Policy for Phoenix Girls Chorus Singers Being in the Phoenix Girls Chorus means much more than the positive benefits singers receive by achieving excellence in vocal and choral performance. It is also a training ground for helping to develop responsible, well-behaved young women as fine representatives of this organization and future leaders of our community. In order to maintain consistently high standards, the following behavior guidelines will be adhered to by all singers, and supported by staff, parents, guardians, and choir families. PGC Singers will: Listen and follow the instructions of the Phoenix Girls Chorus staff and parent volunteers assigned to supervise them, during rehearsals, at concerts, on tour, at camp and all other choir-related activities. Respond in a polite manner when spoken to, especially by concert attendees, special guests and supporters at choir functions, PGC staff and volunteers. Cooperate with PGC staff, volunteers and other designated supervisors by following the specific instructions given for her choir during PGC functions. Be respectful of all adults. Protect voice by avoiding shouting, screaming, or excessive use. Refrain from cursing, profane language, religious or ethnic slurs, inappropriate talk or actions. Observe Voice Rest when instructed to do so. Refrain from being disrespectful to other individuals including pestering, teasing, or excessive touching of other singers. Avoid running, jumping, shouting or being in unauthorized areas of any designated venue for any PGC function including but not limited to rehearsals, concerts, tours and camp. Observe quiet zones posted at each venue. Be respectful of property belonging to the PGC or to other persons. Remain at the facility of any PGC function until dismissal time, unless excused by a PGC staff member. Show persistent effort towards the goals of the PGC and singers choir level. Present a positive attitude during all PGC-related activities. Exhibit reliability by preparedness, punctuality, and team spirit. Show pride in appearance and presentation when representing the Phoenix Girls Chorus. Be courteous during social interaction. Practice safety for oneself as well as for others through caution in interaction with strangers and in all situations. Singers are expected to maintain respectable behavior: During rehearsals While in and around the choir building During performances, including during the backstage timeframes During choir-related functions While on tour and staying with HomeStays While representing PGC during the fun!raiser Any time they are dressed in their PGC uniform Disciplinary Actions for unacceptable behavior will be addressed as follows: A three reprimand system will be used during all PGC functions including but not limited to, rehearsals, concerts, tours and camp. Upon receiving a second reprimand points will be deducted. Upon receiving a third reprimand or continuing any disruptive behavior, the singer will be removed from the performance or activity by decision of the Director or designated staff person. Please note if a singer is not allowed to perform or participate, she will remain in a designated, supervised area for the remainder of the time period. She will also receive a warning letter as outlined below. At the discretion of the staff, a warning letter may be issued without the three reprimand system anytime behavior against another singer, staff member or volunteer is considered unacceptable. A conference may also be called. If a singer has received two warning letters, the next unacceptable incident will result in immediate dismissal from the Phoenix Girls Chorus. Dismissal from the choir for improper behavior will NOT nullify contractual and/or financial obligations. PARENT/GUARDIAN: I have read and understood these guidelines and my singer has read and understood these guidelines. Not abiding by these rules will have serious consequences which can include being sent home from camp or tour at parents /guardians expense and expulsion from the Phoenix Girls Chorus. It is understood by acknowledging this policy, you and your singer will abide by these guidelines during her entire tenure with the Phoenix Girls Chorus.

PHOENIX GIRLS CHORUS CELL PHONE POLICY By acknowledging this policy, you are NOT stating that your daughter HAS a cell phone. You are stating that you and your daughter UNDERSTAND the cell phone policy. Cell phone use is not permitted in choir rehearsal, theory class, sight singing class, performances, backstage, at camp, on tour or during any other choral activity without the express permission of the Artistic Director, Staff, Rehearsal Parents or Adult Chaperones. We understand that PGC singers may have cell phones in their possession, but phones should be turned OFF (not set on vibrate or silent) and must remain stored away for the duration of choral activities. In the event of an emergency when a parent needs to contact their daughter, the parent should call the PGC office and a message will be delivered promptly to the PGC singer. Texting, e-mailing, checking time, or the many other tasks that can be completed on cell phones are not permitted during PGC activities (i.e., if a singer needs to see the time, she should bring a watch or ask someone who has a watch. If she needs to take pictures, she should bring a camera.) If the Artistic Director, Staff, Rehearsal Parent or other Adult Volunteer SEES a cell phone, it will be confiscated. 1. Following a first offense, a singer will be allowed to pick-up her cell phone at the conclusion of the choral activity and a report will be placed in her file. 2. A subsequent offense will result in the singer s cell phone being held at the PGC office until a parent comes in to pick it up. This means that after one offense, if a PGC singer drives herself or is in a carpool and her phone is taken from her, she will leave without her phone and her parent will need to drive to the PGC office to retrieve it. Cell phones are permissible before or after rehearsals or before or after other choral activities. Phones should remain stored away for the entire PGC activity, even when moving between activities (i.e., between rehearsal, theory class, and sight singing, or between a dress rehearsal and a performance.) Both parent/guardian AND singer have read, understand, and agree to comply with the cell phone policy. We understand by acknowledging this policy, we will abide by these guidelines during her entire tenure with the Phoenix Girls Chorus.

Phoenix Girls Chorus Uniform Policy The Phoenix Girls Chorus is known for its professional image and polished look. The uniform process is an integral part of maintaining and upholding this image. To accomplish this, there are certain requirements that need to be acknowledged and followed. Families are responsible for the purchase of two uniforms not included in tuition total (formal and informal) which will be checked periodically throughout the year during uniform fittings, checks and rechecks. The Formal Uniform consists of specific shoes for each choir, black tights/off black knee highs, slip and a formal dress (including jacket for Cantabile) measured to fit each singer. These items will be purchased as pre-owned, from a choir seamstress or PGC directly. The informal uniform consists of the same shoes mentioned above, black socks/black knee highs black yoga pants (purchased outside of PGC), and a Pink PGC logo T-Shirt (purchased from PGC). For returning singers with fun!raiser credits, verified remaining credits may be applied towards purchase of new uniform items sold by PGC. Mandatory uniform initial fittings, checks and rechecks will take place on several Saturdays throughout the season to ensure uniformity with all choirs. This is done to keep up with the growth of the singers and alter the uniforms accordingly. Dates for all mandatory initial fittings, checks and rechecks are located on the PGC calendar for the entire year. Failure to attend or communicate a schedule conflict with the uniform committee for any uniform initial fittings, checks and rechecks will result in point deduction and singer not performing in the next scheduled concert. A detailed list of ALL uniform requirements/guidelines are available in the PGC office and on the family website Uniform page. By acknowledging this policy, it is understood that both parent/guardian AND singer have read, understand, and agree to comply with the uniform policy. We understand by acknowledging this policy, we will abide by these guidelines during her entire tenure with the Phoenix Girls Chorus and will attend the mandatory uniform fitting/checks/rechecks, communicate with the uniform committee and follow the PGC uniform requirements.
