COLLEGE / COMMON APPLICATION SPECIFIC NOTES: DISCLAIMER: These following notes are based on previous years. Colleges may change their procedures, and if a college is not listed, it still might have some special requirements EACH OF THE COLLEGES LISTED HAVE ADDITIONAL REQUIREMENTS. Therefore, you must check on the website of each college to which you want to apply to see what the current procedures are. Boston College: Requires results of either a). the SAT Reasoning Test and any two SAT Subject Tests (except for the SAT Writing Test), OR b). the ACT taken with the optional Writing Test. Brown University: Requires results of either a). the SAT Reasoning Test and any two SAT Subject Tests (except for the SAT Writing Test), OR b). the ACT taken with the optional Writing Test. The University of California (confirmed specifically with Berkeley, Davis, Los Angeles, San Diego. This does not include the University of Southern California, or the University of San Diego.): Do not require transcripts until you are accepted. You are to report your grades on the application. Does not accept the ACT or SAT scores from Northside. You must have There is a section on the online application to request a fee waiver. If you are applying online, enter your financial information in this section. The University of Chicago: Waives the application fee for CPS students. You don t have to do anything, just submit your application on commonapp.org. In the Payment section of the Common Application check "Fee Waiver Request," then "Other." You won't be able to do this until you submit the University of Chicago supplement for your application. Prefers to receive the ACT and SAT scores from the testing agency, but will accept them on the transcript. Recommends but does not require SAT Subject tests. Now accepts the Common Application! Columbia University (New York): Common Application: Only the colleges listed on the Common Application website accept the Common Application. Go to www.commonapp.org and click on
Member Colleges and Universities for a list of the schools that accept the Common Application. The Common Application Secondary School report will be sent electronically. To prepare for this, you will have to perform the following steps: a). Set up an account on www.commonapp.org. b). Enter your information, including your current classes on commonapp.org. c). Access your Naviance account, go to My Colleges, and sign (electronically) the Privacy Notice for the Common Application FERPA CONSENT. You will have to enter your commonapp.org username and password on this screen Early Decision Applicants: : If you are applying Early Decision to a Common App school, you must indicate Early Decision BOTH on your account on CommonApp.org and on Naviance. You must also go to the School Forms section of the Common App application for the school, and invite your parent or guardian to sign the Early Decision form online. You will have to enter an e-mail address for your parent or guardian. Your parent or guardian will receive a link for signing the Early Decision Form electronically. If you submit the Early Decision form and your parent / guardian signs the form electronically, the Counseling Office will be instructed to send the Early Decision form electronically. If you don t submit the Early Decision Form on Common App, or your parent / guardian does not sign the form electronically, then the Counseling Office will not be able to send the Early Decision form. This applies only to Early Decision, not Early Action. DePaul University: Please apply using the Common Application. Harvard: Requires SAT subject tests. Illinois Institute of Technology: The University of Illinois at Champaign-Urbana: specific fee waiver from their website (go to www.admissions.uiuc.edu, and search for application fee waiver ), fill in your name, address, telephone number, and date of birth. Bring the form with you when you submit your transcript request, along with your envelope (addressed to the University at the address on the form) and postage.
Do not require transcripts until you are accepted. You are to report your grades on the application. The University of Illinois at Chicago: Please apply using the Common Application. The University of Indiana - Bloomington: Requires a Guidance Counselor Form. You must bring this form with you when you submit your transcript request form. You will also have to bring an envelope with postage. Loyola University Chicago: Requires a letter of recommendation, which can be from a teacher or your Counselor. If you want your Counselor to submit a letter of recommendation, you must request it when you submit your transcript request. This letter will be sent electronically. MIT: Requires a Guidance Counselor Form which you can print only after you have started your application. (NOTE: You do not have to be finished.) You must bring this form with you when you submit your transcript request form. This form can be fax d to MIT. The University of Michigan: University of Minnesota: Will waive the application fee only if you qualify for free lunch and will not honor our fee waiver letter if you qualify for reduced price lunch. You have to download their fee waiver request from their website. The form is available at http://admissions.tc.umn.edu/pdfs/fee_waiver.pdf Print the form, fill in your information and bring it in with your transcript request, envelope and postage. Let the Counseling Office know when you submit your transcript request that you are requesting a fee waiver. Northern Illinois University: Accepts the ACT and SAT scores on the transcript provisionally, but you will have to follow up and have the testing agencies send the scores directly. Northwestern University: Waives the application fee for CPS Students. Use Waiver Code 312, and enter it into Northwestern s supplement. Then once you have submitted Northwestern s supplement for your application, open the Payment section of the Common Application check "Fee Waiver Request," then "Other."
Requests, but does not require SAT subject tests for regular undergraduate admission. NOTE: The HPME program requires specific SAT subject tests. The HPME program requires a separate application, and you must request the application online by December 1. However, if you wait until December 1, you won t have the application in time to submit the HPME Counselor s form and transcript request to make their January 1 deadline. Request the application by October 1. Visit www.feinberg.northwestern.edu/hpme/ to request the application. Ohio State: Penn State: Princeton: Requires SAT subject tests. Purdue: Requires a Guidance Counselor Form. You must bring this form with you when you submit your transcript request form. You will also have to bring an envelope with postage. The University of Richmond: The University of Southern California: Stanford University: Southern Illinois University - Edwardsville: specific fee waiver from their website (go to www.siue.edu, and search for application fee waiver). Fill in your name, telephone number, and date of birth. Sign the form, and bring it with you when you submit your transcript request, along with your envelope (addressed to the University at the address on the form) and postage.
Tulane University: Requires a Counselor s Form. You must bring this form with you when you submit your transcript request form. You will also have to bring an envelope with postage. University of Washington: Washington University in St. Louis: Does accept the ACT or SAT scores on the transcript. They prefer that you have the scores sent directly from the testing agency to the college. Wheaton College: The University of Wisconsin: Requires an online letter of recommendation. specific fee waiver from their website (go to www.admissions.wisc.edu, and click on Apply, and then on Forms and Fact Sheets. ).
