Introduction CareFlight is a Registered Training Organisation (Provider number 2762) and trains more than 3000 aviation professionals each year. CareFlight courses in Helicopter Underwater Escape Training (HUET) are held in various major cities and regional centres around Australia, including Sydney, Melbourne, Brisbane, the Gold Coast, Cairns and Townsville. This course meets the requirements of; PMAOHS214B Undertake Helicopter Safety and Escape This course has been specifically designed to meet the ever increasing requirements for safety, awareness of dangers and standards for flight over or near water. CareFlight instructors are highly qualified and experienced operational personnel from one of Australia s largest air medical retrieval organisations. The instructors have experience in a wide variety of emergency rescue and aviation areas, including search and rescue operations, marine safety and survival techniques. CareFlight provides training which allows clients to gain the necessary skills and knowledge to confidently manage a situation where underwater helicopter escape is required. All courses offered by CareFlight can be tailored to meet the specific needs of clients, as well as meeting national training program requirements. The course materials and practical simulation training reflect the environment and situations that you may experience. And most importantly every student on a Careflight training course contributes important funds directly to the medical rescue services provided to everyday Australians by Careflight. Aim of the Course This course aims to provide participants with the skills and knowledge to safely egress an aircraft in the unlikely event of an over water ditching or crash. The course develops a thorough understanding of the events and dangers in these situations. These skills are taught by training in the use of pre-flight safety preparation for overwater flights, correct emergency and exit procedures, rescue and recovery systems, sea survival skills and knowledge and treatment of common medical situations related to sea survival. Who Should Attend the Course This course is relevant to anyone who is involved in the aviation industry. In particular those who are required to fly inland over bodies of water (dams, rivers and lakes) as well as near and over the sea. This course is necessary training for air crew, marine pilots, fire industry, government personnel, media representatives, tourism operators, aviation medical professionals and any person whose job role requires them to fly overwater in either a rotary wing or fixed wing aircraft. Content of the Course The course includes important background information in aviation safety procedures, procedural and skill development in underwater escape techniques through theory and practical simulation training. Theoretical subjects include: Emergency Preparation includes the importance of pre-flight preparation, briefing procedures and safe boarding techniques. Personal Protective Equipment and Safety Equipment includes identifying and using common aviation PPE and helicopter emergency and safety equipment. Emergency Procedures and Drills includes brace positions, personal response control, cockpit/cabin management and orientation. Aircraft Underwater Escape Skills including development of escape plans, operation of primary and secondary exits, dealing with exit hazards and complications. Deploying Life Saving Equipment includes knowledge on how to safely deploy life jackets and life rafts. This subject also covers how to safely board a life raft and life raft management, use of survival equipment and ration management. Sea Survival Techniques includes cold water survival, swimming techniques, HELP and HUDDLE positions, maximizing survival time, flotation and group survival techniques. Search and Rescue Procedures includes techniques to minimise your rescue time including effective use of signaling equipment such as GPS systems, EPIRBS, flares and marker dyes. Helicopter and vessel search and recovery procedures. Medical Safety includes prevention and treatment of hypothermia, hydration and dehydration considerations, along with general survivor well being. Practical skill development includes: Underwater escape from an aircraft simulator Situational Personal Response Management Cockpit and cabin orientation and exit location Lifejacket and life raft deployment Use of life saving appliances Practical training will be given in a variety of ditching situations using world s best practice, utilising exit operations including: Upright submersion 180 degree inversion Night time simulation (Blindfold) Cross cabin exit Obstructed exits Life raft and EPIRB deployment Sea Survival Simulator Training CareFlight s generic underwater escape training simulators have been specifically designed and certified for instructing students on how to escape from a submerged aircraft. The training strategy used is designed to provide an understanding of the many difficulties that maybe encountered when faced with a water ditching. The simulator can carry up to six students, two seated in the front cockpit and up to four in the rear cabin. The front doors are hinged with jettison facility, while the rear doors are sliding. Multiple exit points are provided including push out windows on several simulators to give relevant practice in a range of exit situations. Please note: Due to the practical components of the course it is important that you advise us if you are unable to swim, have a fear of water or confined spaces. These issues will not prevent you from participating in the course but will assist us to ensure your well being during training. The content of each course can be specifically tailored to suit any company or organisations operation. These particular needs can be discussed at time of enquiry. Assessment To gain a nationally recognised Statement of Attainment, students need to demonstrate competence in both the skill and knowledge requirements of this course. This assessment will be conducted through observing