Page 1 of 7 Introduction This document is for use assisting with Part II Section B of F101 Application for Accreditation: ISO/IEC 17025. Note 1: Requirement documents contain requirements in addition to ISO/IEC 17025. In some cases the documents relate to a specific program offered by A2LA. Laboratory participation in these programs is voluntary. In cases where a requirement document is referenced, this document may be obtained from our website at www.a2la.org. Note 2: A Selection List is a list that identifies the specific tests/calibrations and types of tests/calibrations for which you are seeking accreditation. The list should be prepared in enough detail so that the desired scope of accreditation for each field selected can be properly determined and presented on the A2LA web site. In cases where a selection list is referenced this list may be obtained from our website at www.a2la.org. Either a completed selection list or draft scope of accreditation must be returned with this application. If a draft scope of accreditation is provided an electronic version in word format is preferred. For environmental laboratories, scope templates have been created in lieu of selection lists. These can be found on the A2LA website. See also Section II and V. Note 3: A Specific Checklist is a checklist that incorporates information from a requirement document for the purpose of evaluation and documentation of those requirements during an assessment. If the applicant elects to participate in a field or sub-field where a checklist is noted, that checklist must be obtained, completed and submitted with this application. Page 2 of 7 Veterinary Testing This program is designed to meet the needs of users looking for quality veterinary testing services, and has incorporated the requirements as established by the OIE (World Organization for Animal Health Quality Standard and Guidelines for Veterinary : Infectious Diseases, Edition 2, 2008). The scope of this Program includes evaluation of diagnostic and clinical veterinary laboratories and animal hospitals whose activities include commercial, government, academic testing, international diagnostic testing for infectious diseases, and screening for growth promoters and drug/chemical residue that includes antibiotics, antihelmethetics, pesticides, metals, organics, DNA/RNA, and GMO. This includes testing performed on items where the results of that testing will be used in a civil or criminal proceeding. Checklist Needed: C221 Specific Checklist: Forensic Accreditation Program (Testing) Geotechnical Includes tests of soil and rock to provide engineering data. This program is designed to meet the needs of users looking to become accredited as a competent geotechnical testing services provider. Internationally accepted accreditation criteria and standards developed by ASTM committees form the basis for accrediting laboratories under this field. The scope of this program is suited specifically for users in Texas. Users in other States or parts of the U.S.A. may seek to include a different mix of required tests and this will be so noted on the scope of accreditation for laboratories which seek accreditation to meet the needs of that user. The scope of this field includes the following materials areas: soils, soil-cement, rock, peat materials and geotextiles. Applications for accreditation may be made for several tests in each area. Additional areas may be added upon request. The requirements of this program are not applicable to the Putting Green Materials Testing Program.
