Sagitta EVL 1 Cutting the neck of the femur The level of the cut of the femoral neck is determined in pre operative planning using X-ray templates. The anatomic landmarks are confirmed intra-operatively. The cut is at to the anatomical axis of the femur. Preparing the greater trochanter and the metaphysis The opening of the femoral canal and preparation of the greater trochanter can be initiated with a small rasp. It can be necessary to rasp the inside of the greater trochanter in order to avoid placing the broaches and implant in varus. «Femur cut» Broaching Connect the broach to the appropriate impactor and progressively impact the cancellous bone up to the neck cut. The definitive size is reached when the broach is stable in torsion and well seated.
SURGICAL TECHNIQUE Reaming for insertion of collared stem Present the powered reamer and ream over the broach in place until impingement of the reamer on the broach. This operation results in a perfect line between the femoral surface and the upper surface of the broach. The calcar reamer smoothes the surface of the calcar for insertion of stems with collar. Sagitta EVL 6 Definitive stem implantation Cemented version The stem is introduced progressively into the cement mantle by hand pressure using the constrained impactor. This impactor when seated in the stem, allows adjustment of orientation, anteversion can be visualised with the positioning rod. Holding handle Cementless version The stem is placed into the femoral canal. Final impaction is achieved when the stem coating is flush with the level of the cut. Impaction marking 7 Femoral heads Trials on the implant It is possible to carry out trial reductions with trial heads. Definitive head assembly Clean and dry the morse taper. Place the definitive head on the morse taper, and impact it with a sharp hammer blow on the impactor, which is also used to reduce the articulation. Head Impactor 6
SURGICAL TECHNIQUE Optional instruments Head extraction Place the extraction connector over the implant head and screw down until in contact on the head. Connect the slap hammer to the end of the connector and gradually slap the head out. Stem extraction After removing the head, connect the stem connector to the male taper on the collar. This connection must be securely fastened by tightening the lateral nut with the end of the slap hammer. The position of the lateral screw can be reversed according to the side on which the operation is performed. *The extraction kit is available on demand.
