Congratulations on joining John Deere and welcome to the Healthy Directions benefit program. Healthy Directions has three essential components a selection of health plans, tax-advantaged accounts to help you budget and pay for your portion of health care costs and wellness resources to support healthy, productive lifestyles. In a recent evaluation, Healthy Directions ranked among the most valuable benefits programs when compared to companies in similar industries. We are able to provide these valuable benefits because of your willingness to be good stewards of our programs and make wise health and health care decisions. Now, it s up to all of us to ensure Healthy Directions remains the leading program it is today; one that helps you and your family and our company be successful. Together, we have a shared responsibility for managing health care costs. John Deere does its part by offering a variety of health, wealth, and lifestyle benefits to help you along the way. There will be elections you may want to make this year, so be sure to review your options and make informed choices. Elections must be made within 31 days of your employment date. Read on to learn more about the benefits and resources available to you and your family in 2016. 2

What s Inside This guide highlights the variety and value of the benefits we provide to you and your family through Healthy Directions. Step-by-Step: 2016 Benefits Enrollment Below is a list of step-by-step instructions for making the most of benefits enrollment. Step Action 1. Understand your benefit options for 2016. Read this guide. 2. Determine the coverage tier that s right for you. 3. Think about which health care and dental benefit plan options are right for you (or you can opt out of coverage if you have it elsewhere). 4. Decide how much you want to contribute to your HSA, if eligible. 5. Decide if you want to contribute to a Limited Purpose Health Care FSA or Dependent Care FSA. Think about who you want to cover (Employee Only, Employee & Spouse, Employee & Child(ren), Employee, Spouse & Child(ren)). See page 7. See the Health Plan Selection Guide on page 4. To open an HSA and make elections for 2016, you will do so through Fidelity. See page 8. Fidelity offers an online modeling tool to help you decide how much to contribute to your HSA. See page 9 for more details on these accounts. Medical Complete the enclosed enrollment form within 31 days of your date of hire or immediately if you are a John Deere employee with a status change. Call Deere Direct at 1-888-432-3373 if you have questions. 6. Congratulations, you are ready to make your elections! Dental Complete the enclosed enrollment form within 31 days of your date of hire or immediately if you are a John Deere employee with a status change. Call Deere Direct at 1-888-432-3373 if you have questions. HSA Go to netbenefits.com or call Fidelity at 1-800-354-3427 to open an account and make your HSA elections. If you do not complete this step, your contribution for 2016 will be $0. FSA Call Deere Direct at 1-888-432-3373 if you want to contribute to an FSA. If you don t want to contribute, you do not need to call. 7. Gather eligibility verification document(s). 8. Submit completed forms and documentation. In order to determine benefit eligibility and ensure timely enrollment in your benefits, you are required to submit document(s). See Dependent Eligibility and Verification on page 5. *Most records are available from the courthouse of the county of marriage, birth or adoption. Health The health benefits John Deere offers through Healthy Directions deliver exceptional value to help you and your family satisfy your health care needs. What to Know John Deere has a Healthy Directions website, which is a great resource for employees. On the site, you will find the Healthy Directions Information for Employees brochure which includes information about key terms, such as deductible, maximum out-of-pocket expense and coinsurance. Benefits summaries for the health plans are also available to you. Your Summary Plan Description provides more detail on your benefits and eligibility. This information is available on the Healthy Directions web site. Health Plan Selection Guide Which health plan has the lowest total out-of-pocket cost for you? It depends on your coverage tier and the amount of non-preventive health claims you have during the year. 1 If you are in this tier... 2...and you expect health care costs in this range for the coming year, then your... Dependent Eligibility and Verification Eligible dependents include your: Spouse if he or she is not enrolled or receiving benefits from another John Deere-sponsored medical or dental plan Children under age 26* Children who are totally and permanently disabled regardless of age, as long as the disability starts while the child qualifies as a dependent * Eligible children include your biological and legally adopted children, stepchildren and children whom legal adoption proceedings have started. In order to determine benefits eligibility and ensure timely enrollment in your benefits, you are required to submit copies of the following (if applicable): Birth Certificate for you Marriage Certificate Birth Certificate for your spouse Birth Certificate for all of your eligible dependents (adoption decree(s), or formal documentation to show legal adoption proceedings have begun is also acceptable) If you do not provide this documentation within 31 days of hire, you or your dependent(s) will not be enrolled in benefits until the appropriate documentation is received. Healthy Directions: How We Stack Up What if I don t have a copy of my birth certificate or marriage certificate? Most records are available from the courthouse of the county of marriage, birth or adoption. In many cases, you can request copies online, by phone or mail. Can I use something other than a birth certificate for verification? Yes. You can submit a copy of one of the following documents: Adoption Papers Formal paperwork that legal proceedings for adoption have begun Qualified Medical Child Support Orders (QMCSO) Aon Hewitt, a global consulting firm, conducts an independent, third-party analysis of benefits programs. John Deere uses this analysis to benchmark our benefits with similarly-situated companies. The results of this comparison, called Benefit Index, show Healthy Directions is in the top quartile of health benefits offered when considering employee premium contributions, point-of-care costs, and company-provided funds. Dental coverage is separate from the health plan, and maintains separate employee premium contributions, deductibles, and annual/lifetime maximums. You can receive dental services from any licensed dentist; there is no UHC dental network. 6

