IA EVALUATION BY OBSERVATION (1) Go to Login and click on IA Login. Then: (A) Enter the following information: Evaluation date- enter the date evaluation was performed. Technician ID - enter the TIN of the technician being evaluated. Note: This is the format for the TIN: A123456789-000. Is Technician an active IA evaluator?- check this box if the Technician ID is an active IA evaluator. Tech s Lab ID or Evaluated Lab ID- enter the Lab ID of the Technician being evaluated. Lab ID of IA Evaluator- enter the Lab ID of the IA evaluator. Tech s E-mail address- enter the e-mail address of the Technician being evaluated. Supervisor s Email- enter the supervisor s email address. Qual Area- select the applicable Qualification Area (Material ID) from the pick list by clicking the right arrow. Skip the Split or Proficiency Sample Reference ID field. This does not apply to evaluation by observation. Leave it blank. (B) Click Initiate on the menu bar. IA Program 10 of 55

This is the screen after it has been initiated. (C) Write down the Sample ID. You will need this for the entire process. Select which test codes you want activated by clicking on the test code then click on the menu bar either: a. Activate All- this is use to activate all tests in the list, especially if all applies. b. Activate Single- this use to activate one test at a time, especially when only few of the test methods from the list applies. (Select the test code then click activate single.) c. Deactivate All- this is use to deactivate all tests after all tests were activated with no intent. d. Deactivate Single- this is use to deactivate one test at time, after all tests were activated. (Select the test then click deactivate single.) Note: the equip and qpr test code of the applicable test method must be activated since the equipment is also evaluated in addition to technician evaluation. IA Program 11 of 55

(D) Click Save on the menu bar. (2) Skip Receipt step since this is Auto Receipt. (3) Go to Result Entry and click on IA Checklist. (A) (B) (C) (D) (E) (F) Enter the Technician ID- this is the TIN of the technician being evaluated. Note: This is the format for the TIN: A123456789-000 Click Search on the menu bar. Look at the Sample ID on the screen. If it is not the right Sample ID, click on either Next Sample or Previous Sample on the menu bar until you get to the right Sample ID. Enter the IA Evaluator s TIN on the performed by field. Note: This is the format for the TIN: A123456789-000 Skip the Split or Proficiency Sample Reference ID field. This does not apply to evaluation by observation. Leave it blank. The result entry screen should display the IA Checklists (procedure and equipment). Enter 0, 1 or 2 on the IA checklist. 0 means not applicable, 1 means pass and 2 means fail. If you have an entry of 2, make sure that it is entered in the last row of the step number. Use the notepad for comments, if any. (G) Click either Qualify all or Qualify Test. Qualify all- this is use to qualify all tests in the result entry screen. (Click on any test then click qualify all.) Qualify test- this is use to qualify only one test at a time in the result entry screen. (Click on the particular test then click qualify test.) (H) Note: Qualify all and Qualify test are features of LIMS which highlights test results that are out of bounds, meaning a failing test. This feature gives you a chance to change the test results entered if you inadvertently entered a 2 instead of a 0 or 1. Qualify all and qualify test are optional steps. Click Update on the menu bar. (4) Skip validation process since this is set to auto validate. (5) Go to Approve and click on IA Evaluations. (A) (B) Choose AC or RE from the status drop down list as appropriate. AC means pass, RE means fail. Click Approve. Note: The top approval status field is used when approving all samples listed on the screen. Do not use if approving only a few samples listed on the screen. Note: The approval screen is linked to the Technician Training Record. An entry of RE will put a strike to the Technician Training Record on the particular qualification. IA Program 13 of 55

