REQUEST FOR QUALIFICATIONS Project Number 105A, 106A City College of San Francisco DSA - Inspector of Record Services for: Balboa Reservoir Development Site Work Academic Joint Use Facility (Revised 5/12/08) City College of San Francisco, Office of Facilities Management hereby referred to as the College is soliciting from interested and qualified Consultants with Division of State Architects certified Class 1, 2, 3 Inspectors of Record, to provide project inspection services for the following projects: Balboa Reservoir Development Site Work and Academic Joint Use Facility. The intent of this Request for Qualification is to select qualified professional(s) for these projects. Project Description: 1. Balboa Reservoir Development Site Work This project consists of the following: Installation of a ground loop geothermal system with approximately 500 wells (each approximately 400 feet deep) and pipe distribution system tied to a future central plant. Imported engineered fill and rough grading to raise the eastern portion of the reservoir to street level and to provide foundation pads for the construction of the Joint Use Facility and the proposed Performing Arts building. The project when completed will provide cooling and heating for all new facilities to be constructed in the reservoir. The estimated cost of this project is $7.3 million and funding for this project is by a combination of resources including the 2001 and 2005 Bonds. 2. Academic Joint Use Facility The Academic Joint Use Facility will be located on the Balboa Reservoir Development Site along Phelan Avenue on the western edge of Ocean Avenue Campus. The Facility will consist of approximately 102,000 square feet of space on three floors with a basement and a mezzanine and will house the following program areas: Community Health, Teacher Preparation, General Education classrooms and faculty offices, miscellaneous music/theater support space and a Central Plant located in the basement of the building. The Facility, designed to be sustainable, will include natural ventilation system heating supported by a geothermal system, day lighting, photovoltaic panels at the south canopy and a green vegetated roof. The estimated cost of the building is approximately $52 million and funding for this project is by a combination of resources including the 2001 and 2005 Bonds. Project Schedule 1. Balboa Reservoir Development Site Work 24 months 2. Response to this Request for Qualifications must be received by 2:00pm May 28 th, 2008. Responses received after this time and/or date will not be considered. All relevant questions concerning the Request for Qualification or Scope of Services shall be directed in writing to the Project Manager at wkwofie@ccsf.edu. All questions must be received prior to 11:00am, May 22 nd, 2008. Answers to these questions will be sent to all prospective consultants. No answers will be given on an individual basis. The College requests that prospective consultants refrain from contacting any other party regarding the work for these projects. The College reserves the right after opening the responses, to reject any or all responses, or to accept response(s) that in its sole judgment are in the best interest of the College. Proposal Format and Submittal Requirements A. Submittal Requirements All prospective applicants must follow the attached Proposal format and all proposed deliverable items. B. Selection Criteria The College shall review qualifications of all applicants for these projects to determine applicant s technical capability and experience to perform the required inspection. The qualification review shall include an evaluation of the certification, education and similar project experience of key staff, project successes and the proposed work plan for the project. The College shall review exceptions taken by applicants and fee schedule shall not be included in the selection criteria. College may consider interviews if considered necessary. C. Selection Schedule Submission of Request for Qualification - May 28 th, 2008 Submittal Review Process May 29 th, 2008 to June 2 nd, 2008 Notification to Applicants for Possible interviews June 6 th, 2008 Selection of Consultant June 26 th, 2008 D. Proposal Format The purpose of this proposal is to demonstrate your firm s ability to provide the required DSA Inspection of Record Services. A major consideration will be given to your experience and expertise in public work inspection particularly higher education institution work with similar scale. 1. Title of Project of Interest 2. Cover letter expressing interest in project 3. Firm Name 4. Firm qualification a. Type of Organization, size, professional registration, certification and affiliation b. Outline of recent public works projects with similar scope of services and scale. Please include examples of success stories that assisted the project team in meeting construction budgets and schedule c. Names, qualification, experiences and classifications of key staff proposed for this project d. Qualification of any proposed sub-consultants or join venture firms, if any e. Names and contacts of reference from related projects f. Current workload and availability 5. Summary of your firm s approach to scope of services defined for each project 6. Description of your organization structure and staffing for this project and identify specific individual who will be interfacing with the College 2

