WEST FORSYTH HIGH SCHOOL PTO SCHOLARSHIP The West Forsyth High School PTO will be awarding three Scholarships in the amount of $500, $400 & $300 each. The recipients must be graduating seniors who plan to continue their education at an accredited college, university, technical or trade school. If you do not have a current PTO membership, you can ONLY join online via our Paypal site. NO paper copies will be accepted at this time, online online payments. The applicant MUST be graduating from West Forsyth High School in May of 2016. 2. The applicant MUST have at least a 2.5 grade point average (weighted). 3. The recipients parent(s) MUST be a current PTO member. 4. All application submission requirements and any attachments, including the required signatures must be submitted as one package. Incomplete applications will be disqualified. Applicants are encouraged to use the specified format. 5. Scholarship money must be applied to tuition, books, room, and board, at an accredited college, university, technical or trade school during the 2016-2017 academic school year. 6. The scholarship check will be made payable to the recipient. 7. If the scholarship recipient does not accept their scholarship or does not graduate, the scholarship will be awarded to another applicant chosen by the scholarship committee. 8. Application must be submitted to the West Forsyth High School Counseling Center by 3:30pm on Friday, April 1st, 2016. Judging: The PTO Scholarship Committee will consider all criteria including area of concentration; academic ability; scholastic honors and awards; extracurricular honors, awards, and activities in both school and community; work experience; and need for financial assistance/unusual family obligations. Recipient Notification: Award of scholarship is subject to recipient submitting verification of enrollment in an accredited institution for the 2016-2017 school year. An award certificate and check will be presented at the West Forsyth Senior Honors Night. Funding: Funds for this scholarship are raised and budgeted annually by West Forsyth High School PTO membership dues.
