Also, if you plan to attend the PTA program at a location different from the one in which you are currently enrolled, you must submit an application to that college. There is a $25 application fee for CWI and NIC. Students who plan to attend the PTA program at the Lewis-Clark State College campus will need to complete an application to North Idaho College. Application to the PTA program- This application must be completed and submitted by the time the application is due to be considered for the PTA program. Unofficial high school transcripts must be submitted with the PTA application. College Transcripts- Official college and military transcripts must be submitted as part of the application process for the college you are planning to attend. Only courses that appear on official transcripts will be used to determine points for admission. Applicants using test scores (AP or CLEP) to meet course requirements must submit a paper copy of the scores if the course does not appear on the transcript. Unofficial college and military transcripts must be submitted with the PTA application. Applicants with prerequisite courses in progress must include documentation with the PTA application from the instructor concerning satisfactory progress. Proof of 50 hours of observation time- Please submit proof of completion of observation or experience in physical therapy hours. You will need a minimum of 50 hours of observation/experience time. Observation time sheets can be found on the website for the program. Observation must be performed with a licensed physical therapist or assistant. If you have been employed in a physical therapy setting, please submit some form of documentation of hours worked. Also document any previous relevant education you may have had in EMT, CNA, athletic training or massage therapy. Letters of Recommendation- 3 letters of recommendation are required for application to the PTA program. These letters will be used as character reference. Must be submitted either in a sealed envelope with the application packet or submitted directly to the program director by the author of the letter. Program requirements Criminal Background Check Upon acceptance into the PTA program, a criminal background check will be required for participation in clinical rotations. Violations which appear on the background check may result in denied access to clinical sites, therefore, inability to complete the PTA program. Violations may also preclude licensing. Immunizations You will be required to submit documentation of immunizations in order to complete the clinical portion of the program. Below you will find the immunizations that must be submitted prior to beginning clinical affiliations. Physical Therapist Assistant Program Application procedures The PTA program admits students based on a point system, with points gained from previous coursework and other criteria as indicated below. A greater number of points increases the likelihood of admission. The admissions process will also contain an interview component for the top-ranked applicants. These components will allow the admissions committee to select applicants which have a high probability of success both in the program and in the profession. A grade of C or better in all general and pre-requisite coursework is required to be considered for application. An overall GPA of 2.5 is needed for acceptance and progression in the program.
