PERSONAL STATEMENT My research focuses on 1) developing, evaluating, and optimizing technology-based smoking cessation interventions that have broad reach to smokers who might not otherwise access treatment, and 2) addressing the unique measurement and methodological challenges in conducting technology-based dissemination and implementation research. I have led and collaborated on a range of smoking cessation trials, with a primary focus on web-, telephone, and online social network interventions. I have extensive experience in the management and oversight of web-based randomized trials, including issues unique to the conduct of Internetand social network based studies such as recruitment and retention, measurement issues unique to online social networks, data safety and monitoring, and human subjects protections. I lead the Schroeder Institute s Software Development Team which has developed and deployed the social media mobile application for young adult smokers and a web-based clinical trials management system that form the foundation for this proposal. Our team has also developed a Facebook smoking cessation app and a dynamic text messaging intervention that operates both as a standalone and adjunct intervention to other treatment modalities. 1. Treatment outcome and mechanisms research in web-based smoking cessation interventions. For the past 15 years, I have conducted programmatic research in web-based interventions for smoking cessation, with a focus on identifying the mechanisms of effectiveness and leverage points for optimizing their reach and impact. This work began with a Phase I SBIR project to develop a web-based smoking cessation medication adherence system (R43 CA097903; Graham, Proposal Lead/Co-Investigator) and a second Phase I SBIR to develop a web-based system to integrate an Internet intervention with quitline counseling (R43 CA099211; Graham, Proposal Lead/Co-Investigator). Both projects developed core infrastructure and features for one of the largest web-based cessation programs that is currently in operation today. This work also provided the foundation for one of the first randomized controlled trials for web-based cessation (The iquitt Study: R01 CA104836, Graham, PI). Cost effectiveness analyses demonstrated that optimal adherence to Internet interventions yielded the highest number of quitters at the lowest cost, and mechanisms research identified sustained treatment engagement as a key mediator of treatment effectiveness. Web-based cessation programs commonly include social media and Web 2.0 applications to facilitate the creation and sharing of user-generated content among current and former smokers. Research that I have led and collaborated on has demonstrated that participation in online social networks for cessation may be a key driver of abstinence. Early publications documented that individuals who participated in an online cessation community were more likely to be abstinent than those who did not and that there was a dose-response relationship between intensity of community engagement and abstinence. In 2010, I co-led the planning efforts of a 2-day conference titled Online Social Networks and Smoking Cessation: Strategic Research Opportunities that we hosted at the Schroeder Institute for Tobacco Research and Policy Studies. The conference brought together more than 100 experts across a broad range of disciplines with the goal of constructing a strategic research agenda for online social network research. A key outcome of the conference was a call for more rigorous and transdisciplinary research to elucidate the mechanisms through which social networks influence behavior change. This is the focus of a current study (R01 CA192345; Graham/Zhao MPI) which examines the social dynamics of smoking cessation in a 6-year longitudinal dataset from one of the largest online social networks for cessation. I have also collaborated on and now lead research that examines the effectiveness of disseminating an evidence-based cessation intervention through smokers existing social networks in Facebook (R01 CA155369). This study represents a model for conducting dissemination research within an open online social network and demonstrates the potential to drive viral spread of an intervention to achieve population-level impact. a.