skills in making a competent underwater escape from the aircraft simulator in a variety of escape situations. These escape situations will include 180 degree rollover, 180 degree rollover in blackout conditions and 180 degree rollover using a secondary exit. Knowledge of the theoretical components of the course will be assessed through the use of a written questionnaire. Both assessments will be conducted at the end of each component of training. Notification of results will be given at the completion of the day s training. Upon successful completion of the course, students will be issued a Statement of Attainment. Recognition of Prior Learning RPL can be gained in this competency by undertaking both the written and practical assessment activities as outlined above. The written assessment will be provided prior to the commencement of the course. This will be marked by an assessor before organising attendance for the practical component of the assessment. If the student is not successful in completing the knowledge component of the course they will be advised to attend the full course. The RPL component of this course can only be offered if you have attended a CareFlight HUET theory component within the last two years. This policy is due to the currency issues created by the lack of being able to utilise the skills and knowledge contained in this course on a regular basis. Course Group Size A minimum of 12 and a maximum of 24 participants is recommended for each course. This number allows instructors to give each participant individual attention if required. Course Duration and Location The course is conducted in one day and the normal duration is approximately seven hours. Training is regularly conducted in the following locations; Brisbane Sydney Melbourne Canberra Adelaide Gold Coast Cairns Townsville Airlie Beach Mackay Gladstone Thursday Island Course costs vary, pending on training location, but prices start at $599 for a Brisbane course. Course cost and refund policy You will be provided with the cost of the course upon making an enquiry with CareFlight. Course costs include instruction, all practical demonstration aids, morning tea and lunch. To secure a place on a nominated course, clients will need to provide payment in full ten working days prior to course commencement. Late enrollments will be accepted if vacant positions on the desired course are available. Late enrollments will need to be paid in full at time of booking the course. For organisations who wish to book a course for a minimum of 12 participants, a 50% booking payment will be required. Group discounts do apply and clients are invited to enquire about these at the time of booking. Group courses can be scheduled to suit the individual needs of the organisation. The final 50% payment will be required at the completion of the training course. Available payment methods include credit card, cheque and electronic funds transfer (EFTPOS). Payment details will be provided when course bookings are made. Refund Terms Cancellations must be received by CareFlight via email or in writing within seven working days of the course commencement date. Another participant may be nominated to fill the booked position in the event of cancellation. If another participant is not nominated in the event of cancellation, a refund of the course cost minus 15% for administration costs will be given if CareFlight Training Services is notified within seven working days. If CareFlight is unable to deliver the course for any reason participants will be notified within five working days of the course commencement. Participants will be offered an alternative date to attend the course. If the alternative date is not suitable the course cost will be refunded in full. To: CareFlight In consideration of CareFlight permitting me to participate in the Training Course I the undersigned (for myself, my heirs, executors and administrators) agree in favour of CareFlight (jointly and severally) as follows: Liability and indemnity under the Australian Consumer Law Liability 1.1 No exclusion or limitation (a) To the extent that I acquire goods or services from CareFlight as a Consumer, I may have certain rights and remedies (including, without limitation, consumer guarantee rights) that cannot be excluded, restricted or modified by agreement. (b) Nothing in this deed poll operates or is intended to operate to exclude, restrict or modify the application of any implied condition or warranty, provision, the exercise of any right or remedy, or the imposition of any liability, implied or conferred under the Australian Consumer Law or any other statute, the exclusion, restriction or modification of which would: (i) contravene that statute; or (ii) cause any term of this deed poll to be void, (Non-excludable Obligation). 1.2 Exclusion of implied obligations Except in relation to Non-excludable Obligations, all conditions, warranties, guarantees, rights, remedies, liabilities or other terms implied or conferred by statute, custom, or the general law that impose any liability or obligation on CareFlight are expressly excluded. 1.3 Limitation of liability (a) Except in relation to Non-excludable Obligations, and only to the extent otherwise permitted by law, CareFlight's liability to me arising directly or indirectly under or in connection with the Training Course or the performance or non-performance of the services to be supplied by CareFlight howsoever related to the Training Course and whether arising under any indemnity, statute, contract, in tort (for negligence or otherwise), or on any other basis in law or equity is limited as follows: (b) (c) Indemnity (d) (e) (i) (ii) (iii) (iv) CareFlight will have no liability to me whatsoever for any loss, harm, damage, cost or expense (including legal fees) in the nature of special, indirect or consequential loss or damage (including, without limitation, economic loss, loss of contract, loss of profit or revenue, loss of opportunity or loss of data); CareFlight will have no liability to me for any loss