Benefits for Your Life

Paid Time Off Taking time off allows you to rest and recharge, and helps you maintain a proper balance between your personal and work life which is key to your overall health and wellness. Annual Leave You begin accruing annual leave on your first day of employment. Some employees are provided Paid Time Off (PTO) and an Extended Illness Bank (EIB) and others receive vacation and sick leave benefits. Regardless, both groups accrue time off based on length of service and hours worked. Designated Holidays are a paid benefit as well. Bereavement Leave Employees are eligible for up to three days of paid leave for a death in their immediate family (spouse, domestic partner, brother, sister, child, step-child, parent, step-parent, grandparent, grandchild, or spouse s parent, brother or sister). An additional two days may be granted for travel over 500 miles. Pay is pro-rated for part-time employees. Jury Duty Leave Employees on Jury Duty Leave shall be compensated for the difference between jury duty pay and their normal rate of pay. Military Training Leave In accordance with Washington State Law, annually from Oct. 1 through Sept. 30, Military Leaves shall be granted for up to twenty-one days upon presentation of official orders which require an employee to maintain status in a military reserve of the US. Employees will receive full wages in addition to military pay, and will not lose benefits accrued to the date such leave commences. Active Duty Leave Under Federal law, employees are eligible for up to five years of excused leave to participate in active duty in the military. Employees will have job and benefits protection for a period of 12 weeks. Employees taking Military Leave may use accrued vacation and personal holiday time before their leave becomes unpaid. Family Medical Leave Act Employees who have worked for EvergreenHealth for at least one year (12 consecutive months) and who have worked at least 1250 hours in the prior 12 month period immediately preceding the date a qualifying leave commences are eligible for Family Medical Leave Act. FMLA provides 12 weeks of job and benefits protection for the birth or placement of a child for adoption or foster care; the need of the employee to care for a qualifying family member (spouse, parent or dependent child under age 18) with a serious health condition; or the employee s own serious health condition that renders the employee unable to perform the full range of essential functions of his or her job, or a qualifying exigency arising out of the fact that a spouse, parent or child has been called to or is on active duty as a member of the military.

