COMMERCIAL AUTO PROGRAM Mt. Morris intent is to write Commercial package policies (Property, Liability, and Auto). There may be cases when only the auto(s) will qualify. Contact the Company to discuss these situations. Rule I Eligibility 1.1 The named insured on the auto policy must be the same as the business name on the Mt. Morris property/liability policy. A. Individual B. Partnership C. Corporation 1.2 The named insured must be age 25 or older with at least three years driving experience. 1.3 Driving Activity Driving records and past loss history must be acceptable for the past five years. See program guidelines. 1.4 Gross Vehicle Weight (GVW) is the maximum allowable total weight of a road vehicle or trailer when loaded. This includes the weight of the vehicle itself plus fuel, passengers, cargo, and trailer tongue weight. This limit must be included for rating the liability portion of the vehicle or trailer. A. Vehicles less than 10,000 GVW will be rated and classified as private passenger type vehicles. B. Vehicles greater than 10,000 GVW will be rated and classified as commercial vehicles. Rule II Ineligible Risks 2.1 Any driver or vehicle requiring a filing. 2.2 Vehicles in excess of 45,000 GVW, unless written in conjunction with an acceptable underlying Farmowner policy. 2.3 Radius of Operations Vehicles operating in excess of 200 miles on a regular basis (contact Company for further consideration). 2.4 Vehicles used for hire. 2.5 Vehicles garaged in other states. 01-14 CAP-1

2.6 Any Rated Individual with a Major Violation A. Major Violations - Felony involving a motor vehicle - Homicide or manslaughter involving a motor vehicle - Hit-and-run, leaving the scene of an accident - Driving under the influence of alcohol or drugs, including implied consent - Driving while license is suspended or revoked - Fleeing or eluding a police officer - Drag racing or similar type racing - Careless or reckless driving - Operating a vehicle without the owner s consent - Open bottle/can of alcohol - Illegal possession of alcohol and/or drugs - Loaning operator s license or registration - Speeding Excess (20 mph or over) Rule III Coverage Options 3.1 Standard Coverages The following are general guidelines for available coverages. Consult the policy declaration page and endorsements for the contract conditions and limits. A. Bodily Injury/Property Damage (BI/PD) All vehicles on the policy must include BI/PD coverage. CSL available $300,000.00 or $500,000.00. B. Medical Payments (MP) Limits available $1,000.00, $2,000.00, $5,000.00, or $10,000.00 (coverage may be rejected). C. Uninsured Motorist (UM) CSL available $300,000.00 or $500,000.00. D. Underinsured Motorist (UIM) CSL available $300,000.00 or $500,000.00. E. Comprehensive Deductible options available: $100.00, $250.00, $500.00, $1,000.00, and $2,000.00. Not available on vehicles 15 years or older. Cost New is the value of a vehicle when it was originally purchased (MSRP). This limit must be included for rating the physical damage portion of the vehicle or trailer. F. Collision Deductible options available: $100.00, $250.00, $500.00, $1,000.00, and $2,000.00. Not available on vehicles 15 years or older. Cost New is the value of a vehicle when it was originally purchased (MSRP). This limit must be included for rating the physical damage portion of the vehicle or trailer. (Contact Company to discuss coverage options.) 3.2 Additional/Optional Coverages A. Towing and Road Service - Vehicle must carry full coverage - Not available on vehicles 15 years or older - Limit $100.00 per scheduled vehicle - Limited to private passenger type vehicles only B. Rental/Loss of Use - Vehicle must carry full coverage - Not available on vehicles 15 years or older - Limit $50.00 per day, $1,500.00 per occurrence; per scheduled vehicle - Limited to private passenger type vehicles only C. Gap/Loan/Lease Payoff Coverage - Auto Loan/Lease Coverage may be provided only to a vehicle carrying full coverage that is a private passenger type vehicle, and - The insured must request the Auto Loan/Lease Coverage within 30 days of leasing or financing the newly purchased vehicle and pay the premium due. D. Hired Auto Liability Provides liability coverage for short term/leasing exposures. E. Non-Owned Auto Liability Provides liability coverage to an employee s auto while used in the course of business. F. Drive Other Car (DOC) When the only vehicle for a families use is a vehicle owned by a company or the business auto policy is the only auto policy the family has, the family doesn t have the broad coverage of the personal auto policy available to them. This endorsement will allow for the broadened coverage. G. Broadened Pollution Coverage This coverage provides a limit of liability to cover pollution arising in, on, or being hauled by a covered auto. 01-14 CAP-3