IA EVALUATION BY SPLIT SAMPLE NOTE: FOR EARTHWORK LEVEL I AND LEVEL II EVALUATIONS: FOLLOW THE NON-LIMS PROJECT INSTRUCTIONS. I. QC/V/IV TECHNICIAN S SAMPLE ENTRY PROCESS FOR SPLIT SAMPLE: Note: This step is for the technicians being evaluated. FOR LIMS PROJECTS: (1) Technician being evaluated enters his split sample in LIMS. (A) (B) (C) If the split sample is a regular production sample, enter it just like a regular production sample using the production material id. If the split sample is a dummy sample, use the IA Material ID (example: IAAGGF, I AAPL1, IALBR). Continue all the way to approval, just like a production sample. OR IA Personnel can log in the technician s split sample, initiate, and process. (The purpose is to have the technician s Sample ID right away. The IA personnel will plug this Sample ID in the IA log in screen s Split or Proficiency Sample Reference ID field. Let the technician continue after log in and complete the process all the way to approval. FOR NON-LIMS PROJECTS: (A) Technician being evaluated doesn t enter his sample in LIMS. This is not applicable. II. IA EVALUATION ENTRY PROCESS: (2) Go to Log in and click IA Evaluation. Then (A) Enter the following information: Evaluation date- enter the date evaluation was performed. Technician ID - enter the TIN of the technician being evaluated. Note: This is the format for the TIN: A123456789-000. Is Technician an active IA evaluator?- check this box if the Technician ID is an active IA evaluator. Tech s Lab ID or Evaluated Lab ID- enter the Lab ID of the Technician being evaluated. Lab ID of IA Evaluator- enter the Lab ID of the IA evaluator. Tech s E-mail address- enter the e-mail address of the Technician being evaluated. Qual Area- select the applicable Qualification Area (Material ID) from the pick list by clicking the right arrow. Split or Proficiency Reference Sample ID- This is the split sample id of the technician being evaluated, from the LIMS production. For LIMS projects: Enter the Technician s Sample ID (LIMS production sample ID) in the Split or Proficiency Sample Reference ID field. For non-lims projects: Skip the Split or Proficiency Sample ID field. This does not apply to non-lims project. Leave it blank. (B) Click Initiate on the menu bar. (C) Write down the Sample ID. You will need this for the entire process. Select which split sample test codes you want activated by clicking on the test code then click on the menu bar either: a. Activate All- this is use to activate all tests in the list, especially if all applies. b. (D) Click Save on the menu bar (3) Go to Receipt and click on Single Sample Receipt. (A) Enter the Sample ID (B) Click Search (C) Enter the rest of the information: Sample No., if any assigned. Destination Lab ID.- this is the Lab ID of the IA Evaluator. Sampled By- is the TIN of the Tech who took the sample. This is optional. You may leave it blank. Sample from- this is Lab ID of the Technician being evaluated. (D) Click Receive on the menu bar. IA Program 17 of 55

(4) Go to Result Entry and click on IA Checklist. (A) Enter the Technician ID- this is the TIN of the technician being evaluated. (B) Note: This is the format for the TIN: A123456789-000 (C) Click Search on the menu bar. (D) Look at the Sample ID on the screen. If it is not the right Sample ID, click on either Next Sample or Previous Sample on the menu bar until you get to the right Sample ID. (E) Enter the TIN who performed the IA Evaluator s Split sample on the performed by field. (F) Enter IA results in the split sample test, and use the test comment field to enter the QC/V/IV technician s non-comparing test results (Example: Did not compare. QC result =3.82). IA EVALUATION BY OBSERVATION/SPLIT SAMPLE NOTE: FOR EARTHWORK LEVEL I AND LEVEL II EVALUATIONS: FOLLOW THE NON-LIMS PROJECT INSTRUCTIONS. I. QC/V/IV TECHNICIAN S SAMPLE ENTRY PROCESS FOR SPLIT SAMPLE: Note: This step is for the technicians being evaluated. FOR LIMS PROJECTS: (1) Technician being evaluated enters his split sample in LIMS. (A) (B) (C) If the split sample is a regular production sample, enter it just like a regular production sample using the production material id. If the split sample is a dummy sample, use the IA Material ID (example: IAAGGF, IAAPL1, IALBR). Continue all the way to approval, just like a production sample. OR IA Personnel can log in the technician s split sample, initiate, and process. (The purpose is to have the technician s Sample ID right away. The IA personnel will plug this Sample ID in the IA log in screen s Split or Proficiency Sample Reference ID field. Let the technician continue after log in and complete the process all the way to approval. FOR NON-LIMS PROJECTS: (D) Technician being evaluated doesn t enter his sample in LIMS. This is not applicable. II. IA EVALUATION ENTRY PROCESS: (1) Go to Log in and click IA Evaluation. Then (A) Enter the following information: Evaluation date- enter the date evaluation was performed. Technician ID - enter the TIN of the technician being evaluated. Note: This is the format for the TIN: A123456789-000. Is Technician an active IA evaluator?- check this box if the Technician ID is an active IA evaluator. Tech s Lab ID or Evaluated Lab ID- enter the Lab ID of the Technician being evaluated. Lab ID of IA Evaluator- enter the Lab ID of the IA evaluator. Tech s E-mail address- enter the e-mail address of the Technician being evaluated. Supervisor s Email- enter the supervisor s email address. IA Program 25 of 55

