SUNY Adirondack 640 Bay Road Queensbury, NY 12804 Dear Nursing Applicant, Thank you for considering Nursing at SUNY Adirondack as your future course of study. The Nursing Program is a competitive and rigorous program of study that prepares students for successful passage of the NCLEX RN licensure examination. All students wishing to be considered for the Nursing Program must complete a SUNY Adirondack Application available online on the SUNY Adirondack website. There is a $35.00 application fee to apply. The Nursing Program requires that you also complete a Supplemental Nursing Program Application that is enclosed in this packet. This application is submitted directly to the Health Sciences Division. The enclosed Admission Policy and Procedure will assist you in the process. Acceptance into the Nursing Program is selective and completion of the supplemental nursing application does not guarantee acceptance. Health Sciences Division Admission Checklist Admission to the Nursing Program is competitive. Prior to submitting the supplemental Nursing application, please be sure it is complete and all required records are attached. Applicants must be sure that they have been officially accepted to SUNY Adirondack in order to be eligible to apply to the Nursing Program. Please note that if you have a Banner ID (500- or 5000- number), you should not need to complete an application to the College. Application to SUNY Adirondack (either College application or SUNY Application) Official High School Transcripts or GED with test scores (Please send to SUNY Adirondack Admissions) Official College Transcripts from other colleges attended (Please send to SUNY Adirondack Registrar s office) Items above are required for acceptance to the College in general. It is the applicant s responsibility to submit all required documents on time.
Nursing Program Admission The Nursing Admissions Committee at SUNY Adirondack welcomes applications from all students who meet the academic and personal requirements to succeed in this program. Applications to the Nursing Program are encouraged from: High school seniors or recent high school graduates Current college students attending SUNY Adirondack or any other college College graduates Individuals interested in making career changes Licensed Practical Nurses Graduate Practical Nurses Applicants who have been found guilty of a crime and/or have pending criminal charges (misdemeanor or felony) must submit a letter to the Office of the Professions for New York with explanation of charges. NY State Education Department Office of the Professions 89 Washington Avenue, Albany, New York 12234-1000. The applicant s eligibility for licensure will then be determined. Criteria for Admission: Admission Process New Applicants Minimum cumulative GPA of 2.8 or higher if enrolled in college courses. Students must be eligible to enter BIO 107 (Regents or college preparatory Biology and Chemistry in last 5 years with Regents score of 80% or above or BIO 103 and CHM 103 or equivalent course with a grade of C or better.) Students must be eligible to enter college level Math and English courses to be determined by SUNY Adirondack Accuplacer Test, or have earned college level Math and English credit. Students who do not demonstrate competency in Math will be required to take Math 167 or Math 097 prior to or concurrent with first semester nursing courses. Procedure for Admission: Please complete a SUNY Adirondack Application available online on the SUNY Adirondack website. Be sure to include an official copy of your high school transcript or a copy of your GED along with the $35.00 application fee. Submit official college transcripts from each college attended directly to SUNY Adirondack Registrar s office. In addition, Applicants must submit the following materials to the Health Sciences Division Nursing Admissions Committee, 640 Bay Road, Queensbury, NY 12804 It is the applicant s responsibility to submit all required documents on time. SUNY Adirondack Supplemental Nursing Program Application High school transcripts or copy of GED Unofficial transcripts from all previous colleges attended. (Transcripts will not be forwarded from the Registrar s Office) Unofficial SUNY Adirondack academic transcript, including current semester enrollment. To be considered for admission, the applicant must submit all Nursing application materials by the following deadlines: Fall admission Applications will be accepted between January 1 st and February 1 st. Spring admission Applications will be accepted between August 15th and September 15th for Spring consideration. Nursing applications received before or after these dates will not be considered for review. Once all Nursing application materials have been received, selected applicants will be notified by mail to schedule an interview. The Nursing Admissions Committee will begin making admission decisions shortly after all interviews have taken place. All applicants for a class will be notified in writing of the Committee decision prior to SUNY Adirondack priority registration. Accepted applicants are required to confirm their intention to enter the Nursing Program by emailing the faculty secretary at: oharac@sunyacc.edu. Failure to confirm your intention to enter by the deadline noted on your admission letter will result in forfeiture of a position in the Nursing Program.
