Home visits usually include oneon-one lactation consults with a Lactation Consultant providing a feeding assessment and education. The WIC does not routinely offer home visits. Home visits are very limited and usually include delivery of a breast pump and/or basic hands-off breastfeeding triage and information. Nurse Family Partnership: A FREE program for women who are having their first baby. If you enroll, a registered nurse will visit with you in your home throughout pregnancy and continue to visit until your baby is 2 years old. Sacramento County Office of Education (SCOE) is implementing Project Screening, Outreach, and Referral Services (SOARS) to provide FREE screening and referral services for children from infancy through age five who are at risk for developmental delays and/or disabilities. Screening and referral services will be prioritized for children and families served by transitional housing programs, WEAVE and the Sacramento Food Bank. Family Advocates will conduct voluntary developmental screenings for children using the Ages and Stage Questionnaires (ASQ) and ASQ-Social Emotional (SE) through home visits to maximize access to these services. Based on screening results, families will be referred for further assessments. Family Advocates will conduct home visits to develop a Service Plan for families to establish agreed upon goals for the families to access services for children with developmental delays and/or disabilities and other appropriate resources. Sunburst Projects provides FREE support services for youth and families living with HIV/AIDS in Sacramento, Placer, and El Dorado counties. Our medical case management and mental health services, home visitation, school advocacy, and legal support are some of the services provided. We also offer groups that provide opportunities to give and receive support from peers and stay current about HIV/AIDS. The maternal and childhood health program provides mothers living with HIV with valuable knowledge and support throughout pregnancy, labor and delivery.
