Musculoskeletal Sonography Certificate Admissions Requirements Applicants must hold an approved credential or have graduated from an accredited sonography program or have a professional licensure to apply to this program. Spring 2016 Entry

HEALTH PROGRAMS ADMISSIONS REQUIREMENTS Below are the general admissions requirements for all health programs followed by program specific requirements. Please keep in mind that admission to all health programs is a two-step process. Step 1: Complete your SPCC Admissions File 1. Complete the SPCC Application for Admission Submit an application for admission online at http://www.spcc.edu. If you already have a CFNC account, use your login information to fill out SPCC s application. If you do not have a CFNC account, you will create one as the first step of the application. Keep track of your account information. You may apply to only one health program. 2. Submit official high school, High School Equivalent, or adult high school transcript It is your responsibility to request transcripts and pay any applicable fees. Official transcripts should be mailed directly from the issuing institution/agency to SPCC. Unofficial (student-issued/faxed/emailed) transcripts will not be accepted. 3. Submit official transcripts from all colleges listed on your application A Request for Transcript Evaluation Form must be completed and sent to the Registrar for evaluation of transfer credit. All foreign transcripts (secondary and post-secondary) must be evaluated by an authorized evaluation company at the student s expense. To evaluate foreign transcripts, the transcript, course descriptions, and the grade system must be translated. 4. Prepare for and take the College Placement Test (CPT) You have one opportunity to take the CPT (Reading, Math and Computer Diagnostics). The initial test is free. Students are strongly encouraged to thoroughly review test study material and use our test preparatory services. Visit the Testing Center webpage for study resources. Reading, Math, and Computer diagnostic scores are good for five (5) years. Accuplacer Reading, Sentence Skills, Computer Competency, Arithmetic, and Algebra scores are good for five (5) years. All students are required to take the CPT unless exempt by a waiver. High school graduates within five (5) years of enrolling that meet certain college ready requirements (Multiple Measures). SAT scores of at least 500 on Critical Reading and Writing and 500 on Mathematics will exempt student from the CPT. ACT scores of at least 22 on Reading and 18 on English and at least 22 on Mathematics will exempt student from CPT. Completion of college-level English and math (with a grade of C or higher) from a regionally accredited institution. Completion of Developmental courses in English and math (with a grade of C or higher) within five (5) years of application packet deadline. Completion of a Bachelor s degree or higher. Appropriate scores on Advanced Placement (AP) or College Level Examination Program (CLEP) exams. 3

SPCC must receive an official score report from ACT, the College Board, or from the college where the Diagnostic was taken. SPCC must receive official high school and/or college transcripts for any waivers related to course credit or degree completion. Step 2: Musculoskeletal Sonography Program Admission Requirements 1. Have valid required College Placement Test scores or appropriate developmental coursework completed within five (5) years of application packet deadline or college placement test waiver. 2. Proof of current certification in at least one Sonography specialty from the following: American Registry of Radiologic Technologists (ARRT) o RT (S) or RT (V) The American Registry of Diagnostic Medical Sonographers (ARDMS) o RDMS o RVT OR Proof of graduation from an accredited program in Sonography, Vascular or a sonography credential. You must provide letter of reference on letterhead from your employer, program director, or from a credentialed sonographer for these options. OR Proof of current licensure as one of the following: Doctor of Medicine (MD) Doctor of Osteopathic Medicine (DO) Doctor of Podiatric Medicine (DPM) Doctor of Chiropractic (DC) Nurse Practitioner (NP) Physician Assistant (PA) Doctor of Physical Therapy (DPT) Physical Therapist (PT) Bachelor of Medicine, Bachelor of Surgery (MBBS) outside of North America You must complete scanning module, including lab, attached to MSK 110 by the end of week one (Require for accepted doctor students, optional all others). 3. Current Healthcare Provider CPR certification. This course is available through SPCC s Career Development & Continuing Education Division, at http://www.spcc.edu, Career Development & Continuing Education, Medical Professions, CPR: Healthcare Provider. CPR classes taken at other facilities must be by a Healthcare Provider. 4. Approved clinical site (recruited by student) - Applicants to this program are responsible for recruiting his/her own clinical site affiliate by the program application deadline. A letter from the clinical site on letterhead stating the following is to be included in the application packet: Clinical site contact name, address, phone, email Facility name Student name 4

