Practice Nurses We have a practice nurse team who are available to offer help and advice on general health matters. They will see children with childhood illnesses, take cervical smears, deal with minor injuries and give "travel advice". They are experts in the care and management of wounds and are involved with the running of the wide range of clinics which are provided by the practice. Doctors Training In General Practice The practice usually has the benefit of one or more doctors training in general practice. After a substantial time spent working in hospital, they spend a year with us before entering independent practice. They are available for consultation in the same way as the partners. Medical Students Occasionally we may have medical students who sit in with the doctor or nurse during consultations. You will be notified in advance if a student is present and have the choice of requesting that the student leaves the room before the consultation begins. TELEPHONE NUMBER 0161 445 9000 APPOINTMENTS Booked Appointments * These can be booked up to six weeks in advance (subject to availability). * Appointments can be made in person at reception or over the internet. * If you are more than 15 minutes late you will need to make another appointment to see a doctor. * If you are unable to attend or no longer require a booked appointment, please cancel the appointment so that it can be offered to another patient. * Patients requesting same day appointments for more urgent problems should contact the surgery before 9.30am Monday to Friday. Appointments may also be booked via the internet. You need to register for this service and the relative information can be obtained from our reception staff. HOME VISITS Home visits are at the discretion of the doctor. Please do not ask a doctor to call unless the patient is genuinely too ill to come to the surgery. Home visits are very time consuming. We can see at least four patients at the surgery in the time it takes to see one at home. If the patient has a high temperature or a rash, coming to the surgery will do no harm and will not endanger others. In this event please inform reception on arrival, so that the patient may go directly into an examination room. When the condition of the patient does require a home visit, please try to give notice before 9.30am on the same day the visit is required. When you request a visit the sister will ask for full details of the patient s name, address, age and telephone number and the reason for the visit. This information enables the doctors to plan their calls and allows urgent visits to be dealt with promptly. CONTACTING THE DOCTOR OUT OF HOURS Our out-of-hours service is provided by Go To Doc Ltd. Telephone 0161 336 5958. If you call when the surgery is closed you will be given their contact details. Please note that this service is for urgent requests only, ie those that cannot wait until the surgery re-opens. In the case of an acute collapse or dire emergency please dial 999. Alternatively, you can call the NHS Direct general advice line on 111 (calls are free to this number). NHS Direct is a helpline staffed by nurses and professional advisors, giving confidential healthcare advice and information 24 hours a day. They can advise you on what to do next. If your problem is more serious, NHS Direct can call out an ambulance on your behalf. The service provides information on local health services, such as doctors, dentists or out-of-hours pharmacies and self help or support organisations. NHS Direct can also be accessed via the internet at www.nhsdirect.nhs.uk

All our staff have access to patient information at various levels. They all receive training aimed at ensuring confidentiality is maintained throughout the surgery at all times. If you wish to speak to a receptionist in private, please ask and we will accommodate your request. You can also ask to write down the information and hand this to a receptionist. In line with the above, we are unable to disclose any information regarding a patient, unless they are deemed a minor, to anyone other than the patient, unless written request is given that we may do so. Please do not be offended if you are refused information regarding another patient, even if they are a family member or close friend. We have all our patients best interests at heart. How can you access your health records? You have the right of access to the information we hold about you on our records. If you require access to/information from your records, a form of access is available at reception MINOR SURGERY We undertake a range of minor surgical operations here at the centre that you may be referred to by any of the doctors. We aim to provide a convenient service without the need to have to travel to hospital.

