About Papworth Hospital Papworth Hospital is the UK s largest provider of specialist cardiothoracic services including cardiology, respiratory medicine and cardiothoracic surgery, and is the country s main heart and lung transplant centre. The hospital first opened in 1917 as a tuberculosis hospital and when Pendrill Varrier-Jones conceived the idea, he wanted to create not just a hospital, but a community. This sense of community remains, but our work has grown over the years. During the 1950s, Papworth became one of the region s main hospitals, developing first thoracic surgery, then cardiac surgery and then cardiology. The hospital has a history of pioneering ground-breaking procedures. It was the site of the UK s first successful heart transplant in 1979, and in 2006 Papworth again took the lead with the country s first beating heart transplant. Today, we are a Foundation Trust providing a comprehensive range of specialist services for adults. These services are supported by highly effective and well-equipped: Radiology, Pathology, Immunology, Theatres, Critical Care and Anaesthetics Directorates. Though historically Papworth has operated as a specialist tertiary centre receiving the majority of referrals from clinicians at other hospitals; increasingly we are developing services that are also directly accessible by GPs through Choose and Book. These include Papworth Direct, our brand new state-of-the-art cardiothoracic diagnostic centre and our Respiratory Support and Sleep Centre. Papworth s Direct Access Services The following services are directly accessible to GPs and hospital clinicians: Papworth Direct Papworth Direct, which encompasses the new cardiothoracic diagnostic centre, will provide patients with quicker access to diagnosis and shorter pathways to treatment as required. It also provides better value for commissioners. This facility enables GPs to refer patients with suspected heart disease and specific respiratory conditions direct to our state-of-the-art diagnostic centre. The centre provides access to a comprehensive range of tests, including the country s first dual source 64-slice cardiac CT scanner. Patients can then be referred on, if necessary, for treatment by our specialist teams. Further information about this service can be found on the website at www.papworthdirect.nhs.uk. Respiratory Support and Sleep Centre Our sleep medicine service is the largest in the UK, treating more than 4,000 patients a year, and a major extension has just been completed. All types of sleep disorders are treated, both respiratory and non-respiratory, and we have several sleep clinics each week with a multidisciplinary input. Quick Access Chest Clinic The Quick Access Chest Clinic is for patients with chest problems such as coughing up blood or worsening breathlessness.

Cardiac Services Papworth Hospital provides a specialist service for cardiac surgery and cardiology. Our expertise covers the full range of invasive and non-invasive treatments for cardiac surgery, cardiology, and electrophysiology. We provide access to the very latest in cardiovascular diagnostic equipment. Further information is available on the website at. Cardiac Surgery Papworth is one of the UK s leading and largest specialist providers of cardiac surgery including Coronary Artery Bypass Grafting (CABG) and valve repair/replacement. CABG surgery is the definitive treatment for coronary artery disease. Papworth Hospital has a lower complication rate than other peer group hospitals in this area of surgery. Papworth is the UK s largest valve repair/replacement centre and is leading the field in deploying innovative methods of repairing heart valves. Electrophysiology and Devices Electrophysiology (EP) is a rapidly developing cardiology sub-specialty. Research into cardiac rhythm disturbances and sudden cardiac death is increasingly improving treatments available particularly in the area of abnormal rhythm ablation. Papworth is one of the UK s leading centres for EP and ablation using the latest 3D mapping equipment and activity is forecast to increase significantly during the next few years. Papworth is also a leading centre for device implantation including pacemakers, biventricular pacemakers and implantable defibrillators. The majority of patients that have pacemakers or defibrillators implanted have technical follow-up, performed by a Cardiac Physiologist. This service is provided through outreach clinics provided within various hospitals within the region. Diagnostic and Interventional Cardiology Papworth offers a wide range of non-invasive and invasive diagnostic procedures as well as interventional procedures for coronary disease, valve disorders, structural heart disorders, and arrhythmias. The department has access to a full range of diagnostic procedures within the cardiology and radiology departments including nuclear medicine, the latest dual source 64-slice CT scanner for imaging coronary arteries and MRI. Papworth is one of the UK s leading centres in coronary angiography, angioplasty and stenting procedures with a lower complication rate than other peer group hospitals. Papworth has a well established noncoronary interventional programme for procedures such as mitral balloon valvuloplasty, closure of septal defects and is one of the leading centres in the development of less invasive methods of valve replacement/repair. Cardiac Technology The Department of Cardiac Technology offers a wide range of investigations for patients with suspected cardiac problems. These include ECG, exercise tests, 24/48 hour ECG recordings, echocardiography, and tilt tests.

