RESOLUTION NO. COUNC IL A RESOLUTION OF THE CITY OF GLENDALE PROVIDING FOR SUPPLEMENTARY BENEFITS AND COMPENSATION OF CERTAIN OFFICERS OF THE CITY OF GLENDALE TO SUPPLEMENT THE CLASSIFICATION AND COMPENSATION RESOLUTION OF SAID CITY AND AMENDING THE GLENDALE MUNICIPAL CODE RELATING TO COMPENSATION AND FRINGE BENEFITS. WHEREAS, the City of Glendale (hereinafter referred t o as "Cit yh) has established a system of classification for all positions within the City service with descriptive occupational titles used t o identify and distinguish positions from one another based on job d uties, essential funct ions, knowledge, skills, abilities and minimum requirements ; and WHEREAS, the City has established a system of supplementary benefits for the classification titles list ed herein, based on resolutions a nd agreements as approved and adopted by the City Council ; and WHEREAS, it is in the Ci ty ' s best interest to periodically review and amend said supplementary benefits to ensure that the City's benefit package is contemporary, competitive and meets all statut o ry requirements; and WHEREAS, the City of Glendale is a charter city and Article XI, Section 5 of the California Constitution grants chart er cities home rule over municipal affairs, which includes the compensation and benefits paid to its officers. NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF GLENDALE: SECTION 1 : PURPOSE The purposes of this resolution is to adopt changes to t he benefits provided to Council Members (hereinafter referred to as City Officers. SECTION 2: SHORT TITLE This resolution may be referred to as the Supplementary Benefits Resolution. SECTION 3 : EXTRA COMPENSATION SECTION 3. 1 : GLENDALE REDEVELOPMENT AGENCY MEETINGS. City Officers shall receive one thousand four hundred dollars ($1,400) per month, representing three hundred fifty dollars (6350) thirty dollars ($30) for each meeting of the Glendale Redevelopment Agency, at up to a maximum of four meetings per month. SECTION 3. 2: GLENDALE HOUSING AUTHORITY MEETINGS. City Officers shall receive fifty dollars ($50) for each meeting of the Glendale Housing Authority, up to a maximum of four meetings per month. SECTION 4: ALLOWANCES SECTION 4. 1 : AUTOMOBILE ALLOWANCE. Members of Council and the Mayor shall receive $525 per month for auto allowance. 2

SECTION 5, BENEFITS SECTION 5.1: GROUP INSURANCE BENEFITS. The benefits of group health, medical and accident insurance shall be provided t o all City Officers and to all full-time City employees compensated on a dependents of such persons. monthly basis and for certain qualified Said insurance benefits shall be provided solely by contracts of insurance approved by the Counci l and purchased by the City from time to time. Group insurance benefits provided in other resolutions shall remain in effect unt il superseded by benefits provided in this resolution. SECTION 5.2: ACTIVE CITY OFFICERS AND EMPLOYEES - MEDICAL. (c) Upon the death of a City Officer insured on any of the City' s medical insurance plans, the City may continue to pay the entire cost of the medical insurance premium for the City Officers' dependents covered at the time of death, for a two (2)~year period of time from the date of death subject to the terms and conditions of that deceased City Officer's insurance plan. SECTION 5. 3 : EXECUTIVE MEDICAL REIMBURSEMENT PLAN (EMRP). The City shall provide a medical reimbursement plan for City Officers and their dependents enrolled in the PPO medical plan. Eligible expenses for this plan shall include deductible and co ~ insurance contributions made for medical, dental and vision treatment. The plan shall include preventive care, immunizations and dependent physical examinations. Cover charges shall be limited to one hundred twenty five percent (125%) of prevailing charges t o an annual maximum of ten thousand dollars ($10,000). SECTION 5.4 : DENTAL INSURANCE BENEFITS. The benefits of group dental insurance shall be provided to all City officers and to all full~time salaried City employees, compensated on a monthly basis, and for their qualified dependents. (a) The City shall pay t he entire cost of the High Option PPO dental insurance plan for all City Officers and employees and their qualified dependents enrolled in this plan for the coverage they are eligible for. (b) City officers and employees covered under the PPO medical plan may participate in dent al coverage under all available dental insurance plans. City officers and employees covered under the HMO 4

