PURPOSE: To communicate the template requirement for the Certified Investigation Report (CIR), the work of the administrative review team and what specific information related to investigations must be entered into the Enterprise Incident Management (EIM) system. DISCUSSION: The Office of Developmental Programs (ODP) has released an updated Pennsylvania Certified Investigators Manual. The 2016 manual reflects evolving procedures related to conducting investigations and incident management. Highlights of the changes include: Template changes to the CIR and the Administrative Review (section V of the CIR)-Also, now available in PDF and Word Format Clarification regarding the use of social media as investigatory evidence Guidance on how to conduct interviews with family members Clarification about writing the investigatory question Personal safety information Cultural Awareness information ISSUE DATE ODP Announcement Number: 029-16 Page 1
Certified Investigators and Administrative Review Teams must utilize the Certified Investigation Report (CIR) template provided in the most recent version of the Pennsylvania Certified Investigators Manual to document the investigation process and to summarize and evaluate investigation findings. Certified Investigators will use the narrative created in Section IV of the CIR to complete the Summary of Investigator s finding text box found on the Enterprise Incident Management (EIM) Investigation Information page. Certified Investigators must use the narrative created in Section IV of the CIR to complete this box. At no time should a Certified Investigator cut and paste the full investigation document into the Summary text box. ODP Communication Number: Announcement 029-16 page 2 of 6
The administrative review team is responsible for section V of the CIR. Section V of the CIR, Administrative Review, Findings, Recommendations, and Implementation, contains a table to capture any corrective actions as a result of the incident. Corrective actions noted in this table must be included on the Corrective Action Description page in the incident report in EIM. ODP Communication Number: Announcement 029-16 page 3 of 6
In addition, entities conducting investigations are reminded that Certified Investigators are not making investigation outcome determinations. This is part of the administrative review process. Section V of the CIR includes a place to document the investigation outcome. Upon completion of the Administrative Review, the investigation outcome is documented in EIM on the Verification of Incident Classification page.
