Kaiser Permanente Providers As a Multi-Choice POS member, you can choose to receive care with a low copayment from Kaiser Permanente Providers. Kaiser Permanente Providers include physicians practicing in our Medical Centers and contracted privatepractice Physicians practicing in their own offices throughout metro-atlanta. The Kaiser Permanente Provider benefit level helps you control your out-ofpocket costs. You normally pay the lowest amount on this plan by using Kaiser Permanente Providers. And, you don t have to worry about submitting any claims for reimbursement. 15

Frequently Asked Questions Doctors Doctors Why is choosing a personal physician important? When you enroll in Kaiser Permanente, the first step to receiving care is the selection of your Kaiser Permanente Provider level personal physician from the physicians practicing at Kaiser Permanente Medical Centers or from our network of contracted Physicians practicing in offices all over town. At this level, your personal physician will be responsible for providing you with routine medical care and for coordinating care you receive in the hospital or from specialists. That s why we require you to choose a Kaiser Permanente Provider when you enroll in our Multi-Choice POS plan. Every member of your family should have his or her own personal physician. Because this relationship is so important to your overall care under the Kaiser Permanente Provider benefit level, if you do not select a Kaiser Permanente Provider level personal physician upon enrollment, we will assist you by identifying a doctor in a medical office near your home. That doctor will be listed in our records as your Kaiser Permanente Provider level personal physician until you make a selection and inform us of your decision. You may change your personal physician at any time by calling our Member Services Department (404) 261-2590. Do I have to select personal physician? Yes. Even if you plan to receive care from Participating or Nonparticipating Providers you still should select a personal physician from the Kaiser Permanente Provider level so you can access your Kaiser Permanente Provider level benefits at anytime. Who can I choose as a Kaiser Permanente personal physician? You have two options. You can select your physician from: The Southeast Permanente Medical Group (TSPMG) Physicians who practice in our medical centers. If you select a physician in one of our medical centers, you ll get: The time-saving convenience of receiving most of your medical services in one location. In addition to the physicians offices, most of our medical centers include pharmacy, lab, and X-ray services. Award-winning doctors (recipient of the American Medical Group Association s 2002 Preeminence Award for exceptional leadership, high quality care, and excellent service). A physician who practices in a group that cares predominantly for Kaiser Permanente members. Access to the hospitals and specialists to whom your physician refers. The support of the full team that works with your physician, including nurse practitioners, physician assistants, and nursing staff. Health Line a single telephone number through which you can make appointments and access a variety of other health services. A firm commitment from the physicians, nurses, pharmacists, and other staff members in our medical centers to provide you and your family with quality care and excellent service. In fact, each of our centers firmly stands by our Quality Service Guarantee. If, at any time, you re not completely satisfied with the service you receive in our centers, we ll refund any applicable copay. Little to no paperwork. Or you may select your physician from: Physicians who contract with TSPMG and practice in their own offices all over town. If you select an Physician, you ll get: A physician who practices in his or her own medical office quite possibly the same family doctor you have now Access to that physician s office and the hospital and specialists to whom that physician refers. You ll need to call his or her office to make an appointment. The Kaiser Permanente Providers available to be selected as a personal physician are located on page 51 through 74. 16

Frequently Asked Questions Doctors Do Kaiser Permanente Medical Center physicians close their practices to new patients? Our goal is to give you flexibility and choice in selecting your personal physician. We also want to make certain that the personal physician you choose is available to provide care when you need it. Therefore, a few physicians with very large practices may, on occasion, not be accepting new patients. By taking these steps, we help ensure that you have easy access to a physician who knows you and is available to meet your needs. However, a few Affiliated Community Physicians may close their practices to new patients from time to time. Those Physicians with closed panels will be designated in this directory as not accepting new patients. How do I tell you who I ve chosen as my Kaiser Permanente Provider? When completing the Enrollment Application and Change Form, please write the in-plan physician s ID number in the designated space. Physician ID numbers are listed in this section under each primary care physician s name in this directory. Remember to select a physician for yourself and each family member. You can select a different personal physician for each member of your family, if you wish. If enrolling as part of an employer group, please also complete any other form(s) required by your employer. Some employers may have enrollment methods other than our Enrollment Application and Change Form. If so, make sure you include the physician ID number(s). What should I do if I want to go to a Kaiser Permanente Provider other than the personal physician I have chosen or that has been designated for me? Your Kaiser Permanente personal physician is responsible for coordinating your care under the Kaiser Permanente Provider benefit level. If you would like to see a different Kaiser Permanente Provider, and have selected one of our Physicians as your personal physician, your care will be provided by that doctor or another who practices in the same office. If you wish to be treated by a doctor at a Kaiser Permanente Medical Center or by another Physician, you should select that individual as your new personal physician before scheduling treatment. If you have a Kaiser Permanente Provider as your personal physician, you do not need to change your physician choice if you simply want to see another physician at a different Kaiser Permanente Medical Center. Basically, if you want to see another doctor in a different group of offices, please call us first so that we can change your physician selection in our system. That way you will avoid any confusion. It is important to remember that switching to a new personal physician may also change the physicians available to you for specialty care. How do I change my personal physician? It s very simple. You may change your physician at any time and as often as you wish. Simply call our Member Services Department at (404) 261-2590. You also may change your personal physician when visiting one of our medical centers (just tell a member of your health care team), or when you call our Health Line. Make sure you have your Kaiser Permanente health record number handy. How does Kaiser Permanente ensure quality practitioners? Kaiser Permanente s credentialing process is very rigorous. All practitioners are reviewed and approved by the Credentials Committee before they are selected to participate in our medical care program. Review and verification of practitioner credentials include 17

