Surgical Technology Annual Application First day to submit is June 20, last day to submit is July 23 each year. The attached materials provide an application submission guide for the Surgical Technology program. Please read all of the application carefully. In order for your application to be considered, all steps must be completed and the required materials submitted to Hawk Central Student Records office at the Pasco campus. Surgical Technologists work as members of the surgical team to prepare the operating room for surgery, assist during surgery, and carry out the proper post-operative procedures. Surgical Technologists ensure that the operating room is prepared for surgery, all instruments are properly sterilized, ready the patient for surgery by preparing the incision site, and assist the surgical team during and after the procedure. A Surgical Technologist assists the surgeons by handing them instruments during the surgery and ensures that the immediate surgical field remains sterile. Surgical Technologists are typically employed in hospital operating rooms but may also be stationed in outpatient surgery centers and in central supply departments. Most Surgical Technologists will work a 40- hour week, but may also need to be available on weekends, evenings, holidays, or on-call. The Surgical Technology program is a selective admission and competitive entrance program. You must apply to the College and also to the program. Applicants are screened according to timely coursework completion and grades completed through spring quarter. CBC s Surgical Technology program is a four-quarter program beginning in fall quarter. During the course of the program, enrolled students will gain extensive hands-on training and acquire more than 350 hours of operating room experience in clinical settings. Upon successful completion of all degree and program requirements, students will obtain an Associate in Applied Science degree in Surgical Technology. After completion of prerequisites and the first quarter of the Surgical Technology program, students will also complete an Operating Room Aide One-Year Certificate. Applications must be received as a complete package. Please read and complete all sections of the application. Section 1, 2, 3, and 4 must be returned to address information provided below. Applications will be evaluated for priority based on the number of prerequisites completed and a composite prerequisite grade point average. Surgical Technology Annual Application Checklist Section 1 General Application 1. This will expedite your Surgical Technology program evaluation. Students who have taken classes at Columbia Basin College do not need to include a transcript from CBC. The Student Records office will provide a current copy of your transcript to accompany this application. Students accepted into the program must submit a request and payment for their national background search to the college-approved vendor. This will include your certified tracker account, where accepted applicants will upload your immunization history by the designated due date. Students accepted into the Surgical Technology program are required to have the following documents and to maintain them for the duration of enrollment in this program: American Heart Assocation Health Care Provider (HCP/CPR) card First aid card Seven Hour HIV/AIDS Bloodborn Pathogens training Applications will be reviewed each year. Surgical Technology Supplemental Application Information The Columbia Basin College Surgical Technology program utilizes two elements to prioritize student applicants for entrance into the program. Those elements include (A) cumulative grade point average among selected priority element courses, and (B) departmental course score. Each of these elements is outlined further below. These two elements will be combined for a total Surgical Technology Admissions Index Score based on a total point value of 20 points. A. Cumulative Grade Point The cumulative grade point of the following courses will be calculated for the entrance index score. 1. Microbiology BIOL& 260 & 260L 2. MATH 106 or above MATH 106+ (except MATH 109) The cumulative GPA (from a total possible cumulative score of 4.0) will be multiplied by a factor of 2.0 for 10 total possible points. Students who have not completed one or more of the courses listed will have a grade of 0.0 recorded for the number of credits identified within the list. Letter grades (including those received from transferring colleges or universities) will be assigned the following decimal scores according to campus policy: B+ = 3.3 C+ = 2.3 D+ = 1.3 A = 4.0 B = 3.0 C = 2.0 D = 1.0 A- = 3.7 B- = 2.7 C- = 1.7 D- = 0.7 B. Departmental Course Completion Score The departmental course completion score is a cumulative score based on the total number of non Surgical Technology courses completed for an individual student. Surgical Technology Supplemental Essay - Section 4 (limit to 250 words or less) Essay Please respond to the following question in 250 words or less. Type your answer and attach a hard copy to the application. Why are you interested in Surgical Technology and what impact do you hope to make in your community when you become a Surgical Technologist? 6

