PRUmyhealth Comprehensive whole life coverage for 69 Major Disease Conditions 1 including 17 Advanced Claim Disease Conditions PRUmyhealth ( the plan ) is available to those aged 1 to 65 5 (age next birthday), offering total care through every stage of life. The plan provides you with comprehensive whole life protection against 69 Major Disease Conditions 1, including 17 Advanced Claim Disease Conditions. The most common ailments, such as cancer, heart disease and stroke, are all covered under the plan. If the Life Assured is diagnosed with any of the covered Major Disease Conditions (except for the 17 Advanced Claim Disease Conditions), a lump sum Major Disease Benefit 1 will be payable. Emergency cash for immediate treatment from Advanced Claims Recognising the stress associated with contracting a major disease, the plan covers up to three claims for 17 specific Advanced Claim Disease Conditions to give you immediate support in seeking appropriate medical treatment at the earliest stage of the disease. Advanced Claim Disease Conditions are divided into three groups, Group One, Group Two and Group Three. With a threeclaim total limit, policyholders may claim up to twice in Group One or Group Two, which include Carcinoma-in-situ 6 and Coronary Angioplasty 7, and once in Group Three, which covers many important child and juvenile disorders. Advanced Claims are limited to a total maximum amount of US$90,000/ HK$720,000 per Life Assured, and a maximum of US$30,000/HK$240,000 8 for each group of disease per Life Assured. Please refer to the Benefit Summary stated in this leaflet. When we calculate the maximum amount payable under Advanced Claims, we will count the payment of Advanced Claims of all PRUmyhealth policies covering the same Life Assured. The payment of Advanced Claims of other major illness plans you have with Prudential Hong Kong Limited ( Prudential ) will not be counted. One-off extra benefit of 20% of current sum assured 11 on selected Major Disease Conditions 2 In case the Life Assured is diagnosed as having Cancer, Heart Attack or Stroke, whether (i) it is the first diagnosis with the condition reaching the level of severity stated in the policy provision; or (ii) the Major Disease Benefit on Cancer, Heart Attack or Stroke has been paid 9 and the condition of the same Major Disease deteriorates to an extent that fulfils the definitions as set out in the policy provision within the specified period 10 following the first diagnosis of respective Major Disease, an additional benefit of 20% of the current sum assured 11,12 will be provided to give you extra peace of mind. Similarly, if being diagnosed as having Alzheimer s Disease, Parkinson s Disease, or Multiple Sclerosis on or before age 61 (age next birthday), 20% of the current sum assured 11,12 will be provided to help you overcome this difficult time. Free 10-year crisis cover term 3 provides you with additional protection of 35% or 50% of basic sum assured for the first 10 policy years The Free 10-year crisis cover term 3 offers Life Assured from age 1 to 18 (age next birthday) an additional benefit equivalent to 50% of the basic sum assured of the PRUmyhealth in the first 10 policy years for added protection. For Life Assured from 19 to 65 (age next birthday), it provides extra 35% of the basic sum assured. Furthermore, if you would like to extend this temporary coverage to a permanent one, you have an option to convert the Free 10-year crisis cover term 3 into a new life insurance policy with cash value (which provides coverage on life and/ or critical illness), without the need to provide evidence of your health.

Level premiums 4 with 6 premium payment terms starting from 5 years The plan provides a wide range of premium payment terms for your selection. You can choose to settle all premiums while you have income and then enjoy comprehensive protection even into your retirement. For your convenience, you have the choice of paying in HK dollars or US dollars 14. The premium structure of this plan is level but not guaranteed 4. 1) Benefit Protector Option 15 to suit your needs To guard against the pressure of inflation, the plan offers a Benefit Protector Option 15 (not applicable to the Free 10-year crisis cover term 3 and any other supplementary benefits 16 ) that allows you to automatically increase your initial sum assured at policy issuance by 5% every year up to 200% of initial sum assured with additional premium payments. Further proof of insurability is not required. Financial flexibility and protection in one plan For your broadest financial flexibility, the plan provides up to 80% of its Guaranteed Cash Value as a policy loan 13, and also provides a lump sum Surrender Benefit upon policy surrender. In the event of the Life Assured s death, the Death Benefit will be paid to the beneficiaries, giving them extra financial support during this difficult time. 2) Total care with optional supplementary benefits for your selection Based on your individual needs, you can add a wide range of supplementary benefits 16 to the plan, with additional premium payments, to cover yourself against extra medical expenses, total and permanent disability, accidents and other unexpected occurrences. Please refer to the policy provision for the exclusion clauses.