Qual Area- select the applicable Qualification Area (Material ID) from the pick list by clicking the right arrow. Split or Proficiency Reference Sample ID- This is the split sample id of the technician being evaluated, from the LIMS production. For LIMS projects: Enter the Technician s Sample ID (LIMS production sample ID) in the Split or Proficiency Sample Reference ID field. For non-lims projects: Skip the Split or Proficiency Sample ID field. This does not apply to non-lims project. Leave it blank. (B) Click Initiate on the menu bar. (C) Write down the Sample ID. You will need this for the entire process. Select which test codes you want activated by clicking on the test code then click on the menu bar either: a. Activate All- this is use to activate all tests in the list, especially if all applies. b. Activate Single- this use to activate one test at a time, especially when only few of the test methods from the list applies. (Select the test code then click activate single.) c. Deactivate All- this is use to deactivate all tests after all tests were activated with no intent. IA Program 26 of 55

d. Deactivate Single- this is use to deactivate one test at time, after all tests were activated. (Select the test then click deactivate single.) (D) Note: the equip and qpr test code of the applicable test method must be activated since the equipment is also evaluated in addition to technician evaluation. Click Save on the menu bar (2) Go to Receipt and click on Single Sample Receipt. (A) Enter the Sample ID (B) Click Search (C) Enter the rest of the information: Sample No., if any assigned. Destination Lab ID.- this is the Lab ID of the IA Evaluator. Sampled By- is the TIN of the Tech who took the sample. This is optional. You may leave it blank. (D) Click Receive on the menu bar. (3) Go to Result Entry and click on IA Checklist. (A) Enter the Technician ID- this is the TIN of the technician being evaluated. (B) Note: This is the format for the TIN: A123456789-000 (C) Click Search on the menu bar. (D) Look at the Sample ID on the screen. If it is not the right Sample ID, click on either Next Sample or Previous Sample on the menu bar until you get to the right Sample ID. (E) For the test codes by Observation (test codes starts with QPR*), enter the IA Evaluator s TIN on the performed by field. (F) For the test codes by Split Sample, enter the TIN who performed the test on the IA Evaluator s split sample on the performed by field. IA Program 28 of 55

(G) The result entry screen should display the IA Checklists (procedure and equipment) and Split Sample tests. Enter 0, 1 or 2 on the IA checklist. 0 means not applicable, 1 means pass and 2 means fail. If you have an entry of 2, make sure that it is entered in the last row of the step number. FOR SPLIT SMPLE RESULTS: Enter IA results in the split sample test, and use the test comment field to enter the QC/V/IV technician s non-comparing test results (Example: Did not compare. QC result =3.82). Do not enter anything in the test comment field unless it is a non-comparing result. Click on the Notepad on the menu bar. You may need to use the notepad to enter the Technician s test results. Then click save on the menu bar and exit the notepad. Qualify test- this is use to qualify only one test at a time in the result entry screen. (Click on the particular test then click qualify test.) (I) Note: Qualify all and Qualify test are features of LIMS which highlights test results that are out of bounds, meaning a failing test. This feature gives you a chance to change the test results entered if you inadvertently entered a 2 instead of a 0 or 1. Qualify all and qualify test are optional steps.. IA EVALUATION BY PROFICIENCY SAMPLE (1) Go to Login and click on Proficiency Sample Login. Step 1 is performed by SMO/DMO. (A) Enter the following information. You may need to click on the drop down menu right next to the field. Type- this is the same as Material ID. (Examples of IA Material IDs: IAAGGF, IAAPL1, IAEWL1, etc.). Make sure that you choose the Material ID with the Spec Authority MM Section 5.5-P. The LVL, Year, and Spec Authority fields will be automatically populated once you choose the Material ID. Technician ID- this is the TIN of the technician who receives proficiency sample. Enter the QC, V, and IV personnel TINs first and then the IA personnel TINs last on the row. IA? box- click this box if the technician to run the proficiency sample is an active IA Personnel/Evaluator. Note: Since IA personnel TINs are entered last, you will only need to check the IA Evaluator box of the first IA TIN entered. Once you click process the empty fields will copy the last entry in each column, down to the last TIN. Please see the picture below to see how to enter a QC, V, and IV technicians and IA technicians. (B) Click Process. Note: Print the screen after you process, to have a copy of the Sample ID assigned to the TIN. The technician will need the Sample ID to complete the sample process. (2) Go to Receipt and click on Receive Proficiency Sample. Step 2 is performed by technicians who received the proficiency sample. This is to acknowledge receipt of proficiency sample. (A) (B) (C) (D) (E) Put a check in the checkbox on the left labeled In Lab? to receive the sample. Enter the Technician s e-mail address. Enter the Technician supervisor s e-mail address. Enter the Lab ID from the pick list. This is the Lab ID where the proficiency sample will be tested. You may need to enter the Lab IDs on the other samples if there is more than one sample on the screen, even if you do not wish to receive them yet. Click Receive IA Program 37 of 55

(3) Go to Result Entry and click on Test Entry by Sample. Step 3 is performed by technicians who tested the proficiency sample. (A) (B) (C) (D) (E) (F) Enter the Technician- this is the TIN of the technician who received proficiency sample. Click Search. Look at the Sample ID on the screen. If it is not the right Sample ID, click on either Next Sample or Previous Sample on the menu bar until you get to the right Sample ID. Enter the TIN who performed the test on the performed by field.