SERVICES FOR PATIENTS OVER 75 We offer all our patients aged 75 and over the opportunity to have an annual health assessment at home or at the centre. REPEAT PRESCRIPTION ARRANGEMENTS Telephone requests are NOT accepted Hand-written additions will NOT be issued Only order repeats when you have one week's medication left Tick required items on the repeat slip attached to your prescription Nobody will be offended if you do not order every item If the repeat slip is lost, hand-written requests must include: name, address, date of birth, name of drug and the dose you are taking Requests can also be made via our website. When Can I Collect My Repeat Prescription? Hospital outpatients' prescriptions should be collected from the hospital pharmacy. Otherwise, they will be ready within one week. In the case of privately issued hospital prescriptions, these should be taken directly to a pharmacy as they are not issued under NHS guidelines. If collecting repeat medications directly from the pharmacist, please allow 72 hours. RESULTS OF TESTS If you need to ring the centre to check the results for blood tests, smears or x-ray investigations, please telephone between 2.30 and 5.00pm. In the interests of confidentiality no information can be given without the patient's consent. DISABLED ACCESS The building facilities comply with the Disability Discrimination Act. PARKING Although parking is provided, this is for patients, visitors and staff permit holders only. All patients and visitors to the surgery must obtain a valid permit from reception which should then be visibly displayed in their vehicle for the duration of their visit. Parking spaces are limited, and priority should be given to the elderly and disabled patients. If at all possible, please take the healthy option of walking to the surgery. Alternatively, bus stops are sited on Wilmslow Road directly outside the centre. Please allow sufficient time before your appointment to obtain your permit or in case you need to park off site. Your co-operation in this matter is greatly appreciated.

SUGGESTIONS AND COMPLAINTS The practice aims to provide the highest possible standard of patient care. If you have any comments, suggestions or complaints about the service we offer, please write to the complaints manager. Alternatively, you may complete a Complaints Report Form which is available from reception. FREEDOM OF INFORMATION PUBLICATION SCHEME The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the classes of information the practice intends to routinely make available. This scheme is available from reception. Zero Tolerance We strongly support the NHS policy on zero tolerance. Anyone attending the surgery who abuses the GPs, staff or other patients be it verbally, physically or in any threatening manner whatsoever, will risk removal from the practice list. In extreme cases we may summon the police to remove offenders from the practice premises. COMMUNITY-BASED NURSING STAFF We work in close partnership with the community-based nursing services. Community (District) Nurses The district nursing team work with us to provide nursing care and support for our patients in their own homes. The district nurses may be contacted by telephoning their office on 0161 443 0615. Community Midwives The community midwives care for our patients during the antenatal period and for two to four weeks after the birth of the baby. They may be contacted by telephoning 0161 447 4233 between 2.00-4.00pm or through our reception staff. Health Visitors The health visitors are community nurses who work with the practice to provide health promotion services for children and their families. They can be contacted on 0161 443 0610. Macmillan Nurses The Macmillan nurses provide advice and support for cancer patients and their families. They can be contacted through your doctor or by telephoning 0161 861 8888 extension 220/239. HEALTH PROMOTION SERVICES We see the promotion and the maintenance of health as a very important aspect of the service we offer and provide the following services to help our patients. Please telephone or enquire at reception if you want an appointment for any of the clinics. Maternity Services The partners are all included in the Manchester Health Authority's Obstetric List and provide maternity services. Health Promotion Clinic This clinic is run by our health promotion sister and offers screening for heart disease and advice and support for you about improving your health and preventing disease. Asthma And Chronic Lung Disease Clinic This clinic aims to help people with asthma to understand and manage their condition so that they can maintain the best possible health. Breast Cancer Screening All women aged 50 to 70 are invited for mammography every three years. Diabetic Clinic This clinic offers patients with diabetes the opportunity to have regular specialist medical and nursing care. Travel Advice Advice, information, vaccination and services are provided to help you maintain your health and prevent disease while travelling. There is a charge for most of the vaccinations. Immunisation We offer a full range of immunisation to help protect our patients from disease. Family Planning We provide a comprehensive range of family planning services. The pill and general advice on contraceptives may be obtained from any of the doctors. Cervical Smears We encourage all our adult female patients to have this simple test at least every three years. You may have a smear taken by your regular doctor, a lady doctor or by the practice nurses.

CHILD HEALTH PROGRAMME The care given to your child in the first five years of life is important for the future. We wish to work in partnership with you to protect your child from disease and promote good health for life. We are also able to treat any health problems that may arise, in the full knowledge of your family's health and medical history. If you wish to have your child immunised, please make an appointment. NOTES NON-NHS SERVICES A fee is payable for all non-nhs work. This includes things such as medical examinations for travel, sport or employment, medical reports for insurance companies or solicitors and all private letters.