E. Fees, Insurance and Indemnification Fees: 1. Provide an itemized fee/monthly schedule as basis to perform the scope of services in a separate sealed envelope. After the selection of a Consultant, the College may negotiate the fees under which the work shall be performed. Insurance: 1. Consultant(s) will maintain in force, during the full term of the contract, insurance is in the following amounts and coverage: A. Workers Compensation, with Employers Liability Limits not less than $1,000,000 each accident; and B. Commercial General Liability Insurance with limits not less than $1,000,000 each occurrence Combined Single Limit for Bodily Injury and Property Damage, including Contractual Liability, Personal Injury, Products and Completed Operations; and C. Business Automobile Liability Insurance with limits not less than $1,000,000 each occurrence Combined Single Limit for Bodily Injury and Property Damage, including Owned, Non-Owned and Hired auto coverage, as applicable. D. Professional Liability Insurance covering the Consultant s negligent acts and errors and omissions, with limits not less than $1,000,000 each claim and with a deductible of not more than $50,000. Such policy shall continue in effect for four (4) years following the date of acceptance by the District of the completed project for which the Consultant provided services. 2. Commercial General Liability and Business Automobile Liability Insurance policies must provide the following: General Indemnity: A. Name as Additional Insured the San Francisco Community College District, its Officers, Agents, and Employees. B. That such policies are primary insurance to any other insurance available to the Additional Insured, with respect to any claims arising out of this Agreement, and that insurance applies separately to each insured against whom claim is made or suit is brought. To the fullest extent permitted by law, Consultant shall assume the defense of, indemnify and save harmless the District, its board, officers, and employees (collectively Indemnitees ), from any claim, loss, damage, injury (including, without limitation, injury to or death of an employee of the Consultant or its sub consultants) and liabilities of every kind, nature and description (including, without limitation, incidental and consequential damages, court costs, attorney s fees and cost of investigation), that arise directly or indirectly, in whole or in part, from (1) the services under Agreement, or any part of such services, and/or (2) that arise out of, pertain to, or relate to the negligence, recklessness, or willful misconduct of the Consultant, its employees, sub consultants, or agents in the performance (or nonperformance) of services under Agreement, subject to the provisions set forth in the Agreement. 3

F. CITY COLLEGE OF SAN FRANCISCO S RIGHTS AND OPTIONS 1. The College reserves the right to postpone selection for its own convenience, to withdraw or change this Request for Qualifications at any time, and to reject any and all submittals without indicating any reason for such rejection. Withdrawals or changes will be posted to the College s website: www.ccsf.edu/build, as well as other agencies advertising this procurement. The College accepts no financial responsibility for any costs incurred by a Consultant in responding to this Request for Qualifications. Submitted proposals become the property of the District. Additional material submitted will not be returned. 2. As a function of the negotiation process, the District reserves the right to remedy technical errors in response to the RFQ and to modify the published scope of services. Any interpretation of, or change in, this Request for Qualifications will be made by addendum, and shall become part of the RFQ and any contract awarded. The District will not be responsible for any other explanation or interpretation. G. Business Tax Registration Certificate Before any contract can be certified, the Prime Consultant and Sub-Consultants must provide the District with a copy of a current Business Tax Registration Certificate pursuant to Ordinance 345-88. Consultants can register for a current certificate with the Business Tax Division of the Tax Collector of the City and County of San Francisco. The telephone number of the Business Tax Division is (415) 554-4426. SCOPE OF SERVICES The scope of the Inspection services include but not limited to the following: 1. The Inspector shall act as an agent for DSA and for the College on the project site to ensure the project is in compliance with DSA code, specifications and quality control as required of a public works facility. Inspector shall issue correction and stop work notices and notify the College s representative in writing if work does not conform to contract documents. 2. All inconsistencies or suspected/apparent errors in the plans and specifications shall be reported promptly to the College s representative for interpretation and instructions by the College s Consultants. In no case shall the final instructions be construed to cause work to be done that is not in conformity with the approved plans, codes and regulations, specifications unless accompanying documents authorize such changes. Inspector shall cooperate with the College s Consultants, Testing Lab, regulatory agencies and appropriate governing bodies during the observation of the work of construction to ensure compliance with the approved drawings and specifications. Inspector shall request interpretations and clarifications of the approved contract drawings and specifications when necessary from the College s Consultants and refer any received code interpretations that cause deviations from the approved drawings and specifications to the College s Consultants for response. Inspector shall provide routine required reports to the Division of State Architect and to the College. 3. The College shall provide a field office, desk and chair, photocopier, telephone and fax at the jobsite. The Inspector shall be responsible for providing his/her own vehicle, and special equipment, personal computer and related equipment, printer and any clerical support and other goads and supplies necessary to perform services as required by the scope of work. 4

