21CFR11 Checklist Key: C = Conforms to Requirement, NC = Non-Conformance, N/A = Not Applicable, IP = In Process Requirements 21 CFR Part 11 Requirements Status Explanation 11.10j Documentation Controls - Establishment and use of appropriate controls over w ritten policies that hold individuals accountable and responsible for actions initiated under their electronic signatures to deter record and signature falsification. 11.10k Use of appropriate controls over systems documentation: (1) Adequate controls over the distribution of, access to and use of documentation for systems operation (2) Revision and change control procedures to maintain an audit trail that documents time sequenced development and modification of systems documentation. 11.3 Controls for Open Systems 11.5 Signature Manifestations 11.50a Signed electronic signatures shall contain information associated w ith the signing that clearly indicates the follow ing: 11.50a cont. (1) The printed name of the signer (2) The date and time w hen the signature w as executed (3) The meaning associated w ith the signature 11.50b All items included in 11.50a(1), 11.50a(2), 11.50a(3) above shall be: Subject to the same controls as for electronic records Included as part of any human readable for of the electronic record (such as electronic display and/or printout or report) 11.7 Signature / Record Linking - Electronic signatures executed to electronic records shall be linked to their respective electronic records to ensure that the signatures cannot be excised, copied, or otherw ise transferred to falsify an electronic record by o SUB-PART C: ELECTRONIC SIGNATURES 11.1 General Requirements 11.100a Each signature shall be unique to one individual and shall not be reused by, or reassigned to anyone else 11.100b The identity of the individual shall be verified prior to the organization establishing, assigning, certifying, or otherw ise sanctioning that individual's electronic signature 11.100c Certification of electronic signatures to the FDA 11.2 Electronic Signatures and Controls 11.200a Electronic signatures not based on biometrics shall: (1) Employ at least tw o distinct identification components such as an identification code and passw ord
