INFORMATION The Housing Authority of the Town of Phillipsburg was organized with the primary goal to provide a decent home in a suitable living environment for families, senior and disabled persons that cannot afford market rate rentals. The Housing Authority s program is administered at the local level in accordance with State Law. Financial assistance, general supervision, direction and program guidance are provided by the U.S. Department of Housing and Urban Development (HUD). REQUEST FOR INFORMATION The Phillipsburg Housing Authority is requesting submission of qualifications to provide services as the Housing Authority s Architect of Record for the period of December 7, 2011 thru June 30, 2012. Please submit Proposal Rate for a second year Professional Service Agreement for Fiscal Year July 1, 2012 thru June 30, 2013 using the same criteria as required for December 7, 2011 thru June 30, 2012. The Housing Authority reserves the right to accept any or all proposals or to waive any informality in the proposals and the right to reject any and all proposals. This Proposal is being solicited through a fair and open process in accordance with N.J.S.A. 19:44A-20.5 et. seq. Your qualifications package for Professional Services should include the following: 1. Provide with your submission: Information regarding your Firm, including ownership, list of Housing Authority Clients, and other pertinent information defining your Firm; and 2. Answer the attached prequalification questionnaire; and 3. Provide a list of Five (5) Housing Authority references; and 4. Provide hourly fee structure; and 5. Provide other information which you believe would be helpful in evaluating your submission; and 6. Provide evidence of insurance for the following(a) Workers Compensation and Employers Liability Insurance, (b) Professional Liability Insurance covering claims made at any time prior, during or subsequent to completion of the Architect s Firms Services with a limit of not less than $1,000,000 and with a deductible not more than $10,000.00, (c) Comprehensive General Liability Insurance covering bodily injury and property damage covering claims made at any time prior to, during or subsequent to completion of the Architects Firms Services with a limit of not less than $500,000 per occurrence on bodily injury and $1,000,000 annual aggregate on property damage. The Housing Authority shall be named as additional insured in these policies (if awarded the Proposal); except in Professional Liability Insurance. Insurance certifications must contain a Thirty (30) - day cancellation clause. 7. Provide evidence of New Jersey Business Registration Certificate issued by the Division of Revenue; and 8. Provide evidence of an active New Jersey License; and 9. Complete the attached statement that the Firm is not debarred, suspended or otherwise prohibited from professional practice by any Federal, State or local agency. What is the Firm s experience with maintenance projects (i.e. roofs, windows, etc) for Public Housing Authorities. 3. Legal Action Has a Housing Authority ever initiated legal action against your Firm. If so, please explain. Have you ever been involved in a legal action against a Housing Authority. If so, please explain. Has a client ever initiated legal action against your firm? If so, please explain. Does the firm have any pending litigation. If so, please explain. The Housing Authority intends to appoint the Architect of Record on December 7, 2011.
