SUPPLIER QUALITY SELF AUDIT & QUESTIONNAIRE FORM The purpose of this questionnaire is to provide a general understanding of the nature of the supplier s quality organization and approach to quality. The responses to these questions should be provided by the facility that will be supplying the product. Please answer each question by typing, or printing in ink, an X in the appropriate box, or by filling in the blanks. Return the completed questionnaire to Fuji Autotech USA, LLC.
