NHS Greater Huddersfield CCG is responsible for commissioning doctor services between the hours of 6.30pm and 8am Monday to Thursday and 6.30pm Friday to 8am Monday and on Bank Holidays.

1 Seeing the Doctor Our appointment system for routine appointments is based on 10 minute appointments. Appointments can be reserved in person at the reception desk or by telephoning reception. Advance Appointments with the doctors can be reserved up to four weeks in advance. If you have an ongoing medical problem which the doctor is monitoring he will ask you to return to see him in one of these appointments. For on-going medical problems we urge you to see the same doctor at each visit and reserve your appointment with us well in advance. Same Day Appointments. We offer a limited number of routine same day appointments which are only available to reserve from 8am on the day. These can be booked by telephone or in person. Emergency Appointments. If you consider your illness requires urgent attention and all routine same day appointments have been booked patients will be asked to provide details of the health problem to pass to the doctor for triage. The doctor may then decide to offer an emergency appointment for that day. Emergency appointments are booked for between 5 and 7.5 minutes each and doctors will deal with just the most urgent of problems. We cannot guarantee which doctor you will be seeing for these consultations and it is possible that the doctor will be running late. It is unlikely that ongoing long-term health problems can be managed in these consultations. An adult should accompany children under sixteen. IF YOU ARE UNABLE TO ATTEND AN APPOINTMENT WHICH HAS BEEN RESERVED FOR YOU, PLEASE INFORM THE SURGERY SO THAT IT CAN BE CANCELLED AND MADE AVAILABLE FOR OTHER PATIENTS. Please order repeat prescriptions at reception and not during consultations HELP US TO HELP YOU: You are responsible: Please keep your appointment or give adequate notice that you need to cancel it. Please do not delay the doctor please show consideration to other patients. Remember an appointment is for one person only. Please come to the surgery when not prevented by serious illness or infirmity. A doctor can see many more patients in the surgery than when out visiting. Please be responsible for your own health take appropriate action to prevent ill health and follow the advice given by the health professional. We Welcome Your Comments We welcome all comments on how to improve the service that we provide. If you have a grievance please ask reception about our complaints procedure. We aim to deal with any grievance sensitively and quickly to resolve any problems and to make appropriate changes to our procedures. Please address any comments or suggestions you have to the Practice Manager. Please ask at reception for a Carer s Referral if you answer yes to either of the following: Do you look after someone? Quality Services for Patients The practice does not discriminate on the grounds of race, gender, social class, age religion, sexual orientation, appearance, disability or medical condition. 10

9 You have a right to: Confidentiality - Anything you share with us will be treated in confidence. In order to maintain confidentiality x-ray and test results will only be given to patients themselves or to parents of minors (under the age of 16) if appropriate. Checks will be made to confirm the identity of the caller. Access to a Doctor Urgently - For an urgent medical condition an appointment will be offered within 24 hours. Courtesy - We will show you courtesy at all times. Appointment Times Whenever possible you will be seen within 20 minutes of your appointment time. Where there is likely to be a longer delay you will be informed. Annual Consultations for those aged 75 years and over - We are delighted to see all our patients aged 75 years and over for an annual consultation and appropriate investigation. Consultations every Three Years - We are delighted to see all our patients of 16 years to 75 years every three years for consultation and appropriate investigation. Information about your Health - You have the right to ask questions about your own health and to have your questions answered. Requests for access to your medical records should be made via the receptionist. Smoke Free Environment - The Practice operates a no smoking policy. Students - Occasionally health care professionals ask to observe the work we do. Our patients will always have the opportunity to refuse the presence of an observer to their consultation. 2 The doctors delegate many of their responsibilities to the practice nursing team. If you are able to discuss your needs with the receptionists they will be pleased to advise you whether the nurse will be able to help you. Seeing the Nurse Our Practice Nurse team is led by Senior Practice Nurse Rachel France. Practice Nurse Judy Harvey and Practice Nurse Joanne Ellis help in the provision of the following specialist services. Health Care Assistant You may also be asked to make an appointment to attend the Health Care Assistant, Jennifer Chivers, who assists the nursing staff with their workload. Home Visits The majority of medical problems are dealt with more appropriately at the surgery. We expect all patients to try to attend surgery unless genuinely housebound or your illness makes it impossible to leave your house. If you are requesting a home visit please make your request to the receptionist before 10am on the visit day. If you require an urgent home visit please inform the receptionist of the nature of the urgency. You should always be prepared to give the receptionist some idea of the nature of the problem when requesting the visit. When making appointments or arrangements for home visits you will be asked if you have a preference for a particular clinician.

