LPN TO ADN TRANSITION NURSING ADMISSIONS REQUIREMENTS 2015 MISSION STATEMENT The LPN to ADN Transition program supports the mission of the North Carolina Community College System and the mission of Vance-Granville Community College (VGCC). The faculty is committed to providing accessible high quality nursing education to meet the diverse and changing healthcare needs of the service area and to promoting the development of qualified students prepared for the professional role of registered nurse at the entry level. Graduates of this program meet the educational requirements to take the National Council Licensure Examination (NCLE-RN ). ADMISSION PROCEDURES Students are admitted to the LPN to ADN program (A45110L) in accordance with the policies set by VGCC. Each applicant is required to meet the minimum requirements as set by the college. Meeting minimum eligibility requirements does not guarantee admission into the program, only an opportunity for consideration. Completion of the VGCC LPN to ADN Transition program does not guarantee success in passing the National Council Licensure Exam (NCLE-RN ) nor does it guarantee licensure by the North Carolina Board of Nursing or other licensing state. The selection process for this program includes: Eligibility Checklist Completion (must be completed for consideration for the LPN to ADN Transition program), Points Allocation, and Acceptance Status (determined after April 1, 2015). Regulations, procedures, and course requirements are subject to change as necessary and without prior notice. CRIMINAL BACKGROUND CHECKS/DRUG SCREENS: Upon admission to the LPN to ADN Transition program, students are required to complete a drug screen and have a criminal background check. The criminal background will be monitored throughout enrollment in the program, and new charges will be reported to the clinical facility in which the student is participating in a clinical rotation. The clinical agency may deny student participation at the facility in regards to a criminal background check or a drug screen and in this case, the student will be unenrolled from the transition program. Vance-Granville Community College cautions applicants that a criminal background check will be performed by the North Carolina Board of Nursing prior to issuing a license, and a license may not be granted due to findings on the criminal background check. NOTE: This program is offered at the Main Campus in Henderson. ELIGIBILITY CHECKLIST COMPLETION 1. Apply to VGCC. Designate LPN to ADN Transition as the desired program of study. Deadline for application to VGCC for the 2015 LPN to ADN Transition program is January 30, 2015, by 3 p.m. in person or by 11:59 p.m. online. Applicants will be placed into a Pre-Health Curriculum (Associate in General Education-General Science A10300S), until selected into the program. 2. Submit official transcripts of high school graduation or equivalency and all post-secondary transcripts to the Admissions Office. 3. Complete Placement Testing with scores that must meet the requirements designated by the College for enrollment into ENG 111, MAT 080, or DMA 060. Students may be able to waive placement testing with: High school transcript: Students currently enrolled in high school should submit a high school transcript after their senior year first semester grades are posted. High school transcripts may be evaluated for placement test waiver if the student graduated within the past five years. Standardized test scores (SAT, ACT) College credit in transferrable English and Math NOTE: Test scores or developmental coursework must be completed within five years of the beginning of summer 2015 entry date. 1 (Rev. 10/14)

