NAME: Date: New Hire and Temporary Employee Safety Orientation Trainees are to initial each box at the time of training. GENERAL RULES AND ACCIDENT PREVENTION 1) No music may be played in the plant at any time. 2) Inappropriate behavior, such as horseplay, fighting, and practical jokes is extremely dangerous and will not be tolerated. 3) Employees that work in the plant are prohibited from wearing loose fitting clothing, items and accessories such as scarves, large jewelry, or outerwear. Long hair should be pulled back. 4) No smoking is allowed at any time inside the building. Employees are only allowed to smoke on break once outside and 25 FEET away from the building in the designated smoking area. Cigarette butts must be disposed of in the proper pails, located away from the building. 5) No eating or drinking is allowed in the filling areas, mixing areas, or anywhere else in the plant at any time, even during break time. Water is permitted, but only if kept in a sealed container. 6) All machinery or equipment is only to be operated by qualified and authorized persons when adequately trained in the use of the equipment. If an employee has been trained but is unsure how to operate any piece of equipment, they must ask their supervisor to train them before attempting to do so. 7) Employees must only use box cutters and utility knives that are provided by management. Always cut away from your body to avoid injury. 8) Employees should refrain from doing any repetitive motion jobs for more than one shift, and positions on the line will be rotated. To reduce excess strain on muscles, employees must avoid manual lifting of materials, articles, or objects which are too heavy. 9) All employees are responsible for cleaning their working areas properly. Trash must be disposed of in the proper bins. All employees must clean up after themselves in the lunchrooms. 10) Employees without batchmaking training will NOT handle any raw materials or go within 5 feet of an actively mixing tank in the batchmaking area, or on the catwalk. 11) Employees shall never enter a confined space (such as a tank or compactor) to do any work or repairs. 12) Proper clean up of a spill is required immediately when it occurs, no matter how large or small the spill is. Never clean up a spill if you do not know what it is. Notify your supervisor immediately that a spill has occurred and wait for instructions from your supervisor. 13) Never pour any chemicals down the drain; all chemical waste must be disposed of in the proper waste totes. 14) Employees are NEVER permitted access to the roof for any reason. 15) If an employee is involved in or witnesses an accident or injury, it is each employee s responsibility to notify their direct supervisor as soon as it happens. 16) Should an employee sustain an injury outside of work hours that could potentially affect their ability to safely complete their regular job duties, they have a duty to report this injury to their supervisor immediately upon returning to EFP and before beginning their next work shift. 17) No employee shall be permitted to remain on premises while under the influence of alcohol or other illicit/recreational drugs or any drug that inhibits their ability to safely perform their job duties. 18) Employees are required to respect each other, and each other s personal property. Discrimination, violence, and harassment will never be tolerated. 19) All visitors/vendors/anyone who does not work for EFP, must check in with reception and wear a visitor badge at all times while on the premises. 2 P age

20) If something appears unsafe, it most likely is! Employees should immediately report any unsafe conditions or equipment to their supervisor. LOCATIONS AND ACCESS All locations will be pointed out at the time of training. 1) In case of a chemical exposure, there are 7 emergency eye washes and 3 emergency showers located throughout the factory. These areas are indicated by the presence of green tape and signage. 2) There are 2 first aid kits and bloodborne pathogen kits in the factory. One set is on the wall outside the Production Manager s office and another on the wall in the production employee break room. 3) For cardiac emergencies, the AED (automatic external defibrillator) is also on the wall, near the Plant Manager s office. 4) The fire alarm is activated by pulling one of the fire pulls located: On the eastern wall of the plant, the northern wall of the plant, next to the front office main exit, between lines two and three. 5) Fire extinguishers are located throughout the factory and are indicated by signage and reflective red tape. 6) Personal Protective Equipment (PPE): PPE storage cabinet between lines two and three, PPE storage cabinet in batchmaking office, labeled drawers in lab. Supervisors will show all employees how to properly wear PPE and which PPE is required for each job. 7) Spill Containment Equipment/Spill Kits: At loading docks, between production lines 2-3, under 2500gal tanks, in raw material stock racks. A small one is also located in the lab. In the west building, spill kits are located at both the north and south entrances. 8) MSDS Binders: Lab above desk in cabinet, Batchmaking office above PPE cabinet, Production Manager s office in flammable closet, Front desk, www.ecos.com. BLOODBORNE PATHOGENS 1) Bloodborne pathogens are infectious microorganisms that are present in human blood and can cause disease in humans. 