Credit Card Authorization for Third Party Services Credit Card Account Information Name on Card: Type of Card: Credit Card Account #: Expiration Date: Security Code Billing Zip Code (Threedigit number on back of credit card) Billing address, if different from present address: Initial Credit Report The name of the credit reporting company, their contact information and the charges will show on the first page of your credit report. Maximum $30 charge for credit report will be charged. Reissue Credit Report Costs There can be additional credit report charges incurred in the placement and processing of your loan application. Repositories are charging additional fees for reissue of the initial credit report to lenders for their underwriting systems. Reissuing of your credit report does not cause multiple inquiries. The repositories are charging between $4.00 to $7.00, per bureau, per lender for reissue and secondary use fees, plus tax. Customer Service Costs and Rapid Rescore Costs There could also be charges for customer service to correct and/or revise information on the credit report. Customer service charges are up to $10 per account, plus tax. Rapid rescore of your credit report could be up to $35 per person, per account for each repository, plus tax. Your Mortgage Consultant will get your permission before ordering these services. Home Valuation Code of Conduct Law: To help enhance the integrity of the home appraisal process in the mortgage finance industry, in March 2008, Fannie Mae entered into an agreement with the Federal Housing Finance Agency and the New York Attorney General s office to adopt certain policies relating to appraisals for loans. Effective May 1 st, 2009 this applies to all loans delivered to Fannie Mae and Freddie Mac in the United States. Appraisal costs vary between $450-$550. The code requires that an appraiser must be licensed or certified by the state in which the property to be appraised is located. The lender / broker is required to provide a copy of the appraisal to the client no less than three business days prior to closing. The law requires the appraisal be paid directly by the lender. Therefore most lenders require the client pay for the appraisal in advance to cover this cost. The lender selection of the appraisal must be done through a random selection process and / or be ordered through a National Appraisal Service. Neither the lender nor the Loan Officer are allowed to directly interact with the assigned appraiser. A lender may be willing to accept an appraisal from a different lender however they may reject the appraisal based upon the appraisal being ordered through a vendor they do not have a relationship with. There is a quality control component that is required on all appraisal services. I hereby authorize charges to my credit card for third party services necessary to process my (our) loan.