PRUmyhealth Notes: 1. The Major Disease Benefit is payable according to the definitions which includes but is not limited to the age limit specified in the policy, and it is payable once only for each policy, except for the payment of Advanced Claims for the 17 Advanced Claim Disease Conditions, and the policy will be terminated thereafter. 2. Selected Major Disease Conditions include Alzheimer s Disease, Parkinson s Disease, Multiple Sclerosis, Cancer, Heart Attack and Stroke. The Major Disease Extra Benefit is payable if the definitions as set out in the policy provision are fulfilled. The benefit is payable once only for each policy and the benefit will be terminated thereafter. 3. The Free 10-year crisis cover term benefit amount is equivalent to 50% of the basic sum assured of PRUmyhealth crisis lifelong care for Life Assured from age 1 to 18 (age next birthday) and 35% of the basic sum assured for Life Assured from 19 to 65 (age next birthday). It excludes the sum assured derived from Benefit Protector Option, if applicable. If the basic sum assured of PRUmyhealth is reduced, the sum assured of Free 10-year crisis cover term will be adjusted proportionally. The Free 10-year crisis cover term benefit will not be eligible for any Terminal Bonus and cash values. The benefit of Free 10-year crisis cover term will be terminated upon the termination of PRUmyhealth or on the 10 th policy anniversary date, whichever is earlier. In the event of conversion of Free 10-year crisis cover term to a new life insurance policy, it must be applied within the period of one month before or after the expiry of the Free 10-year crisis cover term, and must meet all specified conditions and administrative rules at the time of conversion. 4. Prudential reserves the right to review the premium rates on each policy anniversary and adjust the premium rates accordingly across a particular risk class (including but not limited to gender and smoking status) but not to an individual customer. 5. The Life Assured must be at least 30 days old when the proposal document is signed. The premium payment term for selection is subject to the issue age of the Life Assured as listed below: Premium Payment Term Issue Age (Age Next Birthday) 5-year 1-65 10-year 1-65 15-year 1-60 20-year 1-55 Until age 55 1-50 Until age 65 1-60 6. To be eligible for a second claim under Carcinoma-in-situ, the second claim must be a Carcinoma-in-situ of one of the 13 specified organs that is different from the organ of first claim. The second claim for Carcinoma-in-situ will be counted towards the maximum of 3 Advanced Claims per policy. 7. To be eligible for a second claim under Coronary Angioplasty, the treatment must be performed on a location of stenosis or obstruction in a major coronary artery where no stenosis greater than 60% was identified in the coronary angiogram relating to the first claim of this illness. The second claim for Coronary Angioplasty will be counted towards the maximum of 3 Advanced Claims per policy. 8. The calculation includes all PRUmyhealth and Free 10-year crisis cover term 3 covering the same Life Assured. 9. In the event the Major Disease Benefit is paid for Cancer, Stroke or Heart Attack and the policy is terminated upon the payment of the benefit, the Major Disease Extra Benefit for the same Major Disease Condition will still be payable if the definition as set out in the policy provision is fulfilled within the specified period 10. 10.The specified period means: (i) for Cancer, within 24 months from the date of the first diagnosis; (ii) for Heart Attack and Stroke, within 6 months from the date of the first diagnosis. 11.The current sum assured is the latest sum assured of a PRUmyhealth policy (excluding Free 10-year crisis cover term 3 ) which takes into account of (i) any increments of sum assured by exercising Benefit Protector Option; and/or (ii) any reduction of sum assured. 12.The current sum assured of Free 10-year crisis cover term 3 is the latest sum assured which takes into account of the adjustment of coverage after any reduction of sum assured of PRUmyhealth (if applicable). Please also refer to note 15 for further information. 13. Interest will be charged on policy loans. It is calculated at an interest rate as solely determined by Prudential from time to time. If the outstanding loan with interest is more than 90% of the guaranteed cash value minus Advanced Claims payment of PRUmyhealth crisis lifelong care, the policy will be terminated. 14.The amount of Terminal Bonus payable will be calculated based on a number of factors including but not limited to claims, persistency and investment assumptions. The Terminal Bonus is declared annually from the 5 th policy anniversary. The amount payable is not guaranteed and is subject to review and adjustment at the absolute discretion of the Company. The amount declared in HK dollar and US dollar policies may be different. 15.The Benefit Protector Option is available only for standard policies of PRUmyhealth with a premium payment term of 11 years or more, and is subject to our administration guidelines. The calculation of the additional premium 4 required for the increased sum assured of PRUmyhealth will be based on the Life Assured s attained age and our latest record of the Life Assured s smoking status. Additional sum assured 11 derived from the Benefit Protector Option is not applicable in calculating the current sum assured of Free 10-year crisis cover term 3. 16.The addition of any supplementary benefits is subject to normal underwriting procedures and additional premium payment. 17.In respect of Autism and Severe Psychiatric Illness, Advanced Claim of the Major Disease Benefit is only applicable for the Life Assured who is a Hong Kong resident and is holding valid Hong Kong identity document as at the date of diagnosis of such Major Disease Condition. If the Life Assured becomes a Hong Kong resident and obtains a valid Hong Kong identity document after the issuance of this policy, the Life Assured is eligible for Advanced Claim of the Major Disease Benefit of such Advanced Claim Disease Conditions only if he/she informs us of the change and obtains our endorsement before the date of diagnosis.

Important Note: (a) Your investment is subject to the credit risk of Prudential Hong Kong Limited. (b) You should not apply for the plan if you intend to surrender prior to the breakeven point of the plan. Surrender of your policy prior to the breakeven point may result in you getting back considerably less than, or none of, the amount you have paid. Please refer to the relevant sales illustration documents for details of breakeven point. Note: The life assurance plans are life insurance products underwritten by Prudential Hong Kong Limited (A member of Prudential plc group) ( Prudential ) and distributed by Standard Chartered Bank (Hong Kong) Limited ( Standard Chartered ). Some of these plans may have a savings element, in which case, part of the premium will be used to pay for the insurance and related costs. A customer who has bought the life assurance plans has a right to cancel the policy within the cooling-off period and obtain a refund of any premium(s) paid less any withdrawals. Provided that no claim has been made, the customer may cancel the policy by giving written notice to Prudential within 21 days after: (a) the delivery of the policy or (b) the issuance of a notice (informing the availability of the policy and expiry date of the cooling-off period) to the customer/his/her representative, whichever is earlier. After the cooling-off period expires, if a customer cancels the policy before the end of its policy term, the actual cash value may be substantially less than the total amount of premiums paid.
