Introduction Help your understanding what you should be looking for in the report for the COPD discharge bundle. Give you some prompts in what would be useful to be thinking when you are looking at each screen of the report. Please note: this presentation isn t discussing the data itself and you don t need to be able to read the charts.
Trust Data Submitted Screen. What does it show? Who has submitted data when. The number of patients vs. the total number of patients reported by the hospital on SUS. We can then see the % of patients submitted for every trust each month. What do you need to do? Check you are up to date with the data you have submitted. Look to see the % patient you have submitted. Check to see you are over 80% (Most are getting there!) Look to see the trends and try to understand reasons for variation.
Trust Measure Pass Rate vs. All Trusts. What does it show? How your trust delivered the bundle this month compared to other trusts. This is based on the patients SUBMITTED not the total patients. The CQS and ACS The average for the other trusts. BEWARE: this can be misleading. What do you need to do? Identify the opportunities. Understand which trust are doing well...contact them. Look in context of data completeness. The ACS is the goal identify how you are going to deliver it.
CQS and ACS: A Reminder. CQS = Composite Quality Score is the total number of times the clinical measures were successfully delivered. (Everything added up: How many times each part of the care bundle was given across all patients) ACS = Appropriate Care Score is the number of patients successfully given all measures in the pathway. (The number of patients given all the parts of the care bundle) Example: CQS is 7 as in total, 7 measures were passed. (All green ticks added) ACS is 1 as only 1 patient received all measures. Trust Trends for Passing Measures. What does it show? How the trust has delivered each measure over time. The fluctuations in delivery of the bundle. What variation there has been in delivery of the bundle. What do you need to do? Identify the opportunities; Why was the measure high one month and lower another. What changed? Investigate. The greater the variation the greater the opportunity.
Trust Trend for CQS and ACS. What does it show? How the trust has delivered on CQS and ACS over time. The overall fluctuations in delivery of the bundle. What variation there has been overall in delivery of the bundle. What do you need to do? Identify the opportunities; Why was the measure high one month and lower another. What changed? Investigate. The greater the variation the greater the opportunity. Is your ACS lagging behind your CQS then there is something holding it back, it might be just one thing.
Trust Measure Pass Rate vs. All Trusts. What does it show? How each trust has performed for each measure for the selected month. If you want to know who is doing well for each measure and where you stand this is the place to come. What do you need to do? See where your opportunities are. Identify trusts that are strong and delivery and find out what they are doing differently.
Composite Quality Scores and Appropriate Care Scores for all Trusts What does it show? How your trust compares with other trusts CQS and ACS. Gives an indicator of how you are doing overall versus other trusts. What do you need to do? Are you lagging behind others why? Are you ahead celebrate and share with colleagues But also consider how you can achieve 100% Have you submitted data on at least 80% of patients. Are there some missing?
Comparison of CQS and ACS Over Time for all Trusts What does it show? How all the trusts has delivered on CQS and ACS over time. The overall fluctuations in delivery of the bundle for all trusts. What variation there has been overall in delivery of the bundle. What do you need to do? Identify the opportunities; Why was the measure high one month and lower another. What changed? Investigate. The greater the variation the greater the opportunity. If there are trends affecting many trusts such as seasonal variation in delivery (e.g. because of winter pressures) it will show here.
