Dear Home Sleep Test Patient, Thank you for taking this home sleep test from Instant Diagnostic Systems (IDS). Your physician has ordered this test to evaluate you for Sleep Apnea, a condition in which you may stop breathing during sleep. An individual with this condition may not even be aware of it. Untreated, sleep apnea can lead to excessive daytime sleepiness and fatigue, as well as serious health problems such as high blood pressure, heart problems, diabetes and stroke. The good news is that once diagnosed, treatment options are available for every sleep apnea sufferer! Please read all of the home sleep test instructions before starting your home sleep test. Home Sleep Test Equipment Please take a moment to familiarize yourself with what is included in the kit: 1. ApneaLink Air Home Sleep Test device with new batteries pre-installed 2. Clear plastic nose (nasal cannula) tube 3. Finger sensor (pulse-oximeter) 4. A) Black elastic chest belt B) Effort (chest movement) Sensor 5. Extra AAA batteries 6. The video version of these instructions is available at www.idshst.com. An optional DVD containing these instructions may be included. 7. Paperwork and forms to complete and return with device (not pictured). 8. US Post Office return packaging with pre-paid label attached (not pictured). Before taking the home sleep test remember: DO NOT turn the device on until you are ready to go to bed to test! You should continue to take any medications, as directed by your physician. This may include, but is certainly not limited to sleep aids, pain/heart medications, blood pressure, diabetes, etc. Please test and return the device as soon as possible. 13025_IDSBrochure091514.indd 3

Home Sleep Test Instructions WRAP BLACK ELASTIC BELT AROUND CHEST Wrap the belt around your chest and under your arms. Use the Velcro tab to adjust the tightness. The belt should be snug, but not so tight so that it is uncomfortable. The manufacturer recommends that the ApneaLink device be placed at center of your chest, over the breasts. If, however, you sleep on your stomach or have a physical impairment that precludes its being in center of chest, you may rotate the belt so the device is at your side or back. APPLY CLEAR PLASTIC NOSE TUBE A. With prongs towards face, insert nose tube with one prong in each nostril, as shown. B. Wrap clear tubing behind the ears. C. Adjust slider so that loop under chin fits securely, but not so tight that it is uncomfortable. Note: You may apply adhesive bandages or hypoallergenic tape to the tube on your cheeks to keep the nose tube in place. 13025_IDSBrochure091514.indd 4

APPLY FINGER SENSOR Slide sensor on finger with wire on top, as shown. Finger should not stick through the end of the sensor. For greatest accuracy, remove nail polish (especially dark colors) or other coverings from the finger on which the sensor is placed. You may change fingers as needed for comfort. Note: You may apply adhesive bandages or hypoallergenic tape to keep the finger sensor and wire in place. TURN THE DEVICE ON AND SLEEP Just before you lie down to sleep, power on the device by pressing and holding the start/stop button until the green light appears. Next to each sensor connection, there are lights that should turn green within several seconds of powering on the device. If any of the lights remain red for an extended time, please check the appropriate sensor for a secure connection and appropriate placement on your body. It is normal for the lights to occasionally blink red. Note that the light INSIDE the finger sensor where your finger is placed should remain red. DURING THE NIGHT: If you have to use the restroom during the night, DO NOT turn the device off. Simply remove the finger sensor, if needed. DO NOT disconnect the sensor from the ApneaLink device. Put the sensor back on when you return to bed. Only power the device off if you do not plan to return to bed. 13025_IDSBrochure091514.indd 5

IN THE MORNING TURN THE DEVICE OFF If the Test Complete light is green, you are done! Please follow the steps to return the test kit to IDS. If the light is red, there may have been a problem, and you should replace the batteries and test a second night. Please note this on the paperwork. Note: If the light is still red after testing a second night, please return the device to IDS. Our physicians may determine there is sufficient data to get an accurate test result. COMPLETE ALL PAPERWORK When finished taking the home sleep test, please fill out and sign all included paperwork. If you performed the test a second night, please indicate that on the paperwork. Failure to return completed paperwork may delay the results of the study. PACK THE EQUIPMENT AND MAIL IT BACK TO IDS A. You may unscrew the clear plastic nose tube and throw it away. DO NOT disconnect any other sensors. B. Pack the device in the pre-addressed, postage-paid envelope provided and don t forget to include: ApneaLink device Belt(s) DVD if applicable Finger sensor Completed/Signed paperwork Extra Batteries if unused C. Seal the package and place it in YOUR mailbox or any outgoing U.S. Postal Service mailbox the day you complete the test. D. WHAT S NEXT: IDS Processes the Test From the day you mail your device, it will take 2 to 3 business days to ship to IDS. Once received, we will process and score the data, and then one of our board-certified sleep physicians will review. The entire process will take 3 to 7 business days. We will contact you by phone, email, and/or text if there are any issues. You may track the status of this process at www.idshst.com you will need your IDS reference number from the paperwork or emails you have received from us. Test Results Once our board-certified sleep physician interprets the test data, the test report is sent immediately by fax to your physician who ordered the study. In most cases, your physician will call you about the results and schedule a time to discuss treatment options if needed.
