Hawthorne Residential Partners offers Medical, Dental, Vision, Life Insurance, Short & Long Term Disability, Dependent Care Flexible Spending Accounts and Voluntary Life Insurance through our company s benefit plans. The summary of Hawthorne s current insurance is listed below. There is a 60 day waiting period for these benefits for newly hired, full time employees. Hawthorne offers a 401(k) retirement savings plan and all employees are eligible to participate with no waiting period. Medical & Dental Coverage Below is a brief summary of the two plans that we offer for our full time employees through Blue Cross Blue Shield. Hawthorne Residential Partners will continue to pay 80% of the employee only cost of the Core Plan. Buy Up Plan Core Plan Primary Care Office Visit $25 copay $20 copay Specialist Office Visit $50 copay 70% after deductible ER Copay $300 copay 70% after deductible Preventive Care 100% covered 100% covered Plan Year Deductible $2,000 $2,000 In-Patient Hospital 70% after deductible $250 copay, 90% after deductible Out-of-Pocket Maximum (excluding deductible) $4,000 $4,000 Prescription Drug Card (generic/brand name) $10/50% to $100 $10/50% to $100 Dental Coverage (BCBS) Hawthorne is paying 50% of the cost of employee Dental coverage. $1,000 Maximum Yearly Benefit (per person) $50 Plan Year Deductible 100% Preventive Care (no deductible) 80% Basic Care 50% Major Care 50% to $1,000 lifetime maximum for Child Orthodontia 12 month wait for Orthodontia Coverage (for newly enrolled dependents)

Life Insurance and Vision Coverage Employee Basic Life Insurance (BCBS / US Able) 1 x annual earnings to a maximum of $175,000 Paid 100% by Hawthorne Expanded Vision Coverage (VSP) $10 copay for exams (every 12 months) $25 copay for materials Lens covered every 12 months Frames covered every 24 months *Note: Both the Buy Up and Core plans include coverage for vision exam at the Specialist copay every 12 months. The Expanded Vision coverage offers a benefit for a lower exam cost and for lens & frames. Short and Long Term Disability Coverage Hawthorne pays 50% of the cost of your disability coverage. Benefits and costs are described below. Short Term Disability Pays you 60% of your weekly salary to $500 max per week Benefits Begin on the 15 th day of an accident or illness Benefit may continue for up to 11 weeks Long Term Disability Pays you 60% of your monthly salary to $6,000 per month Benefit Begins after 90 days Benefits can continue for: Up to 2 years based on your own occupation Or to age 65 if you cannot perform any occupation due to your training, education and experience Guaranteed coverage if you enroll now. If you desire coverage later, you will be required to complete a health questionnaire and the insurance company can decline coverage.

Dependent Care Flexible Spending Accounts Hawthorne offers a Dependent Care Flexible Spending Account program for all employees. Dependent Care Flexible Spending Accounts are a great way for employees to save on eligible dependent care expenses by reducing their taxable income. Reimbursement for eligible expenses includes child care, incapacitated spousal care and elderly parent care. Voluntary Life Insurance Coverage Voluntary Life Insurance Hawthorne is pleased to offer Voluntary Life Insurance. The plan features: Guaranteed Coverage for you, your spouse, and/or children if you enroll now Purchase $25,000, $50,000, $75,000 or $100,000 of additional life insurance on yourself Purchase $30,000 of coverage on your spouse Purchase coverage on your children ($10,000 benefit for each child) 401(k) Retirement Savings Plan Hawthorne offers a 401(k) savings plan for all employees and eligibility for enrollment is immediate upon hire. The plan provides an opportunity for participants to save for their retirement using pre-taxed dollars. Contributions are subject to annual dollar limits set by the IRS.

Paid Vacation, Sick Leave & Personal Days Full time employees accrue both sick and vacation time, beginning immediately upon hire as follows: Vacation is accrued at 0.385 days per biweekly pay period (or 3.08 hours per pay period), which is equivalent to two weeks on an annual basis. After 5 years of eligible service, employees are entitled to 15 vacation days each year, accrued biweekly at the rate of 0.577 days (or 4.62 hours per pay period). After 10 years of eligible service, employees are entitled to 20 vacation days each year, accrued biweekly at the rate of 0.769 days (or 6.15 hours per pay period). Employees may roll over a maximum of 40 hours (5 days) of vacation into the next year. Sick leave is accrued at the rate of 5 days per year (or 1.54 hours per pay period). Unused sick leave will accumulate until it reaches 10 calendar days. Further sick leave accruals after this maximum will be suspended until the employee reduces the balance below the limit. Full time employees must have completed one year of service by January 1 st to be eligible to receive two (2) personal days off with pay to be used during the upcoming calendar year. Unused personal days will not roll over into the next calendar year.
