6 Friend for Life Cancer Support Network s Mission To help persons recently diagnosed with cancer and their loved ones navigate the path through diagnosis, treatment and recovery by pairing them with a trained survivor of a similar experience so they can face cancer with someone who's been there.

8 Who We Are: Volunteers 242 Cancer Survivor & Caregiver Peer Navigator Volunteers Current Age: 23 to 91 Age at Diagnosis: 16 to 75 Over 50 different types of cancer DCIS, Stage I-IV Living in Alabama, California, Florida, Georgia, Illinois, Indiana, Maryland, Michigan, New York, Ohio, Pennsylvania, South Carolina, Texas, Wyoming and of course, Kentucky Counties In Kentucky: Boyle, Bullitt, Fayette, Hardin, Henry, Jefferson, Madison, Marshall, Meade, Montgomery, Oldham, Shelby, Spencer

10 Who We Are: Support Seekers 56 yr old male with salivary gland tumor, no other support 28 yr old female with small children living with stage IV cancer 71 yr old male on feeding tube for extended period of time for esophageal cancer 39 yr old female caring for husband with stage III thyroid cancer Deciding between lumpectomy & mastectomy Uncertain about particular chemo drug side effects Homeless Caring for disabled spouse
11 Who We Are Not: Friend for Life is Not A support group Support is provided 1:1 model rather than in a group setting Providing medical advice Peer navigators speak from their experience and do no give suggestions on course of treatment their support seeker should take. Dependent upon location Support seekers are usually matched with a peer navigator based on diagnosis details rather than location

15 How Do We Know Support Is Working? /Mailed letter sent within one week of match made Ensure PN in touch with support seeker Feedback form ed/mailed within 3-6 months of contact To support seeker to ensure good support, additional resources needed, etc. If treatment completed, asked if interested in volunteering To peer navigators to ensure needs met as volunteers Continuing education opportunities available Invited to yearly training of volunteers Informal feedback and check in calls from staff/office volunteers

18 Med/Nursing School Outreach Beginning in 2007 Friend for Life volunteers began participating in conversations with first-year medical students at University of Louisville Similar project in fall 2009 at Galen College of Nursing Now: UL Medical, Nursing & Social Work, Galen College of Nursing Purpose? An effort to improve communication and understanding of patient and caregiver perspectives by future healthcare practitioners.
19 Rural Outreach Received American Psychological Association s Socioeconomic Cancer Related Disparities (SESRCD) grant in summer 2013 Focuses on counties in Buffalo Trace & Big Sandy areas Ads in area newspapers Visited sites and met with persons in oncology field across area Handed out brochures Enhanced by connections already made through Kentucky Cancer Consortium Goal: increase awareness of support offered as well as network with existing programs to fill gaps

21 Commission on Cancer (COC) Guidelines E10 Psychosocial Services: A policy or procedure is in place to ensure patient access to psychosocial services either on-site or by referral. STANDARD 3.2 Psychosocial Distress Screening (2015) Cancer committee develops and implements a process to assess and address the psychosocial distress of patients with cancer. 22 E10: Psychosocial Services Psychosocial services are essential components of comprehensive cancer care and are provided to patients with cancer and their caregivers throughout the continuum of care. These services address physical, psychological, social, spiritual, and financial support needs that result from a cancer diagnosis and help ensure the best possible outcome. Services are available on-site or by referral, a process is in place to make patients aware of them, and their use is monitored. The program provides a copy of the facility-wide or cancer program policy or procedure that ensures access to psychosocial services and identifies the psychosocial services provided either on-site or by referral. Annually, the program identifies in the Survey Application Record (SAR) the psychosocial services that are available either on-site or by referral. An NCIdesignated Comprehensive Care Center Program (NCIP) facility has the option to complete this section of the SAR to display information about available psychosocial services in the CoC Hospital Locator. 23 Standard 3.2 Psychosocial Distress Screening Assessment and Referral: As recommended in the 2007 Institute of Medicine (IOM) report, if there is clinical evidence of moderate or severe distress, the patient s oncology team (oncologist, nurse, social worker, and/or psychologist) is to identify and examine the psychological, behavioral and social problems of patients that interfere with their ability to participate fully in their health care and manage their illness and its consequences. This evaluation will confirm the presence of physical, psychological, social, spiritual, and financial support needs and indicate the need to link patients with psychosocial services offered on-site or by referral. 26 How You Can Help Us Refer cancer patients & their family members to FFL for support Refer cancer survivors & caregivers of persons who have survived cancer to FFL to volunteer Help us identify additional funding streams to keep up with the increased intakes FFL has received a 142% increase in requests for support and information over the past 5 years