Thoracic Services Papworth Hospital s comprehensive thoracic services provide expertise in cystic fibrosis, lung defence, thoracic oncology, thoracic surgery, interstitial lung disease, immunology and the treatment of pulmonary vascular diseases. It also has the UK's largest Respiratory Support and Sleep Centre and a Quick Access Chest Clinic. Respiratory Infection, Inflammation & Immunology (RIII) Papworth s RIII service provides a regional and national centre for patients with recurrent respiratory infections. Cystic Fibrosis Our regional centre cares for patients from the age of 16 in outpatient clinics or on the ward. A well-established home-based intravenous therapy service is also provided. The centre has gained a national and international reputation for its contribution to research and is regularly involved in multi-centre clinical trials. Lung Defence Clinic This is a specialist clinic for patients with recurrent chest infections, many of whom have existing lung damage. The current practice is to train patients for home care therapy whenever possible and we have developed a comprehensive home therapy training programme. Thoracic Oncology This is a quick access service for patients referred from DGH respiratory clinics with thoracic malignancy, run in partnership with Addenbrooke s Hospital as a joint Cancer Centre. The emphasis at Papworth is on diagnosis, staging, surgery, and management of pulmonary complications of thoracic malignancy. A cryotherapy service has also been established on the Papworth site. Most investigations take place during a single visit. Treatment may include surgery at Papworth, or radiotherapy or chemotherapy at Addenbrooke s Hospital. Thoracic Surgery The thoracic surgeons play an integral role in the oncology service. They are key to the management of lung cancer and to the research efforts in mesothelioma. They also offer a full range of thoracic interventions for patients with benign thoracic disease such as correction of pectus deformity and lung volume reduction for selected COPD patients. Interstitial Lung Disease Tests are carried out on the Thoracic Day Ward. A multi-disciplinary team then discuss outcomes to confirm diagnosis and formulate a management plan. Pulmonary Vascular Disease Unit (PVDU) The Pulmonary Vascular Disease Unit at Papworth Hospital is one of the UK s leading specialist centres for the diagnosis and treatment of Pulmonary Hypertension. Although Pulmonary Hypertension remains an uncommon condition, its incidence is increasing. Papworth Hospital has been designated as one of six specialist treatment centres by the National Commissioning Group. Papworth s Pulmonary Vascular Disease Unit is unique in its ability to offer a spectrum of medical therapy and surgical treatment under one roof. It is the only specialist centre in the UK that is designated for the pulmonary endarterectomy procedure where scar tissue that is causing blockage in the pulmonary arteries is removed as a potential cure.

Respiratory Support and Sleep Centre (RSSC) Referrals are received from GPs and hospital doctors from all over the UK, for sleep disorders such as obstructive sleep apnoeas, narcolepsy and unusual behaviours during sleep. Additional services at the RSSC include: Progressive care programme for patients failing to wean from invasive ventilation despite conventional therapy Domiciliary ventilation for more than 1,000 patients with chronic respiratory failure Assessment of specialist oxygen systems, particularly portable systems for use outside the home and when oxygen is required in conjunction with home ventilatory support Respiratory Physiology Laboratory The unit provides facilities for the most advanced diagnostic methods used in Respiratory Function Measurement and is also available for research work. The laboratory has a training programme for technicians run in association with Anglia Ruskin University. Quick Access Chest Clinic (QACC) Patients of the Quick Access Chest Clinic benefit from same-day testing and diagnosis in many cases, and the availability of specialist support services including radiology and respiratory physiology. Transplant Services Papworth is the UK s main heart and lung transplantation centre. Performing the UK s first successful heart transplant in 1979 and pioneering the first transplant of a beating heart in 2006, Papworth Hospital has established global renown as a clinical and research centre of excellence. Following referral, the transplant team will decide if the patient is suitable for formal assessment. Patients are contacted by the transplant team prior to admission, which is arranged for them and a family member. A range of planned tests is carried out over a two-three day period and the process concludes with a consultation with the transplant team and the appropriate options discussed. Whatever the operation, all patients are followed up by the team in the Transplant Continuing Care Unit. Medical care is co-ordinated between the transplant team and the patient s GP, who provides prescriptions for the range of medications necessary for post-transplant life. Telephone contact is available for the patient, their family and their GP at any time. Allied to its expertise in transplantation, Papworth is one of just three designated hospitals in the UK that performs Ventricular Assist Device (VAD) surgery. VADs are essentially artificial hearts that are used to support very sick patients with heart failure. Pathology Papworth Hospital s Pathology Department offers a full spectrum of pathology support to all departments of the hospital. The department is committed to quality and fully CPA accredited. It offers a flexible, seamless service to all of the hospital s departments. Radiology The Radiology Directorate provides a timely and highly efficient service using state-of-the-art imaging technology, to support the hospital s departments. Direct access to a range of imaging tests is also available to GPs and hospital clinicians from other trusts via Papworth Direct.

TCCA The Theatres, Critical Care and Anaesthesia Directorate (TCCA) supports all surgical activity across the hospital. There are five operating theatres with adjoining anaesthetic rooms within the surgical unit, each equipped to carry out the full range of cardiac, thoracic and transplant operations. Papworth s twenty-five bedded critical care unit can provide Intensive Care or High Dependency care depending upon the patient s needs. Most patients are received into critical care immediately following cardiothoracic surgery, but admissions also come from other parts of the hospital or from outside the Trust. Research Programmes Research activity at Papworth is organised into a Cardiac and a Thoracic research programme. An important component of our activity is the assessment of new technologies and methods of service delivery, often in multicentre trials which involve multidisciplinary collaborations. Our main academic partners are the Medical Research Council Biostatistics Unit in Cambridge, the Health Economics Research Group at Brunel University and divisions of the Departments of Medicine and Surgery at the University of Cambridge. All researchers at Papworth are appreciative that research activity depends upon active involvement of our patients. We have a long working relationship with many patient interest groups and fully support the principles of the INVOLVE Programme, for example, the Involve Guidelines for Public involvement in research are disseminated to all new researchers as a matter of routine. Patient Advice and Liaison Service (PALS) Papworth has an excellent PALS service for patients, relatives and carers. The confidential service acts independently of the hospital when handling patient and family concerns, liaising with staff, managers and, where appropriate, relevant organisations, to negotiate immediate or prompt solutions. If necessary, PALS can also refer patients and families to specific local or national-based support agencies. Access to Our Services Referrals to our services can be made via letter or fax and our direct-access outpatient services can now be booked through Choose and Book. This means that GPs can now refer patients to Papworth Direct, our Respiratory Support and Sleep Centre and Quick Access Chest Clinic services via the Choose and Book system. Initially we are deploying an Indirectly Bookable Choose and Book solution whereby once the GP decides to refer, the patient will need to call Papworth s booking centre in order to confirm a booking. GPs will be able to make direct electronic bookings to Papworth from May 2008.
