How use of this new data source can assist the hospital based registrar? 12 Data Capture and Evaluation a Florida Pilot Project Is there potential for creating a virtual abstract from disparate data streams? 2

Data Capture Data capture via multiple methods CER -- Comparative Effectiveness Research Project Expanded treatment captured by CTR from Florida Cancer Specialists electronic medical record system Florida Cancer Specialist Data submitted via 837 claim feed since July of 2012. Goes back to 2011. 13 Objectives: General Descriptive Analysis To compare summary chemo treatment information from claims records against core treatment records using CER as a gold standard. This will help us answer two main questions 14 Routine capture using consolidated hospital abstracts Registry Core Record Answer Two Questions 1. Can the claims data produce incident Tx data according to NAACCR standards (first course chemo)? Chemo given yes/no 15 2. Can the claims data augment the existing NAACCR standard treatment data? Chemo single/multiple agents Methods for Identifying Study Sample Treatment Consolidation Zexion NAACCR Record Patient-Tumor Linkage 16 Gold Standard CER Dx 2011 Matching TC Record Chemo Treatment by Dataset (N=660) 17 Q1: Can the claims data produce incident Tx data according to NAACCR standards (first course chemo)? 18 Treatment Consolidation Zexion NAACCR Record 21% Chemo Gold Standard CER Dx 2011 45% Chemo 30% Chemo How do they intersect? Core FCDS Incident Tx Data Treatment Data Yes No Total Yes 67 127 194 No 71 395 466 Total 138 522 660 Study sample N=660 70% agreement on Treatment 71 records from core Tx No to Tx Yes Existing FCDS Chemo Tx given went from 30% to 40% Treatment data validated by CER (82%) Limitations: claims records have gaps in services Updated to 40% Chemo 3

Q2: Can the claims data augment the existing NAACCR standard treatment data where treatment is given? Core FCDS Incident Tx Data 19 Incident Tx Data Computed from Records is greater than* Yes No Total Yes 0 19 19 No 26 22 (same code) 48 Total 26 41 67 *NOS to single/multi agent chemo, or single to multi agent chemo Date of Last Contact Data Enhancement 20 94% of matched records updated Treatment Chemo treatment changed by 37% Treatment NOS went down from 31% to 18% 21% Granular Tx detail (chemo agents) Chemo NOS _61_ Chemo Yes 194 NOS at 31% Update NOS only Chemo NOS _35_ Chemo Yes 194 NOS at 18% Two Questions 21 1. Can the claims data produce incident Tx data according to NAACCR standards (first course chemo)? YES! 2. Can the claims data augment the existing NAACCR standard treatment data? YES! Limitations 22 claims data for 2011 was not all inclusive. Pilot project ramp-up approach Core registry record is made up of multiple sources, not necessarily a 1:1 comparison to source claim records. Example: Core Tx yes (multiple facilities) and Tx no (single facility) Treatment window Needs to be defined by site Next Steps Compare chemo agents on claim against CER record to validate granular data Establish treatment timing rules based on site 23 Acknowledgments CDC s Comparative Effectiveness Research project (CDC) Florida Department of Health Florida Cancer Specialists FCDS CER staff Dr. Lynne Penberthy and Mr. Davis Gentry 24 4