Upon acceptance into the program, the Program Director and/or Clinical Coordinator will assist the student in orientation for the clinical site as well as initiating the Clinical Agreement between SPCC and the site. Musculoskeletal Sonography Application Period for Certificate Option Ends November 30, 2015 for Spring 2016 Entry How do I apply for admission to the Musculoskeletal program? Complete Step 1 and 2 listed on pages 3, 4, and 5. When you have completed both steps, collect only the required documents listed below and put them into a 9 x12 envelope with your full name, SPCC ID, and program printed clearly in the upper right corner. Your packet should only include: Musculoskeletal Sonography Application pages 8 and 9. Proof of completion of an accredited program in General Sonography, Vascular or Radiography or a Sonography credential OR at least one approved credential OR professional licensure listed on page 4. A printout or copy to prove the above credential or licensure. Copy of current CPR card or certification document. The CPR class must be offered by a healthcare provider. Make sure card is signed and copy both sides of CPR card. Letter of reference on letterhead from your employer, or program director, or from a credentialed sonographer (Non-MD, DO, DPM, DC, NP, PA, DPT, PT Only). Letter on letterhead from clinical site affiliate. When are students admitted into the Musculoskeletal Sonography program? Health programs at SPCC have limited enrollment and a competitive admissions process. Students are admitted into the Musculoskeletal Sonography program fall and/or spring semesters for the certificate programs each year. Students who submit a complete application packet by the November 30 deadline will be given priority consideration for admission for the Spring semester class. NOTE: If the number of qualified applicants meeting the November 30 deadline is not sufficient to fill all program openings, complete application packets will continue to be accepted until the maximum enrollment number is met. What do I need to do to be considered for entry into the Musculoskeletal Sonography program? To be considered for admission into the Musculoskeletal Sonography program, you must complete the requirements listed on pages 3, 4, and 5 and submit a complete admission packet by the deadline. What are the required CPT scores for the Musculoskeletal Sonography program? If you do not qualify for a College Placement Test (CPT) waiver, you must meet the scores listed below on the CPT to qualify for the Musculoskeletal Sonography program. 5

Combined Reading and Sentence Skills score 151 or higher Computer Skills: 80 Arithmetic: 55 (MAT 060) Algebra: 75 (MAT 070 and MAT 080) OR Mastery in DMA 010 through DMA 080 If you do not meet these required CPT scores, you must enroll in and complete the appropriate developmental course(s). A waiver may be used in place of some or all of these requirements. Is there a waiting list? There is no waiting list to enter into any SPCC health program. Students are chosen for the Musculoskeletal Sonography program based on first come-first filled basis as determined by the date the packet is received by SPCC. Note: Proof of appropriate credentials OR proof of completion from an accredited Sonography, Vascular or a Radiography program and a Sonography credential OR proof of professional licensure (see page 4) is required for admission. If you are not accepted into the program for the upcoming fall or spring semester, your packet will be shredded. Please make a copy of your packet documentation prior to submission. Enrollment Services is not able to copy your packet for you. How will I know if I m accepted into the Musculoskeletal Sonography program? You will receive a letter from the Admissions Office within a month of the program application deadline notifying you of your provisional acceptance into the program. ONCE YOU RECEIVE THIS LETTER, you must do the following: (1) Submit the Student Medical Form for North Carolina Community College System Institutions. Deadline and instructions for completing your medical form will be included in your provisional acceptance letter. NOTE: Accepted students will be required to have a physical exam and complete listing of immunizations. Cost is the responsibility of the student. Applicants accepted for admission into the Musculoskeletal Sonography program may be required to complete a criminal background check and a drug screen after notification of acceptance and prior to participation in the onsite clinical portion of the program. Based on the results of the criminal background check and the drug screen, hospitals or clinical facilities where you will participate in onsite training may deny you access to their facility, resulting in your inability to complete the clinical portion of the Musculoskeletal Sonography program. If you are unable to complete the clinical portion of your training, you will be unable to progress in the program. Disability Services SPCC provides an ongoing program of services that encourages students with disabilities to integrate themselves into all aspects of college life. The Disability Services provider is available to assist with registration, counseling, academic advisement and arrangements for appropriate accommodation. Reasonable accommodations are established on an individual basis. Students must provide documentation of a disability from a physician or other licensed professional in order to establish eligibility for services.