Continuous quality reviews are conducted on an ongoing basis. Members are also asked to evaluate their practitioners regularly through satisfaction surveys. How do I know I m choosing a health plan that s committed to quality? Our commitment to quality shows. Here are just a few of the honors we re proud to have earned: National Committee for Quality Assurance (NCQA): In 2001, and for the second year in a row, the Kaiser Permanente health plan received Excellent Accreditation the highest level of accreditation possible for both HMO and Medicare plans. NCQA is an independent organization that monitors health plan quality. The American Medical Group Association (AMGA): The Southeast Permanente Medical Group, Inc. (TSPMG) received the American Medical Group Association s 2002 Preeminence Award. When selecting a Kaiser Permanente Provider keep in mind that your physician choice will determine which specialists are available to you. Your personal physician has an established relationship with a specific group of specialty care doctors with whom he or she works and trusts. By referring only to a certain group of specialists, your physician is better able to ensure that you receive quality care. You must get a referral from your personal physician in order to see a specialist. If you choose a Medical Group physician as your personal physician, your physician will refer you to Medical Group specialists or specific community specialists. If you choose an Physician, your physician will refer you to an Affiliated Community Specialist. Are there certain services that do not require a referral from a personal physician? Appointments with obstetricians/ gynecologists, dermatologists, psychiatrists, ophthalmology, and behavioral health specialists (these are self-referral specialists) do not require a referral from your personal physician. Because your personal physician also has a relationship with a select group of self-referral specialists, we encourage you to see only those self-referral specialists affiliated with your personal physician. If you choose a Medical Group physician as your personal physician, you may self-refer to any Medical Group Self-Referral Specialist. If you choose an Physician as your personal physician, you may self-refer to any Affiliated Self- Referral Specialists. Self-referral specialists and other specialists are listed by medical center or county after the primary care physicians listing in each section of this directory. (If your Physician has two asterisks (**) next to his or her name, you are encouraged to receive care from Kaiser Permanente Medical Group selfreferral and referral specialists.) As an Added Choice member, you can receive care from any specialist you wish, but you ll be required to pay a percentage of the costs after your deductible has been met. Frequently Asked Questions Member Services/After-hours Urgent Care Member Services What do I pay when I use the services of a Kaiser Permanente Provider? You pay a copayment for office visits. The cost of your office visit will be determined by your plan. Your copayment is due at the time of your visit. How do I schedule an appointment? If your doctor practices at a Kaiser Permanente Medical Center, there is one number for you to call to make or cancel appointments, speak with an advice nurse, or access afterhours urgent care regardless of which Kaiser Permanente Medical Center you use. Locally, the Health Line number is (404) 365-0966. If dialing long distance, you can dial the Health Line number toll-free at 1-800-611-1811. The Health Line is open to schedule appointments or to speak with an advice nurse 24 hours a day, seven days a week. (If your personal physician is one of our Physicians, call his or her office directly to make an appointment.) Do I need a referral from my personal physician to receive specialty services? Under the Kaiser Permanente Provider benefit level, specialty care services are coordinated by your personal physician. Each physician works with a select group of specialists to ensure that you receive quality care. Your personal physician will be responsible for coordinating care you receive in the hospital or from specialists, and will write a referral for specialty care. When you require a specialty service, you will need a written referral order from your personal physician. The form will show the services the specialist is to provide as well as provide clinical information for the specialist. Be sure to take the referral form with you to the specialist as some specialist may cancel your appointment without it. Your personal physician will let you know if your referral requires authorization by Kaiser Permanente. Please note the first referral usually authorizes only one visit. After-hours Urgent Care What if I need medical advice from a Kaiser Permanente Provider after regular office hours? You can call one of our advice nurses 24 hours a day, seven days a week if you simply want advice on what to do if you are sick or hurt, and you believe the situation is not life or health threatening. Our advice nurses are registered nurses. The advice nurse will answer your questions and may suggest you make an appointment, go to one of our After-hours Urgent Care Centers, or go to the nearest emergency room. To reach an advice nurse, just call the Health Line (404) 365-0966 locally or 1-800-611-1811 long distance. What if I need to see a Kaiser Permanente Provider after regular office hours, but it s not an emergency? After-hours urgent care can help you during those times when your medical office is closed and the situation is not critical or does not jeopardize your life, but immediate medical attention is needed. It is a more convenient and less expensive alternative than hospitals emergency rooms. It offers shorter waits for medical attention than most emergency rooms. After-hours urgent care is available evenings, weekends, and holidays. (If you have an emergency, call 911 or go to the nearest emergency room.) 20

Frequently Asked Questions After-hours Urgent Care An emergency is any sudden, severe illness or injury that jeopardizes life or health and would lead a reasonable person with no medical training to seek immediate medical attention.) Please remember that your copayment for after-hours urgent care will usually be higher than the copayment you pay during normal office hours. Please check your benefits plan for specific copayment information. Frequently Asked Questions Pharmacies Pharmacies How much will it cost me to use a Kaiser Permanente Medical Center pharmacy? Under the Kaiser Permanente Provider benefit level, your coverage includes the generic prescription drugs and brand prescription drugs listed on our formulary.