4. Complete a high school or basic post-secondary Chemistry and Biology course with a grade of C or better. 5. Have a current, active LPN permanent license with no charges or discipline against license from the NC Board of Nursing or other state board of nursing. The LPN license must be maintained with no charges or disciplines against it throughout the program. 6. Submit completed Employment Verification Form documenting continued work for at least six months as an LPN. 7. Complete the ATI Step exam by February 27, 2015. A fee will be charged for the exam, and only one attempt is allowed. The exam must be scheduled and paid for in accordance with instructions from the advisor. The required score for entrance to the LPN to ADN Transition program on the ATI STEP exam is 70%. A practice exam for the test is available for purchase at www.atitesting.com. Applicants are encouraged to purchase and use the online practice exam to provide the best chances of meeting the required score. 8. Achieve a program-specific GPA of 2.5 or higher as calculated on February 27, 2015. A grade of "C" or better is required for all general education courses. BIO course(s) required for the program must be completed within 10 years of the beginning of summer 2015 entry date. 9. Complete the TEAS exam by February 27, 2015. A fee will be charged for each exam. All TEAS exams are administered only on the Main Campus and only on specified dates during the year. Visit www.vgcc.edu/testingcenter/testing-allied-testing-teas.cfm for more information. Appointments may only be made after Items 1-4 under Eligibility Checklist Completion are completed. The TEAS V study guide may be purchased in the bookstore or by visiting www.atitesting.com. TEAS scores must be completed within five years of the beginning of summer 2015 entry date. Official copies of TEAS test scores completed at other testing sites may be accepted if received in Health Sciences Admissions by 3 p.m. on February 27, 2015. 10. All indebtedness to the College must be paid by 3 p.m. on February 27, 2015. 11. The student must complete the LPN to ADN Transition Eligibility Checklist and submit to Health Sciences Admissions by February 27, 2015, at 3 p.m. to be considered for Summer 2015 admission. ADMISSION POINTS ALLOCATION A points allocation formula will be used to rank students for admission utilizing the highest adjusted individual total score on the TEAS exam, BIO 168 grade, HSE/AHSD/High School unweighted GPA, and residence in the four-county service area. The students with the highest ranking will be eligible for admission. ACCEPTANCE STATUS - (Determined after April 1, 2015) Accepted Not Accepted NOTES: 1. Any student completing the Eligibility Checklist but not admitted into the program this year must complete a new Eligibility Checklist for entry into the LPN to ADN Transition program the next year if interested. 2. It is mandatory for the student to inform the Admissions Office of any changes occurring since the date of application in order for corrections to be made to phone number, address, etc. 3. After acceptance into the program, a grade of C or better is required for all general education courses and a grade of B or better for all NUR courses. 4. Students with Post Office Box addresses must provide proof of county residence by January 30, 2015 by 3 p.m. Students who complete change of address forms into the four-county service area by the January 30, 2015 application deadline will receive county of residence points. 5. Completion of all requirements for admission does not guarantee acceptance into the LPN to ADN Transition program. 6. Applicants who do not meet the required entrance STEP exam score of 70% will be eligible for admission to the Associate Degree Nursing Program if they so desire. A change of major form and ADN Eligibility Checklist must be completed by March 31, 2015, at 5 p.m. A current LPN license will be required in place of the CNA I certification. 7. If there is a tie in total points, students will be accepted in the order their fully-completed Eligibility Checklist to the LPN to ADN Transition program is received in Health Sciences Admissions. 2

8. BIO 165 and BIO 166 may be substituted for BIO 168 and BIO 169 only if the series is completed prior to enrollment in the Transition Program. BIO 168 and BIO 169 are preferred for this program. 9. Students will have a maximum of two (2) admission attempts in the LPN to ADN Transition program. Failure to turn in admission paperwork as required, withdrawing from an NUR or related course, or receiving a "C" or lower in an NUR course or a "D" or lower in a general education course will result in dismissal from the LPN to ADN Transition program and will count as one admission attempt. 10. All related general education courses not completed before acceptance into the LPN to ADN Transition program must be taken before or during the semester in which they are listed in the course sequence for the program. Failure to do so will result in dismissal from the program. BIO 168, ENG 111, and PSY 150 must be completed prior to program admission. 11. The majority of classes, labs, and clinical assignments are offered during the day hours, Monday through Friday. However, due to instructor and clinical site availability, some students will be scheduled for a combination of evening and/or day classes and weekend clinical assignments. The proximity of students' homes in regards to the distance traveled to the clinical site will not be a determining factor in the decision as to where students will be placed for clinical. The final decision for clinical sites may not be available until the semester begins. 4

Deliver your completed form to Health Sciences Admissions after completing Items 1-4 and one TEAS attempt. Any subsequent TEAS scores will be added if attempted through February 27, 2015.
