Please type or print your entries. This form may be filled in on the computer. 3. When this form is completed, submit it in person to the Human Resources Office, 33 Gough Street. Applications are due by the close of business on September 8, 2016. The staff in that office will issue you a receipt. You will receive notice of the results of your application by October 13, 2016. AGREEMENT 1. I have read, understand and accept the conditions set forth for accepting a sabbatical leave. I have attached a completed sabbatical leave proposal form. 2. I understand that if I am awarded a one-year or one-year split-semester sabbatical, that I shall receive sixty percent (60%) of my full salary for such period(s). In particular, if I am awarded a split-semester sabbatical, my salary for the academic year will not be averaged, but will switch from 100% of my full salary on the non-sabbatical semester to 60% of my full salary for the sabbatical semester. 3. If I have indicated a preference for a two non-consecutive semester (split) sabbatical, but have not indicated second semester preference, I understand that I must submit an application for the second half of my sabbatical during the Fall application period immediately following return from the first half of a split, or no later than the Fall application period an academic year in advance of the second half of the split. (Article 17.L.4.1.d) 4. I understand if a one-year sabbatical leave is granted, no switching of the beginning semester is allowed. (Article 17.L.4.1.g) 5. I understand that if a one-semester sabbatical is granted, no switching of the semester is allowed. (Article 17.L.4.1.f) 6. I understand that within 60 days of my return to duty, I must submit documentation to the Human Resources Department, in accordance with the type of sabbatical leave and AFT contract, Articles 17.L.7. 7. I understand that upon my return, I must serve at a full load upon my return from sabbatical (two years of service after a one year sabbatical; one year of service after a one semester sabbatical). (Article 17.L.8) 8. I understand that any changes to my proposed sabbatical plans must be approved by the Sabbatical Review Committee in advance of the applicable sabbatical term. Your sabbatical report/project/transcript(s), as defined in this proposal and any subsequent modifications approved by the Sabbatical Committee, will be used to evaluate all sabbatical materials submitted upon completion of your sabbatical leave. 1. STUDY SABBATICAL 1.1 Listing of Coursework Below, list the courses you intend to complete while on sabbatical leave. All coursework used to fulfill your sabbatical obligation must be offered by an accredited institution of higher learning and you must receive either a satisfactory letter grade or credit for the coursework completed. Upon completion of your sabbatical leave, an official transcript must be submitted to the Office of Human Resources. Any changes or substitutions in coursework used to fulfill your sabbatical obligation must be reviewed and approved by the Sabbatical Committee prior to enrollment. 1.2 Listing of Alternative Coursework In the space below, please list your alternative coursework. This coursework is to be completed in the event you cannot complete the coursework listed above, and may be taken without further consideration by the Sabbatical Review Committee. If you are applying for a one semester sabbatical leave, please list at least two additional courses. If you are applying for a one-year or one year split sabbatical, please list at least four additional courses. 2. RESEARCH SABBATICAL (Project) Below please indicate the subject and nature of your research sabbatical. Indicate specifically the focus of your research and the type of research (e.g. survey, archival, observational, etc.) you intend to conduct. 2.1 Draft Table of Contents Please attach a draft table of contents for your research project. A draft table of contents will enable the Sabbatical Committee to better understand your proposed research and provide clarification regarding the report you will submit upon completion of your sabbatical. 2.2 Research Project Rationale Please indicate how your research project will benefit your students, department, and the college. 3. TRAVEL SABBATICAL (Report) Below please indicate the destination(s) you intend to visit while on sabbatical inclusive of the dates you will be at the stated destination(s). Upon completion, your submitted sabbatical travel report should include a travel itinerary and a detailed description of your travel activities (e.g. photos or a journal). Please include photocopies of your airline tickets and lodging receipts if applicable.