4. The Inspector shall keep a file of approved plans and specifications (including all approved document authorizing changes) on the job at all times, and shall immediately return any unapproved documents to the Construction Manager for proper action. 5. The inspector shall keep a record of certain phases of construction procedures including, but not limited to the following: a. Concrete pouring operations. The records shall indicate time, date and location of concrete placing and the time, date and location of removal of forms in each portion of the structure. b. Welding operations. The record shall include identification masks of welders, lists of defective welds, manner of correction of defects, etc. c. All such record of construction procedure shall be kept on the job until the Completion of work. These records shall be made a part of the permanent records of the Owner. 6. The Inspector shall notify the Contractor, in writing of any deviations from the approved plans and specifications that are not immediately corrected by the Contractor when brought to his or her attention. Copies of such notice shall be forwarded immediately to the College Representative. 7. Failure on the part of the Inspector to notify the Contractor of deviations from the approved plans and specifications shall in no way relieve the Contractor of any responsibilities to complete the work covered by his or her contract in accordance with the approved plans and specifications and all laws and regulations. 8. The Inspector shall inspect and verify Contractor s record documents to ensure that they are updated regularly as applicable. 9. Inspector shall submit, on a daily basis, an activity report to the Construction Manager, including but not limited to the following information as it pertains to work inspected: a. Activities performed by the subcontractors, and areas, where work are performed b. Manpower assigned to each subcontractor and second and third tier subcontractors c. Equipment and materials delivered to the site d. Weather conditions e. Construction equipment and vehicles utilized f. Nature and location of the work being performed g. Verbal instruction and clarifications of the work given to subcontractor h. Inspection by representative of regulatory agencies i. Note occurrences or conditions that might affect Contract Sum or Contract Time j. List of telephone calls made of a substantial nature, including statements or commitments made during the call 10. Inspector shall record any work or material in place that does not correspond with the drawings or specifications, as well as resulting action taken. List any other problems or abnormal occurrence that arises during each day, including notations of any particular lack of activation on the part of the Contractor. Note corrective actions taken. 11. Inspector shall review and monitor subcontractor s construction methods and procedures during all construction activities, including earthwork, concrete placement, steel erections, all finishes, electrical, mechanical, fire alarm, etc. 12. Inspector shall attend all meetings as requested in contract documents and requested by the College, such as billing meeting, specification review, coordination, progress and pre-subcontractor meetings. 5

13. Inspector shall assist the Construction Manager and College in scheduling all required tests and testing laboratory visitations required by the Contract Documents. Inspector shall observe and record dates and times of all test procedures. 14. Inspector shall inspect, verify and document subcontractor s delivered equipment and materials to ensure that they meet submittal and specification. Such inspection must occur with 48 hours of subcontractor s delivery to the job site. 15. Inspector shall submit to the Construction Manager, in a timely manner, a detailed report of request for clarification whenever any corrective changes are necessary in field construction that will result in variance from the drawing or specification as originally issued. 16. Assist in reviewing the Construction Manager s Payment Requests at billing meetings. 17. When the subcontractor s work or a designated portion thereof is substantially complete, the Inspector shall prepare a list of incomplete or unsatisfactory item via a punch list and submit to the Construction Manager. 18. Assist in the review of subcontractor s submittals. 19. At completion of the project, deliver all inspection records and project correspondence to the College. 20. Prior to commencement of work, Inspector shall work with the Construction Manager and the College s Architect to develop an Inspection Plan for the project. 21. All inspection services shall be in conformance with DSA requirements.