medical plans are restricted to dental coverage under the Mid-Level PPO dental insur ance plan or t he HMO dental p l an. (e) Reti red Ci ty Of ficers covered under the PPO medical i nsurance plan may continue to utilize all available dental insurance plans. Ret ired City Officers in the HMO medical insurance plans are rest ricted t o coverage under t he Mid-Level PPO dent al insurance plan or the HMO dental plan. The entire premium cost of such coverage shall be paid by the retired Cit y Officer or ret i r ed employee. SECTION 5.5: VISION OPTICAL PLAN. The benefits of a group vision care plan shall be provided to City officers and t heir dependents. The City shall pay t he entire cost of such insurance. SECTION 5.6 : LIFE INSURANCE BENEFITS. Benefits of group life insu ran ce shall be provided to City o fficers o f the Ci ty of Glendale. The amount of this benefit is $50, 000 for t e r m life/accidental death and dismember ment (AD&D) i nsurance. The City shall pay the cost of such i n surance for each such City officer. SECTION 5.7: LONG-TERM DISABILITY PLAN. Benefits o f a long- term disability plan shall b e provided to City Office rs. The City s hall pay the cost of such plan fo r each said designated Ci t y Office r or employee. SECTION S.B: PHYSICAL FITNESS/CIVIC ORGANIZATION MEMBERSHIP. The Ci ty shall reimburse City officers an amount not to exceed four hundred fifty dollars ($450. 00) per f iscal year for the cost o f membershi p for themselves and their families in a physical fitness 5

facility or program, a wellness program, and/or or for related physical fitness exercise equipment as defined in the Administrative Policy Manual. This reimbursement may also apply to membership in the Glendale Chamber of Commerce and/or other related civic organizations. SECTION 5. 9 : COMPUTER LOANS. The Director of Finance is authorized and directed to establish loans up to four thousand dollars ($4,000) for qualified City Officers for the purchase of City- approved computer equipment. SECTION 6, DEFERRED COMPENSATION PLANS. Such plan or plans may provide for a deferral of a portion of base salary for such City officers may authorize deductions to be made from their base salary for the purpose of participating in such deferred compensation plan or plans. Such sums as are deposited by the City of Glendale into such plan or plans shall be invested in such manner as shall be lawful under the laws of the United States and of the State of California applicable to the investment of deferred compensation plan funds and in effect at the time of such investment. The Director of Finance is hereby authorized and directed to establish all accounts made necessary to implement the provisions hereof, and to cause the deductions from base salary of, and in the amounts designated by the City officers who elect to participate in such plan or plans. Participation in such plan or plans, and payment of benefits into such plan, shall be by separate and individual written agreement between each such eligible City Officer. SECTION 7: PROVISIONS SIMILAR TO PRIOR SALARY RESOLUTION-CONSTRUCTION 6

The provisions o f this resolution, so f ar as they are subs t ant ial ly the same as t he provisions of the former supplement ary salary resolution, must be const r ued as continuat ions thereof and not as original enactments. SECTION 8: SEVERABILITY OF PARTS OF RESOLUTION. It is hereby decl are d to be t he int ent ion of the Council that the sections, paragraphs, lines, sentences, clauses and phrases of this r esol ution are severable, and if any phr ase, clause, sentence, line, paragraph or section of thi s resoluti on shall b e declared unconsti t u t i onal by the valid judgment or decree of a court of compet ent j urisdiction s uch unconstitutionality shall not affect any o f t he remaining phrases, clauses, sentences, l ines, paragraphs and sections of this resolution. SECTION 9 : EFFECTIVE DATE. This Resolution shal l take effect and be in force as o f September 1, 2011. An y payment of compensati on or benefits made between September 1, 2011 and September 6, 2011, consistent with this resol ution is hereby ratified and approved.
