General Information about AmeriCorps Internship Position (AIP): Utah Healthcare Corps (UHC) has contributed greatly to the improvement of health outcomes in the areas of immunization, diabetes, reproductive health, and has worked to reduce health disparities for special populations. UHC has been supporting community health centers and their community partners since 1995. We are celebrating our 20th year of service in Utah! This program supports members serving in community health centers and other community health based organizations throughout the state of Utah. UHC seeks candidates to fill 300 hour terms. These positions are in conjunction with BYU's Public Health program. AIPs will work directly with health centers to support uninsured, economically disadvantaged, and medically underserved individuals in utilizing preventive and primary healthcare services through coordinated care management and improved healthcare access in rural and urban areas throughout Utah. AIP Members will specifically: Plan, implement, and evaluate patient satisfaction to improve patient care; Identify patients with health indicators out of compliance and contact those patients to schedule follow up appointments and tests; Assist patients in coordinating follow up appointments and referrals, particularly those patients in need of translation services; Educate patients on available healthcare services; and/or Plan, implement, and evaluate a staff Wellness Program. Requirements for serving with AmeriCorps: 1. Citizen or resident or permanent legal resident

2. Not have murdered anyone 3. Not a registered sexual offender 4. High school diploma or GED or working on achieving either 5. No aggravated assault misdemeanor or felony In addition to hands on experience, the AIP will also receive an education award in the amount of $1212. This can be used to pay off qualifying student loans, used for upcoming tuition or other qualifying education expenses. These are new positions in Provo, Salt Lake City, Logan, St. George and Bicknell. At each of our four locations in Provo, Payson, Wastach (located within Wasatch Mental Health), and Vernal, we work hard to provide the medically underserved with a medical home where they can receive coordinated and comprehensive care. Since its inception, MCHC has provided on average over $3 million dollars annually in discounts to Utah County residents in need of medical services. Currently, MCHC serves nearly 15,500 patients and provides 40,000 patient visits each year. We are extraordinarily proud of the work we do at Mountainlands Community Health Center. Utah Partners for Health understands the importance of a healthy community and have crafted a program that offers full access to primary care services for the uninsured. We help pregnant mothers with prenatal visits to ensure a healthy delivery for them and their children. We offer services so that children receive well care checks and immunizations to attend school, or so when they grow up they can receive physicals or care if they get the flu. In Utah, lack of affordable healthcare is a top concern and one of the leading causes of bankruptcy for working families. With the faltering economy, the number of uninsured and jobless residents is rapidly increasing. Many of the working poor have lost access to coverage through jobs or state/federal programs. At conservative estimates, the number of uninsured residents in Utah is about 350,000. Nine out of ten of these families have at least one working adult, but do not have access to affordable insurance through the workplace. For communities that can t access our services in clinic, our partnered mobile clinic brings access to care directly to them with our mobile events. Bicknell Wayne Community Health Center opened for medical service on April 3, 1978 with one provider and one worker. The project director estimated 3 patients per day would make the project a success. The first day the center opened there were 8 patients seen and the center has continued growing ever since. We are located in a high mountain valley very isolated and far from the city life. Long distances between small townships and wide open country are common in our area. We provide a wide variety of services and serve almost 4,000 patients now. Services routinely provided include preventive, acute and emergency medical care, comprehensive dental services, management of chronic conditions for prenatal, pediatric, adolescent, adult and geriatric patients, and diagnostic laboratory and radiology services. WCHC realizes that health is more than the absence of disease and we have integrated behavioral health services and traditional counseling services into our primary care services at the center.

The clinic has made a tremendous impact since its inception. As of today we have the Medical/Dental clinic and pharmacy in Bicknell, UT. As well as our Kazan Medical Clinic, Pharmacy, and Dental Clinic in Escalante, UT. Helping to save our patients both worry and countless dollars in travel and emergency room costs. Wayne Community Health Center is also an economic engine for our community. We now have over 40 employees and contribute to the local economy in many indirect ways. We are a private, non profit, Federally Qualified Health Center that has been dedicated to serving our communities since 2003. We offer Family Practice, Preventative Health Care, Dental, Behavioral Health Care, OB (Obstetric), and Pharmacy services. With clinics located in North Logan, Logan, Hyrum, Garden City, Utah, and Evanston, Wyoming, it is our mission to help maintain a healthy community through education, prevention, and a community networking system. Currently, BLCHC serves 9,580 patients and provides 24,960 patient visits each year.

We have remarkable providers who are dedicated to providing the highest quality of care, regardless of an individual s ability to pay. St. George Family Healthcare has three sites in Washington County and one site in Iron County, both rural counties in the Southwest portion of Utah. Family Healthcare provides comprehensive, primary medical services including medical, dental, mental health, insurance assistance, and pharmacy services.
