Service Overview and Pricing Guide

Millard Health s Service Overview and Pricing Guide Millard Health provides rehabilitation services for both work-related and non-work-related injuries. The rehabilitation services required for a work-related injury are managed by a WCB claim owner. For more information about services for a specific work-related injury, please contact the WCB claim owner or WCB s Contact Centre at 1-866-922-9221. Assessments Medical Status Examination (MSE) The MSE confirms an individual s current medical status including diagnosis and work restrictions. This exam identifies any requirements for further medical investigations, consultation, treatment or clearance to participate in a Functional Capacity Evaluation. Price: $960 (For head trauma cases: $1,720) Basic Functional Capacity Evaluations (BFCE) The BFCE is a snapshot of an individual s current level of function compared to the demands of their job (what they can and cannot do). This half day assessment helps determine if the individual requires further rehabilitation, treatment and/or services in order to meet the demands of their job and return to work. Price: $825 1 Comprehensive Functional Capacity Evaluations (CFCE) The CFCE is a standardized functional assessment that identifies working abilities and restrictions. The two day assessment will evaluate the individual s safe maximal functional abilities, comment on consistency of performance, outline any relevant work restrictions, and provide recommendations for treatment or return to work. Price: $1,300

Gait Assessment A Gait Assessment evaluates the positioning and functioning of the lower extremities including a biomechanical evaluation and a visual walking analysis to determine the cause of difficulties while walking. Recommendations are then made for optimal functional improvement including exercise prescription and recommendation for provision of footwear and/or orthotics. Price: $650 Independent Medical Examination (IME) A clinical examination performed by an independent medical examiner (usually a specialist) who has not been previously involved in the individual s care and will not be actively involved in the care afterward. The purpose of an IME is to get an impartial clinical judgment that is made based on a review of the medical records and reports that have been submitted. Price: $1,410 (For head trauma cases, please call for a quote) Private Interdisciplinary Team Assessment The PITA is an interdisciplinary team assessment that includes evaluation by a physical therapist, occupational therapist and exercise therapist. The assessment will include a medical history review, musculoskeletal evaluation, fitness testing, assessment of occupational performance and psychosocial issues as well as a functional evaluation. The assessment report will outline all rehabilitation barriers, relevant treatment goals, predicted outcome and any recommendations for treatment. Worksite Analysis Price: $650 (With psychology: $850. With BFCE: $1,300) A Worksite Analysis is a collaborative method to help develop a return-to-work plan that ensures the individual returns to work safely, gradually and in a timely manner. The purpose of a Worksite Analysis is to confirm critical job demands, identify suitable modified work options and develop a specific return-to-work plan. This return-to-work plan is designed to promote recovery and assist with a return to full duties. Price: $985 2

Psychology Assessments Head Trauma Assessment The Head Trauma Assessment is completed by a neuropsychologist or psychologist with experience working with head trauma injuries. The assessment includes an interview with review of the accident history and current symptoms as well as treatment to date. The assessing psychologist will provide education regarding recovery expectations and indicate treatment recommendations to help facilitate recovery. Further recommendations for diagnostic testing or evaluation will also be outlined if required. Neuropsychological Assessment Price: $950 A Neuropsychological Assessment can be used to confirm or rule out brain injury and possibly identify other causes for recent behavioural changes. The Neuropsychological Assessment consists of an interview and standardized psychological testing that includes questions which examine intelligence, problem solving skills, personality, attention and concentration. The assessment also involves a number of tasks which assess a wide range of abilities such as coordination, memory, speech, hearing and academic skills. The assessment is usually scheduled over a one to three day period and will include a summary of test results, comment on the validity of test results, and recommendations. Traumatic Psychological Injury (TPI) Screen Price: $2,750 The TPI Screen is a psychological assessment of an individual following a traumatic incident and who is experiencing symptoms of post-traumatic stress. The TPI Screen incorporates clinical interview and validity testing to provide a working diagnosis and treatment recommendations. Factors affecting recovery and work restrictions are also outlined to assist with return-to-work planning. Price: $1,170 3