2) Only first aid trained employees should clean up a bloodborne pathogen spill. Should an employee assist an injured co- worker that is not trained in first aid, they may do so at their own risk (under the Good Samaritan law). Protective goggles, gloves, mask, booties, and apron must be worn when assisting a co- worker with exposed bodily fluids or when cleaning up a bodily fluid spill. Full instructions for clean up are on the outside of each bloodborne pathogen kit. 3) If you become exposed to potentially infectious material, immediately notify your supervisor. You have the right to a free medical assessment and Hep- B vaccination. ELECTRICAL AND MACHINE SAFETY 1) You must inspect all equipment and machinery prior to each work shift to make sure it is in good working condition. If there are any signs that a machine or electrical device has damage, you should immediately notify your supervisor and not plug in or operate the device. If you are uncertain at any time if a device is safe, check with your supervisor before beginning work. 2) Avoid operating electronic devices in wet conditions when possible, and always make sure your hands are dry before plugging in or unplugging electrical cords. 3) Before any electrical equipment is cleaned, it must be turned off and unplugged. 4) Always exercise caution when operating machinery and keep hands away from any loose or rotating parts, moving belts, and rollers. Follow your supervisor s instructions for safe operation of all equipment onsite. 3 P age

5) NEVER tamper with, remove, or otherwise damage any machine s guard. If a machine has a broken or missing guard, notify your supervisor immediately, and discontinue use until the guard is properly repaired or replaced. 6) Do not repair any machinery or electrical equipment without instruction from your supervisor to do so. EMERGENCY ACTION PLAN 1) Upon discovering an emergency situation, immediately notify other employees in the area, including your supervisor (when possible). 2) All exits will remain unlocked and unobstructed during working hours. 3) You have been shown the evacuation maps and understand the fastest and safest way to evacuate the building. You can locate at least two emergency exits that are close to your primary workplace. 4) In case of a fire, extended power loss, or after an earthquake report to the grass on the north side of the building; in case of a severe storm or bomb/terrorist threat, report to the lab; in case of a flood, report to the employee lunch room upstairs. When possible, the nature of the emergency will be communicated over the intercom with instructions of where to report. If you are unsure of where to go, seek instructions from another employee or a supervisor. FIRE PREVENTION 1) Fire extinguishers are only to be used in the case of a small fire from which the employee is able to easily escape from by backing away. 2) To properly use a fire extinguisher : Pull the pin, aim at the base of the fire from 8 feet away, squeeze the lever, sweep side to side. These instructions can be found on the extinguisher as well. 3) ALL flammables are to be kept in the yellow flammables storage cabinets in the batchmaking area and between lines two and three on the production floor when not being used and at the conclusion of the shift each day. On the line, ethanol may only be stored in the approved blue plastic bottles and red safety cans. Rags must be disposed of in the proper metal bins. 4) ALL secondary containers must be properly labeled. Check with your supervisor for all necessary labels. HAZARDOUS COMMUNICATION 1) You have a right to know what hazardous substances you are coming into contact with in the workplace so you may properly protect yourself from the potential negative effects of these chemicals. Please see the one of the chemists for a list of all hazardous material onsite. Material Safety Data Sheets (MSDS) with information on safe handling and PPE requirements, first aid procedures, fire fighting procedures, and labeling requirements are kept for each and every chemical that is warehoused. 2) Hazardous materials are labeled RED throughout the factory in the following places: On every primary drum or container containing the hazardous material, in the MSDS folders, and on the PPE Charts. 3) If an employee is assigned to a new job involving hazardous materials, they will be properly informed of all the associated risks and trained before starting the new job. PERSONAL PROTECTIVE EQUIPMENT 1) It is required AT ALL TIMES that production line workers wear: Steel toed shoes, nitrile gloves, and safety glasses. Back supports must be used when doing any heavy lifting. 2) Disposable filtering face piece respirator use is voluntary. Please read, sign and understand the voluntary respirator document provided at the time of training. 4 P age

3) PPE must be kept in proper working condition. Check your PPE every shift and never use PPE that has visible tears, cracks, or other damage. Ask your supervisor for instructions on disposal of PPE. 4) Materials are color coded around the factory to help indicate the hazards when handling them, and what level of PPE is necessary. White (health 0) and Green (health 1) are the safest materials and do not require special PPE to handle. Additional PPE is required when handling Yellow (health 2) or Red (health 3) materials. Only batchmakers will be exposed to and permitted to handle health 2 and health 3 materials. Flammable materials are also color coded red, as they are hazardous materials, but they may or may not require additional PPE. Check with your supervisor, or reference a PPE chart if you are unsure about what level of PPE is required to handle a material. POWERED INDUSTRIAL TRUCKS 1) Only certified forklift drivers are authorized to operate the forklifts at ANY time. Never go under the elevated part of any PIT, whether it s loaded or