Surgical Technology Program Costs All fees are estimated and are subject to change. However, the following figures may be used as a general guideline to assist applicants in preparing for the costs associated with enrollment into the program. 1. Tuition: as stated in the CBC Quarterly Information located on the web at columbiabasin.edu/schedules. For additional information and resources on paying for college, see CBC s Paying for College webpage at columbiabasin.edu/payingforcollege. 2. All students are encouraged to have accident/health insurance. A student injury and sickness insurance plan is available to purchase. Forms are available at Hawk Central or at the Health Science Center. 3. All students are required to have malpractice insurance. This cost is included in your course fees at the time of registration, typically $7.50 per quarter. 4. Books: approximately $1,200 for the four-quarter course sequence. 5. Supplies: notebooks, pencils, and other materials estimated at $100 per quarter. 6. Applications must include a current copy of these health documents: American Heart Association Health Care Provider CPR/HCP AED card Current first aid card. A valid certificate of training for seven hours of HIV/AIDS Bloodborne Pathogens. Training is available (no cost) at the CBC Library, call for hours at (509) 542-4458 in Pasco or (509) 544-8336 in Richland. You do need to view and complete the DVD training series in the library, materials do not leave the library. Applicants who are selected are required to comply with the following items: A. National background check requirements by the college-approved vendor. The national background check is an additional expense to the student $97 or more. B. Immunization costs will vary according to the vaccination history that you have available, estimate costs from $100 to $400. C. Uniforms: The scrub color for the Surgical Technology program is red. Lab coat is part of the uniform requirement from the approved vendor. The jacket should be plain white, button/snap front, with long sleeves. Ordering details will be provided upon acceptance into the program. D. Shoes: black and washable without straps or holes, approximately $40 a pair. E. Name Pin: No charge for first clinical identification badge, $3.50 for reissue of lost or stolen badges. F. AST examination fees are: $190 for current members and $290 for non-members. These fees are nonrefundable and includes a $60 processing fee. Make checks payable to NBSTSA. National Board of Surgical Technology and Surgical Assisting website at http://nbstsa.org/examinations-cst.html#fees. G. There are lab field trips with mandatory attendance. Lab fee costs, travel, and additional expenses are the student s responsibility. Health Sciences Center Student Health Record Guidelines REQUIRED IMMUNIZATIONS: TUBERCULIN TEST (PPD) HEPATITIS B (HBV) MEASLES, MUMPS, AND RUBELLA (MMR) VARICELLA VACCINE (Chicken pox) TETANUS, DIPHTHERIA, AND PERTUSSIS TOXOIDS COMBINED (Tdap) INFLUENZA A two-step process is required for compliance with clinical facilities. The two-step method should be completed if: You have no history of a TB test You have not followed through with a yearly TB test and you are over due This two-step method includes the following: 1. Obtain TB skin test #1 2. Have test #1 read within designated time 3. Obtain TB test #2 within 2-3 weeks of first test 4. Have test #2 read within designated time 5. Provide documentation of two tests to Columbia Basin College. If at any time, the PPD is positive, the student must go to the Benton County Public Health for follow up. The student will need to provide documentation of their visit from the health department. The TB test is to be repeated yearly from the date of the last (2nd) test. *If currently working in a position that requires yearly TB testing and you receive your annual TB test prior to the expiration date, you do not need to complete TB test #2. The Quantiferon TB Gold whole blood test is provided by many facilities. If you obtain this test, it will meet the requirements for the two-step TB testing and negates the need for the TB tine test. Documentation that the HBV vaccine series has been initiated prior to entry of any Health Science program or documentation of the completed vaccine series (3 injections). If no documentation is available and the student states they have received the series, the student will sign a verification of immunization form, and will be required to obtain a titer and follow up with an HBV booster if results are non-immune. Further follow-up may be required but will be reviewed on an individual basis. Students born in or after 1957 must provide documentation of: a. administration of two doses of live measles, mumps, and rubella virus vaccine or b. laboratory evidence of immunity (MMR titer) Students born before 1957 must provide documentation of immunity (titer) and if no documentation is available, one dose of MMR is required. Documentation of disease history or immunizations. Students who are relying on history will sign a Verification of Disease form. Students who are unsure of their history must obtain laboratory evidence of immunity (varicella titer) or provide proof of two doses of varicella vaccine, separated by 4-8 weeks. Documentation for a Tdap immunization prior to entering any Health Science program. Those with a documented Td immunization within the past two years are exempt from obtaining the Tdap, but must provide documentation of their Td. If >2 yrs since the Td, you will be required to obtain a Tdap. The Tdap is good for 10 years. Influenza vaccine is mandatory for all Health Science students. Those students who are unable to receive the vaccine need to fill out a waiver and follow the facility policy for wearing respiratory protection. Proof of immunization needs to be provided yearly before entering any clinical facility. RECOMMENDED VACCINES: HEPATITIS A (HAV) POLIO A combination HBV and HAV vaccine (Twinrix) is now available. This vaccine is administered in a three-dose series. Students should have completed a primary series of polio vaccinations. MENINGOCOCCAL Meningococcal vaccine is highly recommended for all healthcare workers. 8 REV. 01-13-2013