Treatment Programs Interdisciplinary Rehabilitation Program The Interdisciplinary Rehabilitation Program brings together a team of professionals to address the unique needs of the individual. The resulting customized programs focus on maximizing functional restoration and return-to-work capabilities. The program also addresses any relevant psychosocial issues and includes referral to applicable psychological and educational workshops. Educational workshops are included with the program. Complex Psychosocial Program Price: $400/day The Complex Psychosocial Program provides solutions for individuals with complex medical conditions where significant pain or psychological issues are barriers to rehabilitation. We focus on self-management techniques that break negative pain and disability cycles. We work with individuals to successfully return to functional independence, increase ability to manage pain and improve quality of life, increase coping skills to manage stress, decrease an individual s dependency on medications and improve sleep patterns, and encourage adoption of healthy lifestyle practices. Educational workshops and psychology groups are included with the program. Head Trauma Program Price: $475/day The Head Trauma Program provides interdisciplinary assessments and rehabilitation services to address the cognitive, occupational, physical and psychological consequences of head/brain injury within a supportive environment. In addition to treatment for the effects of the head trauma, we also offer counseling and education for both the individual and his or her family. Educational workshops and psychology groups are included with the program. Price $575/day 4

Traumatic Psychological Injury (TPI) Program The TPI Program provides interdisciplinary assessments and rehabilitation services to address the psychological, occupational, physical and psychosocial consequences of psychologically distressing injury within a supportive environment. We assess the individual s condition and needs and focus on strategies to manage the symptoms of post-traumatic stress and engage the individual in their pre-injury activities and occupations. We aim to maximize physical and functional abilities while promoting selfconfidence, empowerment and a productive lifestyle. We want to educate their support system and support a return to sustainable employment by removing injury related barriers. Educational workshops and psychology groups are included with the program. Price: $475/day (This price does not include exposure therapy) 5

Individual Treatment Services Acute Physical Therapy The Acute Physical Therapy clinic focuses on treatment during the acute stage of recovery. Modalities include active and passive interventions with an emphasis on functional restoration and return-to-work planning. Price: $150/hr for assessment, $75 per treatment Hand Therapy Hand therapy is a specialized service that provides functional and vocationally focused treatment of complex hand injuries. A hand assessment includes objective measurements and outlines functional limitations along with corresponding treatment goals. Hand therapy provides exercise prescription and application of modalities including custom splinting or recommendations for bracing or adaptive equipment. Price: $350 for an assessment, $100 per treatment Individual Clinical Services Direct one-on-one clinical services provided by an occupational therapist, physical therapist or exercise therapist. Treatment could include exposure therapy, direct physical therapy (acupuncture, needling etc.) or exercise therapy. Assessment and reporting is included. Price: $150/hr Once an initial assessment has been complete, Supervised Exercise is available. This could include general exercise, strengthening, conditioning or work simulation that was prescribed by an occupational therapist, physical therapist or exercise therapist. Please note: progress and discharge reports are considered part of direct one-on-one clinical services and are not included with the Supervised Exercise fee. Price: $75/hr Individual Psychology Services Direct one-on-one services provided by a psychologist. Interventions could include cognitive behavioral therapy, prolonged exposure therapy, EMDR, cognitive processing therapy or psycho-education. All interventions would include an initial assessment outlining specific barriers and goals, regular progress reporting including objective measures and recommendations for further treatment. Price: $195/hr 6

Prevention Services Ergonomic Services Ergonomic Services are available to assess a position or work station and to identify possible improvements to minimize the risk of injury. Recommendations may include; ergonomic accessories, work station set-up, safe work practices, job design or education on correct body mechanics. Proper ergonomics can prevent injury and reduce the costs associated with injuries. Price: $150/hr* (average assessment is two hours) Job Site Education Prevention and sustained return to work are the focus of job site education. Our professionals assist employers and staff with prevention education in areas such as; back and shoulder education, lifting techniques, job specific stretching programs and ergonomics. Price: $500* (for subsequent presentations the cost is $250) Physical Demands Analysis (PDA) A Physical Demands Analysis is a document that outlines all the essential job tasks and the physical demands required to perform a specific position. It is important to create a PDA for each position in your organization. This will help with disability management when determining modified work opportunities and return-to-work planning. Skilled clinicians, experienced in movement/functional analysis and jobsite analysis will complete the PDA s. Price: $150/hr* (average assessment is four hours) 7 Post Offer Pre-Employment Screening (POPE) Post offer Pre-Employment Screening is a tool used to determine whether or not a prospective worker is able to perform the physical demands of the job, safely and consistently. This assessment includes a medical exam and a physical assessment that provides employers with baseline functional capacity levels for their employees. Recommendations are provided as either a pass or fail. Price: $350