empty, or to put any arm or your body between the uprights of the mast. 2) Exercise caution when walking around the factory. Look and listen for forklifts and remain alert at all times. If you are walking behind a forklift, make sure you know for certain the driver can see you. Use the mirrors around the factory to watch for traffic. Never run in the factory! A copy of this program is located in the lab and available upon request for any employee to view. A copy will also be presented to an employee or their representative within 15 days of their request. I have attended, listened, and understood this Safety Rules training. Any questions I might have will be directed to the General Manger, the Office Manager, and/or the Chemist. I understand that translation of this document and all related training is always available in additional languages, as necessary. Should I need translation and training in a second language, I will let the General Manger, the Office Manager, and/or the Chemist know immediately. I understand that it is a part of my regular job requirements to follow all safety rules as outlined above. Failure to do so will result in disciplinary action. I have read and understand the Disciplinary Action Policy. This abridged training serves as a safety initiation for new hires and temporary workers to communicate the basic safety procedures at EFP. This training does not replace the ongoing/annual and in depth safety training that each employee will undergo. No employees shall begin any specialized job, such the positions of Line Leader, Batchmaker, or Forklift Driver, without completing the additional required safety training. Employees that sign below understand that they are to always ask their direct supervisor if they are unsure how to safety and properly complete a job or task. Employees agree to never do any task that is not specifically assigned to them by their supervisor, with instructions on safe and proper procedures. Advisory Information for Employees Who Voluntarily Use Respirators Advisory Information for Employees Who Voluntarily Use Respirators Respirators protect against airborne hazards when properly selected and used. Respirator usage that is required by DOSH or your employer is not voluntary use. With required use, your employer will need to provide further training and meet additional requirements in this chapter. DOSH recommends voluntary use of respirators when exposure to substances is below DOSH permissible exposure limits (PELs) because respirators can provide you an additional level of comfort and protection. If you choose to voluntarily use a respirator (whether it is provided by you or your employer) be aware that respirators can create hazards for you, the user. You can avoid these hazards if you know how to use your respirator properly AND how to keep it clean. Take these steps: Read and follow all instructions provided by the manufacturer about use, maintenance (cleaning and care), and warnings regarding the respirator's limitations. Choose respirators that have been certified for use to protect against the substance of concern. The National Institute for Occupational Safety and Health (NIOSH) certifies respirators. If a respirator is not certified by NIOSH, you have no guarantee that it meets minimum design and performance standards for workplace use. A NIOSH approval label will appear on or in the respirator packaging. It will tell you what protection the respirator provides. Keep track of your respirator so you do not mistakenly use someone else's. DO NOT wear your respirator into: Required use situations when you are only allowed voluntary use. Atmospheres containing hazards that your respirator is not designed to protect against. For example, a respirator designed to filter dust particles will not protect you against solvent vapor, smoke or oxygen deficiency. I have read an understood the above information, and will comply with all of these rules if I choose to use a respirator. I understand that use of a respirator is my own decision. Disciplinary Action Policy Employees are expected to use good judgment when doing their work and to follow established safety rules. Earth Friendly Products has established a disciplinary policy to provide appropriate consequences for failure to follow safety rules. Employees failing to meet performance standards, standards of conduct, rules, regulations, and guidelines, including safety related will be subject to disciplinary action, up to and including termination of employment. Discipline will generally involve counseling, verbal or written warnings, suspension, or termination based on the nature, scope and severity of the violation(s), an employee s overall job performance, past record, as well as an assessment of relevant factors. Consequently, the first offense or violation may involve either counseling, a verbal or written warning, a suspension, or termination of employment. As previously mentioned, the Standards of Conduct Policy, the Employee Handbook, this Disciplinary Policy, or any other EFP policy or procedure does not create a formal progressive discipline system or require that any particular steps be followed prior to counseling, disciplinary action or termination of employment nor does it change the at- will employment relationship between the Company and its employees. I have attended, listened, and understood today s Disciplinary Action Policy meeting. Any questions I might have will be directed to the General Manger, the Office Manager, and/or the Chemist. I understand that translation of this document and all related training is always available in additional languages, as necessary.
