Footnotes: 1. The applicant must be between age 16 to 65 next birthday for PRUCrisis Cover Vantage and age 21 to 65 next birthday for Crisis Cover Vantage Benefit. 2. For certain medical conditions, there is a waiting period of 90 days from the date of issue or reinstatement whichever is later. If the life assured is diagnosed as having any one of the Critical Illnesses listed, we pay 100% of the PRUCrisis Cover Vantage policy / Crisis Cover Vantage Benefit sum assured, less any prior successful claim under the Early Stage Payout benefit and / or Intermediate Stage Payout benefit. After paying out this benefit, your PRUCrisis Cover Vantage policy / Crisis Cover Vantage Benefit terminates. 3. There is a maximum payout of $500,000 for the Early Stage Payout benefit for each life assured, across all the PRUCrisis Cover Vantage policy(ies) and / or Crisis Cover Vantage Benefit(s) that the life assured may have. Once the maximum payout limit is reached, this benefit terminates and no further payout will be made under this benefit against any of the PRUCrisis Cover Vantage policy and / or Crisis Cover Vantage Benefit that the life assured may have. 4. There is a maximum payout for each of the Special Benefit Medical Conditions mentioned above, for each life assured, across all the PRUCrisis Cover Vantage policy(ies) and / or Crisis Cover Vantage Benefit(s) that the life assured may have. We pay this benefit only once for each of the above two Special Benefit Medical Conditions, i.e. once for Angioplasty and other Invasive Treatment for Coronary Artery and once for Diabetic Complications. 5. Once a claim for Diabetic Complications is made, no further payout will be made under Diabetic Complications across all PRUCrisis Cover Vantage policy(ies) and / or Crisis Cover Vantage Benefit(s) that the life assured may have even if the maximum Diabetic Condition payout amount is not reached. Each covered diabetic complication has a maximum benefit payout amount which could be lower than $250,000. 6. There is a maximum payout of $1,000,000 for the Intermediate Stage Payout benefit for each life assured, across all the PRUCrisis Cover Vantage policy(ies) and / or Crisis Cover Vantage Benefit(s) that the life assured may have. Once the maximum payout limit is reached, this benefit terminates and no further payout will be made under this benefit against any of the PRUCrisis Cover Vantage policy and / or Crisis Cover Vantage Benefit that the life assured may have. Any Intermediate Stage Payout will less off any prior successful claim under the Early Stage Payout benefit. Once a claim for this benefit is paid, the Early Stage Payout benefit will also terminate. 7. We waive all future premiums from the next premium due date following the date of diagnosis of a Early Stage or Intermediate Stage Medical Condition. 8. Once a claim for Angioplasty and other Invasive Treatment for Coronary Artery is made, no further payout will be made under Angioplasty and other Invasive Treatment for Coronary Artery across all PRUCrisis Cover Vantage policy(ies) and / or Crisis Cover Vantage Benefit(s) that the life assured may have. 9. The Accidental Death benefit is only payable if the Accidental Death occurs within 12 calendar months from the date of the Accident. Notes: Buying a life insurance policy is a longterm commitment.