Wealth The wealth benefits John Deere offers through Healthy Directions provide opportunities for you and your family to save pre-tax dollars for health care expenses and receive contributions from the company to help you pay for health care. What to Know Health Savings Account (HSA) To open your HSA and make elections for 2016, you will do so through Fidelity our HSA administrator Go to netbenefits.com or call Fidelity at 1-800-354-3427 to make your HSA elections Annual IRS limits on how much can be contributed to an HSA: $3,350 (Employee Only), $6,750 (Employee & Spouse, Employee & Child(ren), and Employee, Spouse & Child(ren)) John Deere provides a contribution of $750 for Employee Only coverage or $1,400 for Family coverage, pro-rated depending on your eligibility or date of hire Limited Purpose Health Care Flexible Spending Account (FSA) The contribution limit for the Limited Purpose Health Care FSA is $2,550 Call Deere Direct at 1-888-432-3373 if you want to contribute to an FSA Did You Know To determine if you and, if married, your spouse are eligible for an HSA, follow these steps: First: Verify that you meet the four main HSA eligibility criteria: You must be enrolled in a high deductible health plan such as or MAX You may not be enrolled in another health plan that is not a Health Savings Account-eligible health plan, including Medicare* You cannot be claimed as a dependent on someone else s federal tax return You or your spouse may not participate in a general purpose ** Health Care Flexible Spending Account (FSA) Second: If you are married and your spouse would like to have his/her own individual HSA, verify that your spouse meets the four main HSA eligibility criteria above. If your spouse meets all four criteria, he/she is eligible to open his/her own individual HSA. Please note that only John Deere employees are eligible for the company contribution to the HSA. If your spouse does not meet all four criteria, but you do, you remain eligible for an HSA. HSA eligibility criteria is defined by the IRS and is subject to change. *Exceptions include specific injury or illness insurance, long-term care insurance, and specific dollar insurance, such as AFLAC. If you use military benefits, there will be a three-month waiting period to make an HSA contribution. **A general purpose Health Care FSA is an FSA that is not used with a high deductible health care plan. 8

Health Savings Account (HSA) An HSA offers a tax-favored way for you to pay for current and future qualified health care expenses, including your deductible, coinsurance, and eligible medical expenses not covered by our health plans. Contribute Grow Know John Deere contributes up to $750 (Employee Only) and $1,400 (Family) to your HSA (pro-rated, depending on your eligibility or date of hire) if you enroll in either or MAX You can also contribute pre-tax dollars to your HSA. The IRS limits on contributions from any source are $3,350 (Employee Only) or $6,750 (Family). If age 55 or older in 2016, you can make a $1,000 catch-up contribution You can change your contributions any time during the year If you re enrolled in either or MAX and are not eligible for an HSA, you ll receive the company contribution automatically through your paycheck rather than a company deposit into an HSA. The company contribution will be taxable income The money you and John Deere contribute to your HSA goes in tax free When HSA funds are used for eligible medical expenses, the money comes out tax free Investment earnings on your HSA funds grow tax free Any remaining HSA balance at year s end rolls over; there s no use it or lose it feature Account balance can continue to grow for use on eligible medical expenses in retirement To make HSA contributions, you must actively elect how much you want to contribute per paycheck HSA funds can be used for qualified health care expenses, including out-of-pocket vision and dental expenses New HSA participants need to open an account by the first of the month in which medical coverage begins to receive a pre-tax contribution from John Deere If you don t open your account in time and receive a contribution as a taxable payment, you can increase your payroll contribution election to receive the full tax benefit The HSA is yours to keep; you take the money with you if you retire or leave the company. *To see other eligibility restrictions, please search for Publication 969 on www.irs.gov. Flexible Spending Accounts (FSAs) A Limited Purpose Health Care FSA uses pre-tax dollars to pay for out-of-pocket vision and dental expenses in combination with an HSA. A Dependent Care FSA uses pre-tax dollars to pay for dependent care expenses*. Contribute Grow Know John Deere does not make contributions to FSAs You can contribute pre-tax dollars to either or both a Limited Purpose Health Care FSA and/or a Dependent Care FSA The IRS limit for Limited Purpose Health Care FSA contributions is $2,550 per household; the minimum annual deferral is $48 The IRS limit for Dependent Care FSA contributions is $5,000 per household; the minimum annual deferral is $48 To change contributions during the year, you must experience a qualifying life event, such as a marriage or birth of a child There is no opportunity to invest FSA funds The Limited Purpose Health Care FSA allows you to roll over up to $500 of unused funds.
