Failed students from other faculties are rarely considered for readmission to the Faculty of Engineering and Computer Science. Once a student is in good standing, he/she may apply for a degree transfer through the Office of the Registrar. 2. Failed students from Engineering and Computer Science who wish to apply for readmission into the faculty must complete this form. Guidelines for readmission to other faculties are available in Birks Student Service Centre. 3. The decision to readmit failed students is a Faculty decision and is considered a privilege and not a right of students who have been unsuccessful in their initial admission to the University. Readmission is based on the information you provide on this form and should demonstrate that you have the potential to succeed if readmitted. For this reason, it is in your best interest to ensure that all information provided is clear and supporting documents are attached to this form. 4. Any student readmitted to the Faculty will be required: a. repeat all D grades ( in the case of technical electives, an alternative technical elective course may be chosen by the student and obtain a grade of C- or better); b. must successfully complete all courses required to repeat prior to further registration in other courses; c. must return to acceptable standing at the time of their next assessment; 5. Students readmitted to the Faculty must register for all failed courses (and if relevant, outstanding prerequisite courses) in their first year of readmission. Failure to do so may mean withdrawal from your courses and the cancellation of your readmission. 6. Students readmitted to the Faculty are responsible for knowing the University deadlines for discontinuing courses, applying for deferred exams, etc. No special arrangements will be made should students fail to respect these deadlines and/or follow the outlined procedures upon readmission. 7. Course(s) taken at other institutions while in failed standing will not be considered for the credit at Concordia University. 8. Should a student be readmitted to the Faculty of Engineering and Computer Science he/she will be required to sign a letter of readmission indicating that he/she is aware of the above guidelines and will comply with them if readmitted to the Faculty. 9. No information regarding your request for readmission will be disclosed to a third party unless the applicant has provided this consent on the application form (page 4). 10. Students who have been assesses in failed standing for the first time must complete the Student Readmission Agreement that is attached to this application for readmission form. STUDENT READMISSION AGREEMENT (the Agreement ) ( to be completed by students in failed standing for the first time only ) Student Name: Student Number: Program: Date: Introduction This Agreement outlines the requirements and responsibilities for students who have been assessed in failed standing for the first time and intend to apply for readmission. In order to be readmitted to Concordia, either in a Faculty program or as an Independent, the student must fulfill all imposed requirements and timelines. The requirements include but are not restricted to the following: Download and view the Readmission Information video at http://www.concordia.ca/extendedlearning/advising/students-in-failed-standing/readmission-requirements.html Completion of the Learning and Study Skills Inventory (LASSI) test. Information about the LASSI, as well as the required username and password, is found in the Readmission Information video. Once you have completed the LASSI, print a copy of the results. If readmitted, full compliance with all conditions stipulated in the letter of readmission Completion of any and all other assessments and requirements prescribed by the Faculty. Academic Advising and Assessment If readmitted, and in addition to the conditions stipulated in the letter of readmission, students may be required, in consultation with his/her academic advisor, to complete assessments in the following possible areas: learning and study skills, writing skills, quantitative skills (math and statistics), and personal management. Outcomes The primary objective of this readmission process is that, with his/her full and active participation, the student will: gain a better understanding of the path he/she needs to follow to achieve his/her academic goals improve his/her skills in areas identified through assessments develop strategies for future academic success Understand and access ongoing support as needed 5
Terms and Conditions for Continuing to Be Enrolled at Concordia University Readmitted students must meet all of the following requirements to remain in a faculty program or to continue as an Independent student after having been in failed standing: Satisfactory academic performance, as defined in the applicable section of the Undergraduate Calendar. Attendance of the Readmission Workshop. Successful completion of the Skills for Success at University Study course, if prescribed Timely completion of all other academic assessments/requirements prescribed by an academic advisor, and compliance with all conditions set in the student s letter of readmission. If a personal or academic situation arises which could interfere or affect a readmitted student s studies or progress (e.g., poor mid-term grade, increased part-time work load), the readmitted student is required to contact his/her academic advisor in his/her Faculty as soon as possible. Readmitted students must attend all required appointments and meetings. Any occurrences of lateness, no-shows, and/or last minute cancellations of appointments will weigh against the student in any future request for consideration and/or readmission decisions. The student understands and recognizes that all information gathered by any department within Concordia University which is relevant to a student s compliance with the terms of his/her readmission will be shared with the appropriate administrator or advisor. The following conditions also apply to this Agreement: Completion of the learning skills assessment does not guarantee readmission to the student s current program at Concordia. A student who is not meeting all of the requirements outlined in this Student Readmission Agreement may have his/her readmission revoked.
