RESOLUTION NO. 21-11 CENTRAL JERSEY HEALTH INSURANCE FUND DECLARATION OF DIVIDEND WHEREAS, The Fund has a surplus balance in its closed year account of $7,641,043 as of 12/31/10 (audited) and this balance is above and beyond what is necessary in the foreseeable future to assure the liquidity of the Fund; NOW THEREFORE BE IT RESOLVED by the Executive Committee that 1. A dividend of $2,000,000 be and is hereby declared and shall be payable from the closed year contingency account. 2. Dividends shall be distributed in accordance with the enclosed schedule, which is based upon share of assessments by fund year. 3. Any balances due and owing to the Fund from any member shall be reduced from the dividend prior to release. 4. Members shall have the option of receiving dividends in the form of either a check or credit against 2012 assessments. However, members are encouraged to avoid reducing appropriations by the amount of the dividend because the dividend is exceptional in nature and cannot be counted on to be duplicated in subsequent years. Members are therefore strongly encouraged to accept the dividend as a check rather than as a credit. 5. A certified copy of this resolution shall be filed with the N.J. RESOLUTION NO. 22-11 CENTRAL JERSEY HEALTH INSURANCE FUND AUTHORIZING THE ISSUANCE OF NOTICES AND CRITERIA FOR CONTRACT AWARDS FOR VARIOUS PROFESSIONAL AND OTHER CONTRACTS FOR FUND YEAR 2012 WHEREAS, the Central Jersey Health Insurance Fund is duly constituted as a Health Benefits Joint Insurance Fund and is subject to certain requirements of the Local Public Contracts Law and the Local Unit Pay to Play Law (N.J.S.A. 19:44A-20.4, 20.5 et. seq); and; WHEREAS, the Executive Committee of the Central Jersey Health Insurance Fund has deemed it necessary and appropriate to award contracts over $17,500 for services in 2012, with the consideration of Fund Years 2013 and 2014, in accordance with Fair and Open contract provisions of the Local Unit Pay to Play Law; NOW THEREFORE BE IT RESOLVED by the Executive Committee of the Central Jersey Health Insurance Fund that the Fund shall hereby post the enclosed legal notice (Appendix 1) on its official web site (http://www.cjhif.com/) and shall make available to interested vendors and professionals the enclosed Central Jersey Regional Employee Benefits Fund Criteria For Submission Of Qualifications For Professional And Other Contracts Under The Fair And Open Process (Appendix 2); CENTRAL JERSEY HEALTH INSURANCE FUND ADOPTED: SEPTEMBER 27, 2011 BY: CHAIRPERSON ATTEST: SECRETARY

Appendix 1 - For publication on website: LEGAL NOTICE REQUEST FOR QUALIFICATIONS FOR PROFESSIONAL AND OTHER CONTRACTUAL POSITIONS FOR THE CENTRAL JERSEY REGIONAL EMPLOYEE BENEFITS FUND NOTICE IS HEREBY GIVEN, that pursuant to N.J.S.A. 19:44A-20.4, 20.5 et. seq. the Central Jersey Regional Employee Benefits Fund shall pursue a Fair and Open Process in awarding contracts for positions set forth more fully below, and hereby solicit qualifications in the form of resumes and letter applications for the below stated positions of the Central Jersey Regional Employee Benefits Fund, which shall be submitted to Emily Koval, Assistant Secretary, Central Jersey Regional Employee Benefits Fund, 250 Pehle Ave, Suite 701, New Jersey 07663, with documentation showing qualifications and compliance with criteria set forth by the Fund and by State Statute. All qualifications shall be submitted in a sealed envelope to the Executive Director to be received no later than 2:00 p.m. on Monday, October 17, 2011. The envelope containing the proposal shall be clearly marked on the outside Sealed Contract Proposal. Resume and letter application for professional position and other contracts including all documentation that provides the following for evaluation by the Fund s Executive Committee: a. Experience, qualifications and reputation in the field for the position sought. b. Knowledge of the area of expertise for the position sought (i.e. Fund Attorney, Fund Auditor, etc.) c. Experience and knowledge of the Central Jersey Regional Employee Benefits Fund. d. Availability to accommodate any required meeting of the Fund. e. Designated professional and support staff and location of firm s offices. f. References in general and in particular from municipal health insurance funds where the professional or contractor has provided similar services as sought in the Fund. II. III. Selection of professionals shall be solely on the Fund Executive Committee s evaluation of the submitted material in the criteria set forth in this document. The Fund reserves the right to negotiate the terms and conditions of a contract with the successful firm or firms to obtain the most advantageous situation for the Fund.