FMLA also provides employees with an eligible family member or next of kin that s a covered service member up to 26 work weeks of Service Member Family Leave for a covered service member undergoing medical treatment, recuperation, or therapy; otherwise in outpatient status; or on the temporary disability retired list for a serious injury or illness; or to care for a veteran who is undergoing medical treatment, recuperation, or therapy for serious injury or illness that occurred any time during the five years preceding the date of treatment. Family Care Act Employees may use earned and available sick, vacation and personal holiday benefits to care for a child under the age of 18 with a health condition requiring treatment or supervision; or a spouse, state registered domestic partner, parent, parent-in-law or grandparent who has a serious health condition or emergency condition. Maternity Disability Leave Maternity Disability Leave affords an employee job protection for all periods of disability related to pregnancy and childbirth. The accepted period of post-delivery disability is six weeks for a natural delivery and eight weeks for a cesarean delivery. Maternity Disability Leave does not include benefits protection; however an employee who is also eligible for FMLA leave will have benefits protection from the first day of work missed until their FMLA entitlement expires. Eligible employees may combine FMLA leave with Maternity Disability Leave to extend time away from work beyond the period of post-delivery disability. Family Military Leave The Washington State Military Family Leave Law allows an employee whose spouse or state registered domestic partner is a member of the United States armed forces (Army, Navy, Air Force, Coast Guard or Marine Corps Reserve), National Guard or reserves who has been notified of an impending call or order to active duty, or who has been deployed, or when the military spouse or State registered domestic partner is on leave from deployment, a total of fifteen days of unpaid leave per deployment. An employee is only eligible for this leave during a period of war declared by the President or U.S. Congress. Extended Disability Leave Upon termination of an employee s entitlement to statutory leave with job protection, EvergreenHealth will consider granting an Extended Disability Leave to an employee who continues to be disabled due to a serious health condition. Extended Disability Leave does not include job or benefits protection. The sum total of other leaves and Extended Disability Leave should generally not exceed six months in any 12-month period. Personal Leave When an employee wishes to take a leave of absence for reasons that do not qualify under other types of leave (such as unpaid vacation), they may apply for a Personal Leave of Absence. Generally, an employee needs to have been continuously employed by Evergreen for at least one calendar year prior to being eligible for a Personal Leave. EvergreenHealth determines whether or not the leave shall be granted and the duration. Personal Leaves do not involve job or benefits protection once available paid time runs out. Personal Leaves will not generally be granted for longer than three months. Education Leave After one year of employment, employees may request an Education Leave of Absence for job-related study. EvergreenHealth determines whether or not the leave shall be granted and the duration. Education Leaves do not involve job or benefits protection once available paid time runs out. Shared Leave Employees may voluntarily donate part of their vacation to co-workers whose paid leave has been depleted in response to extraordinary circumstances (i.e. severe illness) concerning themselves or immediate family. Employee Assistance Program This voluntary confidential assessment and referral service is designed to assist employees and their families in handling personal and work related problems. Alaska USA Federal Credit Union All employees are eligible for membership in the Credit Union. Direct Deposit Bi-weekly paychecks are automatically deposited in your bank account. Continuing Education EvergreenHealth supports the professional development of its staff by providing financial assistance to regularly scheduled employees for workshops, conferences, seminars, and other continuing education courses or programs. Benefit Opt Out Eligible employees with other medical coverage are given the choice to opt out of the benefits package for an increase in pay of 12 percent or 15 percent, depending on position. Cafeteria Discount All employees receive a 15% discount on food items upon presentation of an ID badge. Commuter Benefits All benefit eligible employees may purchase an ORCA transit care for an annual fee. Carpoolers, bikers, walkers, motorcyclists and vanpoolers may sign up for special transportation incentives. Dependent Care Spending Account Employees can set aside money on a pre-tax basis to pay for child care expenses (annual max $5,000). EvergreenHealth pays the cost of administering the plan. Health Care Spending Account Employees can set aside money on a pre-tax basis to pay for out of pocket medical expenses (annual max of $2,500). Evergreen pays the cost of administering the plan. Evergreen Professional Pharmacy Discount All employees and their immediate family members receive prescriptions and over the counter items at cost plus a minimal handling fee when purchased at the EPC Pharmacy.

Planning Your Future Life Insurance EvergreenHealth provides a basic life plan that automatically insures employees for 1 times the base salary. AD&D coverage pays 1 times the base salary to you or your beneficiary if you die or suffer certain injuries as a result of an accident. Supplemental Life Insurance Employees may purchase additional life insurance in multiples of salary (1x, 2x, 3x, etc.) up to a maximum of $350,000 or 5 times annual salary, whichever is less. Dependent coverage is also available. Rates are determined based on the age of the employee/dependent and coverage. Short-Term Disability EvergreenHealth offers a voluntary short-term disability option to tall benefit-eligible employees. The plan provides up to 60% salary replacement after a 30 days waiting period if you become disabled and unable to work. Employees pay the cost of coverage. Rates are determined based on the age of the employee. Long-Term Disability EvergreenHealth provides long-term disability coverage to all benefit-eligible employees. Long-term disability replaces 60% of your salary up to a maximum of $12,500 a month when you re disabled and unable to work for an extended period. LTD replaces 40% of your salary for non-supervisory positions with an option to buy-up to 60%. Deferred Compensation Plan As a public hospital district, EvergreenHealth sponsors a 457 Deferred Compensation Plan available to all fulltime, part-time and per diem employees. Employees may defer up to $17,500 or 50% of their compensation, whichever is less on a pre-tax basis. Employees 50 or older may defer an additional $5,000. 401(a) Match This plan is available to all full-time and part-time employees scheduled to work at least 20 hours a week, upon completion of one year of continuous employment. Enrollment is the first of the quarter upon meeting the eligibility requirements. Evergreen matches 50% of deferrals up to a total match of 4%; for supervisors and above, the match is 100% up to 6%.

401(a) Base Pension EvergreenHealth contributes a percentage of compensation for all full-time and part-time employees scheduled to work at least 20 hours a week upon completion of one year of continuous employment in a benefit-eligible status.