Rule IV Binding Authority 4.1 Agents have binding authority up to $500,000.00 CSL for an acceptable auto policy. Risks requiring higher limits, must obtain an Umbrella policy. 4.2 Risks with driving activity or young drivers may only qualify for lower liability limits (contact Company). Rule V Commercial Usage Categories 5.1 Service A. Any eligible class auto that does not do delivery. B. Transporting the insured s personal tools, equipment, and incidental supplies to or from a job location. C. Farm vehicles eligible for the Commercial Program. 5.2 Retail Delivery Vehicles used to pick up property from and deliver property to individual households (florists, dry cleaner, etc.). 5.3 Commercial Delivery Vehicles used to deliver items to other businesses in the course of a day (business to business). Rule VI Commercial Auto Rating and Guidelines 6.1 Rating A. The Mt. Morris Commercial Auto Program is a single tier program for qualifying autos. See program guidelines for details. B. If an account does not qualify under the program guidelines, contact Company to discuss options. 6.2 Commercial Auto Guidelines A. Eligibility Driving Record - Any combination of accident involvements or minor moving violations limited to two in the past three years (per driver). - Any combination of incidents limited to three in the past three years (per policy). - Majority of vehicle usage less than 200 miles. - No major moving violations in the past five years. - Drivers under the age of 25 limited to two incidents in the past three years. 01-14 CAP-4

B. Eligibility Classes If the commercial risk qualifies for coverage with Mt. Morris on the underlying property/liability policy, the risk will likely qualify for commercial auto coverage (contact Company to discuss unique situations). Vehicles that do not qualify for coverage: - Emergency Vehicles - Towing Operations, Tow Trucks - Race Cars, Vehicles Used in or for Competitions - Dune Buggies - Altered Vehicles (Custom Equipment is not Considered Altered) - Street Rods, Custom Cars, Kit Cars C. Individual Risk Premium Modification (IRPM) The Company reserves the right to credit/surcharge a new or existing policy based on the risk s own merit (contact Company). Rule VII Definition of Incident 7.1 Incidents are defined as all claims except: A. Vehicle struck while legally parked. B. Vehicle struck while legally stopped for traffic and the insured was not convicted or did not receive a citation. C. The party responsible for the accident reimbursed the insured for the accident or the insured has a judgment against the responsible party. D. Uninsured/Underinsured claim E. Windshield repair (windshield replacement is an incident). F. Loss of use claims G. Towing claims *All other comprehensive claims are considered an incident. 7.2 Incidents are defined as all moving violations except: A. Equipment violations B. Seat belt violations 01-14 CAP-5

Rule VIII Commercial Auto Credits 8.1 Multi-Policy Credit A. 10% credit applies to the commercial auto policy only. B. Both the property/liability policy and auto policy must be in force to qualify for the credit. 8.2 Fleet Credit/Classification A fleet credit will automatically apply on a policy with four or more vehicles. 8.3 Farm Lay-Up Credit A. 30% credit on each farm vehicle in excess of 20,000 GVW with limited farm usage throughout the year. B. Usage of these farm vehicles is limited to seven months a year. These vehicles are principally parked during the winter off season months of December May. 8.4 Claim Free Credits (per policy) Claim Free Credits are based on the total policy premium at inception or renewal provided the Policyholder has completed the following: 3 years claim free 5% discount 7 years claim free 8% discount 10 years claim free 10% discount Prior insurance is required for Claim Free Credit On new Mt. Morris policies we will allow the 5% credit if the loss runs support eligibility guidelines. *Do not count the types of claims listed in Rule 7.1 when figuring the Claim Free Credit. Rule IX Vehicles with Snowplows 9.1 The plow will be part of the described auto and will be covered under the auto policy while it is attached to the described auto. The cost new of the truck plus the cost new of the plow will be added together for one cost new. The comprehensive and collision deductible will also apply to the plow. (Contact Company if coverage is desired for the plow while it is not attached.) 9.2 Snowplowing risks should have both the auto and general liability coverage with Mt. Morris to eliminate gaps in coverage. 9.3 A $100.00 rate will apply to the policy for all snowplow operations with gross receipts up to $2,000.00. Risks in excess of $2,000.00 gross receipts (contact Company). 01-14 CAP-6

Rule X Permanently Attached Equipment 10.1 The cost new of the vehicle plus the cost new of the equipment will be added together for one cost new. The comprehensive and collision deductible will also apply to the equipment. 10.2 The Commercial Auto Liability will provide the BI/PD coverage while the vehicle or trailer is being used as a vehicle. The business Liability Coverage will provide the coverage while the equipment is in operation. 01-14 CAP-7