Other Services Psychology Groups and Educational Workshops A variety of psychological workshops and groups are available with a psychoeducational approach. Topics include: pain education and pain management, stress and relaxation training, depression and lifestyle management, communication and anger management, sleep management, coping with chronic pain, dealing with anxiety and post-traumatic stress. Educational workshops are also available and topics include: hurt vs. harm, back education and shoulder education. Price: $50 each Vocational Rehabilitation (VR) Services VR Services are delivered by a team of professionals using a client-centered approach. The focus is on an individual s sustainable return to work following a job change or separation due to an accident or injury. Individuals are empowered through one-onone or group assistance in job planning and job search to develop a return-to-work action plan that is suitable, accessible and sustainable. VR Services also provide customized services to meet their vocational rehabilitation needs. Academic Assessment A detailed assessment to help determine an individual s suitability for an academic program or long term retraining. The assessment includes standardized psychological testing and the test results will be reviewed by a registered psychologist. Price: $775 Career Counseling Provided to individuals where career changes and new employment opportunities need to be considered. Specific employment options that are realistic and attainable will be identified with the individual and referral source. Price: $150/hr Job Coaching For individual s needing assistance in returning to a previous position or beginning a new position; job coaching assists with the transition back to work. A job coach provides onsite support to both the employee and the employer and can assist with establishing a gradual return-to-work plan to facilitate a successful and sustainable return to work. Price: $150/hr* 8

Job Search Skills Development Learn employment search skills such as, how to prepare a cover letter and resume, research prospective employers, and fine tune job interview skills and follow-up techniques. Price: $50/workshop Resumé Development /Review A resumé details an individual s goals, skills, experience, education, interests and references in a professional format. A review of your resumé is completed and recommendations are made to enhance the quality. If a resumé does not currently exist, we will also assist in developing one. Price: $300 Vocational Profile Assessment Assesses current skills and abilities including educational background, employment history, interests, hobbies, language ability, and transferable job skills to help identify realistic employment options. Price: $450 Travel Rates *These services require travel and are subject to travel time ($120/hour), and mileage ($0.49/km) and other related travel expenses. 9

Pricing is subject to change without notice. Cancellation and No Show Policy Millard Health s policy regarding cancellations and no shows is adhered to without exception. Regardless of the reason (whether it be illness, emergency or inclement weather) charges will apply for all appointments cancelled, rescheduled or missed with less than 24-business hours** notice. As the referral source/payer you are responsible for paying the associated cancellation, no show and late arrival fees. In the event that these fees are applied, Millard Health will issue an invoice for the applicable amount as defined below. Cancellation: A cancellation is defined as a service that has been booked and confirmed but is then cancelled or rescheduled by the client or referral source/payer with less than 24 hour notice. The period between 5pm Friday and 7am Monday and any day Millard Health is closed for business shall not be included in calculating the 24 hour period. An appointment may be cancelled by a Millard Health Physician or Clinician if it deemed unsafe to proceed due to medical or non-medical issues (eg. high blood pressure, alcohol/drug use). A 90% fee will apply to all cancellations received with less than 24-business hours** notice. No Show: A No Show is defined as a service that has been booked and confirmed that the client then fails to attend. A fee of 100% will apply to all missed appointments. Late Arrival: A 15-minute grace period will be allowed for medical appointments and 30-minute period for other appointments. After this grace period, the appointment shall be deemed to be a No Show, as defined above. However, if the service cannot be delivered due to the late arrival, the appointment may need to be rescheduled. A fee of 100% will apply to all late arrivals. Please note: All clients will be required to sign a consent form prior to their appointment. A parent or legal guardian must accompany individuals under the legal age of 18. Refusal to sign a consent form will be considered as a No Show as defined above.