of, damage to and/or loss of use of any property in my possession or control; CareFlight will have no liability to me for any personal injury, mental illness, disease and/or death; the aggregate of CareFlight's liability to me is limited to an amount not exceeding the consideration payable to CareFlight for the Training Course I participate in and/or the cost of re-supplying the services supplied to me by CareFlight. In relation to the Non-excludable Obligations, except for goods or services of a kind ordinarily acquired for personal, domestic or household use or consumption (in respect of which CareFlight s liability is not so limited under this deed poll), CareFlight s liability to me for a failure to comply with any Non-excludable Obligation is limited to an amount not exceeding the consideration payable to CareFlight for my participation in the Training Course and/or the cost of re-suppling the services supplied to me by CareFlight. Nothing in this deed poll operates or is intended to operate to exclude, restrict or modify any obligation or duty of Careflight to any participant in the Training Course who is an employee of Careflight including any common law duty of care and any duty or obligation under any law relation to workers compensation. To the extent permissible by law, I release CareFlight from: (i) (ii) all expense, liability, cost, loss, including consequential loss; any loss of, damage to and/or loss of use of property; and/or (iii) any personal injury, mental illness, disease and/or death; that I suffer or incur howsoever arising from my participation in the Training Course and/or the provision of services supplied by CareFlight to me in respect of the Training Course, except to the extent that any of those things arise from the breach of a Non-excludable Obligation under the Consumer Law by CareFlight. To the extent permissible by law, I indemnify CareFlight for all expense, liability, cost, loss (including consequential loss) of any kind that CareFlight suffers or incurs (including liability to a third party and legal costs on a solicitor client basis) to the extent that such expense, liability, cost, loss (including consequential loss) arises from or is contributed to by any act, error or omission of mine. Medical Treatment (f) In the event that I suffer any injury, I give permission for CareFlight to seek appropriate medical attention for me. (g) I consent to and agree to pay for any medical treatment (including transport by ambulance) which is considered in the reasonable opinion of CareFlight to be required for me except to the extent that CareFlight is liable for any such expenses pursuant to any breach of a Non-excludable Obligation. Declaration of fitness and Medical Declaration (h) I acknowledge that I have read the Handbook and understand the information contained in it. (i) I declare that I have been advised that participation in the Training Course can involve strenuous physical activity and may increase the health and safety risks to me, in particular if I am suffering from: (i) (ii) any medical conditions that may be made worse by physical exertion (e.g. heart disease, asthma, some lung complaints) any medical condition that can result in loss of consciousness (e.g. some forms of epilepsy and some diabetic conditions) PMAOHS214B Undertake Helicopter Safety and Escape Student Handbook V1.3 2015 8

(j) (k) (l) (iii) asthma that can be brought on by cold water or salt water mist. I acknowledge that participation in the Training Course involves a heightened risk of injury and will involve strenuous physical activity and stressful situations that may cause mental distress. I have been advised to notify CareFlight if I have any medical condition. I warrant that I have fully disclosed to CareFlight all information that may place me at risk during my participation in this Training Course and that I have truthfully and accurately answered the training questionnaire and any medical questionnaire provided to me by CareFlight. Statement of Understanding (m) I acknowledge that I have read and understood the conditions set out in this deed poll and that I am of lawful age and legally competent to give this waiver, release and indemnity. (n) I acknowledge that the conditions set out in this document are contractual in nature, are intended to have legal effect and are not merely a warning or recital. (o) I have signed this document of my own free will and without any representation or inducement by CareFlight. (p) If any clause or part of any clause of this document is found unenforceable or invalid, then that clause or part of that clause (as the case may be) shall be severed from this document and the remainder of this document (and any clause from which a part has been severed) shall, notwithstanding, that severance continue in full legal force and effect. Definitions In this Application: Australian Consumer Law means the Australian Consumer Law set out in Schedule 2 to the Competition and Consumer Act 2010 (Cth) as given effect under Part XI of the Competition and Consumer Act 2010 (Cth), and under the same or similar provisions of the Fair Trading Act 1989 (Qld), Fair Trading Act 1987 (NSW), Australian Consumer Law and Fair Trading Act 2012 (Vic), Fair Trading Act 2010 (WA), Fair Trading Act 1987 (SA), Australian Consumer Law (Tasmania) Act 2010 (Tas), Consumer Affairs and Fair Trading Act (NT) and Fair Trading (Australian Consumer Law) Act 1992 (ACT) as amended or replaced from time to time. CareFlight means CareFlight Queensland Limited ACN 010 316 462 trading as CareFlight Training Services (including its directors, employees, agents and independent contractors). Consumer means a consumer within the meaning of section 3 of the Australian Consumer Law. Handbook means the Course Information Handbook provided to me in the course of the Training Course. Training Course means the training course conducted by CareFlight, details of which are set out in this Application, which may include theoretical and practical components as outlined in the Handbook.