How to Register with Us We are happy to accept new patients on to our practice list who live within our practice area. New applicants may be required to show proof of address. All new applicants wishing to join the list aged 14 years and over are invited to have a health check with the Practice Nurse to discuss service requirements. The receptionist will make arrangements for this appointment. Children of 13 years of age or under will need to attend with their parent/guardian for the health check. Although patients are registered with the Practice and not an individual doctor they can request a preferred doctor when registering. When registering you will be asked if you have a preference for a particular doctor who might then become known as your usual doctor. Services Available Community Midwives Ante-natal care is shared between the doctors and community midwives. The midwives hold a weekly clinic at the surgery for expectant and nursing mums on Tuesday and Thursday afternoons. They can be contacted at other times for advice on Tel 347770. Minor Surgery Minor surgery including incisions, excisions and injections of muscles tendons and joints is performed by appointment at the discretion of your doctor. Warts are treated in a special clinic after referral by your doctor. Influenza Immunisation In the early autumn we offer appointments for flu vaccination which is particularly recommended for those with heart, lung or kidney disease, diabetes, residents in nursing and residential homes, carers and all patients over 64 years of age. Re-vaccination is required annually to continue the protection. 6

3 Practice Manager Mrs B A Collinge, B.A., is responsible for ensuring the smooth running of the Practice and the co-ordination of Patient, Doctor and Staff demands. Please let her know if you have any particular difficulties or suggestions concerning the Practice. Reception and Administrative Staff There are approximately 15 clerical staff employed by the Practice including receptionists, secretaries and administrative assistants. All staff are bound by the same rules of confidentiality as the doctors and nurses. They are used to handling confidential information. District Nurses and Health Visitors District Nursing teams and Health Visitors are commissioned to support the practice and are based at Mill Hill Community Health Centre, Dalton Green Lane. District Nurse tel 01484 221600 Patients who are confined to their homes and need nursing assistance may arrange with their doctor for the district nurse to visit. Health Visitors tel 01484 467615 Health Visitors are available to give advice on family health. Access to Patient Information Access to information about you is restricted to those who work in the practice who must know about you to help provide you with the health care you need. Consent will always be obtained prior to any of your details being passed to any person outside the practice. Both from a health and personal identification point of view we treat your personal details with the utmost security. Sickness Certificates The first week of any sickness does not require a doctor s certificate. A form SC2 is available from the surgery for self-certification. If, after this week, you continue to be unfit for work you will need to consult a doctor to obtain a sick note. Should you remain unwell when this note runs out please inform the receptionist who will enquire if a further certificate can be issued. It is sometimes not necessary to make an appointment with a doctor to request a continuation sick note. Tests and Investigations If we send you for a test or investigation you will need to contact us approximately 2 weeks after having had the procedure so that we can tell you the results. Alternatively, you should make an appointment for a review of your results. Non NHS Examinations and Certificates Private medicals, for example for driving and insurance purposes, are performed by appointment outside normal surgery hours. Please request an appointment and details of charges. Passport applications, private prescriptions, insurance certificates and private health care claims are not available on the NHS. Please be prepared to pay for these. Receptionists will be happy to quote for the service you require. A deposit of 15.00 must be left with us when requesting this service. Consent to Treatment You will be required to confirm your consent to be treated particularly in the cases of minor operations and injections. You may be requested to sign your consent on a special form or in your medical records. Consent for treatment of children will be required by the parent. Training GPs Dr Nazareth and Dr Frankland are qualified trainers of GPs. The Practice is therefore known as a teaching practice and has to adhere to additional working standards. From time to time trainee GPs will be working on site offering consultations for patients. They also participate in home visits. The trainees are fully supervised. 7 8

Pneumonia Immunisation Immunisation against pneumonia is available throughout the year. Patients of 65 years of age and over are encouraged to have this vaccination. If you are under 65 years of age but have a chronic illness the vaccination may also be recommeded. New Patients All our new patients are invited to attend a consultation to discuss their health needs. We would encourage all our new patients to arrange this at their earliest convenience. Dental Problems The doctors advise patients to see a dentist for dental problems. We recommend all our patients register with a dentist. Please check the arrangements your dentist has for providing an emergency service or contact NHS 111 on freephone 111. Repeat Prescriptions The doctor may decide that you will require medication on a long term basis and that you should be able to obtain that medication without attending an appointment. This medication will be provided on repeat prescriptions. Treatment issued to you in this way will have a slip of paper attached to the prescription which lists all the repeat items you can order. Hold on to this list as you will need it to order further supplies. To request a supply of this medication you should tick the items on the slip which are required and either post or drop in the slip to the health centre. You can also request your medication by telephoning the receptionist. Please do not request further supplies more than 7 days before your medication runs out and allow 48 hours between the request and collection of the prescription (excluding weekends and bank holidays). When the receptionist deals with your repeat prescription request she will consider your compliance with the doctors instructions for usage of that medication. The doctor will need to know if you are using the treatment more frequently or less frequently than his instructions suggest. Repeat medication is reviewed by the doctor at least annually. It is not usually necessary for patients to attend an appointment for this review. If you are requiring a prescription for medication which you have received from us before but does not appear on your repeat prescription list this is not referred to as repeat medication and it may take a little longer to prepare. Please expect to have to provide details about when, why and by whom you were last prescribed the item. Repeat prescriptions can be ordered using our website www.thewaterloopractice.co.uk. Register to use this service by taking some photo ID and a passport size photograph to reception. You will then be issued with a password to give you access to your repeat medication list for secure ordering of prescriptions. Repeat medication cannot be issued more than 7 days before your previous supply runs out. Repeat prescriptions can be ordered by e-mail to: repeat.px@gp-b85024.nhs.uk Please provide your name, date of birth, daytime contact telephone number and list the items you require. Again, please allow at least 2 working days between placing your order and collecting the prescription. Please note this is not a secure method of transmitting data and patients who choose to use this means of ordering do so at their own risk. You are strongly advised not to send any details which you consider to be confidential. 4
