Health Savings Plan (HSP) You have the freedom to use any physician or hospital you want. But you can save money when you choose physicians (including specialists), hospitals and labs in the UnitedHealthcare network. Health care providers who are in the network have agreed to charge lower prices. You also have coverage if you receive care outside the network. However, you will likely pay more for the service. A national network to help lower your costs No matter where you are in the U.S., a network physician is likely nearby. You do not need to choose a primary care provider. But a primary care provider (PCP) can be helpful in managing your care. You do not need a referral to see a specialist. See any physician, including specialists, without referrals. YOU CAN OPEN A HEALTH SAVINGS ACCOUNT. This plan lets you open a Health Savings Account (HSA). An HSA is a personal bank account to help you save and pay for health care, while giving you real tax savings. You can use the HSA to pay for qualified medical expenses. Use it for covered services such as physician office visits and prescriptions. Use it for other qualified medical expenses like dental and vision services. You own the HSA. The money is yours to keep. There s no use it or lose it rule. The money stays in the account until you use it. If you leave your employer, change medical plans or retire, you take it with you. Use it to pay for services today or save it for a future need even into retirement. You don t have to pay federal or, in most instances, state income taxes on: Deposits you or others make Money you spend on qualified expenses Interest earned while money is in the account Health savings account vs. flexible spending account (FSA) Keep in mind, the IRS does not permit you to elect both a health care FSA and an HSA. If your spouse has one of the two, for example, through another employer, you cannot elect the other type of tax-advantaged plan. Open your HSA with Optum Bank SM, Member FDIC. TWO STEPS TO START YOUR SAVINGS 1 Open your HSA. You may be able to open your HSA through Columbia University. See your health plan documents for details. 2 Set a savings goal. Even small deposits can make a big difference. You can make regular, pre-tax deposits through payroll deductions. 2015 HSA limits The IRS limits how much you can put into your HSA each year. The 2015 limits are: $3,350 for individual coverage $6,650 for family coverage Are you 55 or older? You can put in an extra $1,000 this year. HOW THE PLAN WORKS Here s how the plan works in three simple steps. Remember, the Plan covers your preventive care at 100% when you use network physicians. STEP 1 STEP 2 STEP 3 Your deductible You pay until you reach the deductible. The deductible is the amount that you pay for covered health care services before your medical plan starts to pay. You will pay for all covered services (medical and prescription drug) until you pay your deductible. You can pay for services with the money in your HSA. Or, you can pay another way (cash, credit card or check) and let your HSA grow. Your coinsurance You and your plan share the cost of services. After paying the deductible, your plan will have coinsurance. Coinsurance is when you and your plan share the cost of covered services. Your out-of-pocket maximum You are done paying. If your deductible and coinsurance payments reach the out-of-pocket maximum, you are done paying. Your plan will pay 100% of in-network covered medical services, including prescription drug costs, for the rest of the calendar year. This limit is there to protect you if you have a major medical event.* Paying for prescriptions Prescription drug coverage is integrated with the HSP medical coverage. This means you pay for your nonpreventive prescription drugs until you meet the HSP deductible. Once the deductible is met, the prescription copay applies. Prescription drugs that are categorized as preventive under federal guidelines are not subject to the HSP deductible, so you are only responsible for paying the appropriate copay, which accumulates toward the HSP out-of-pocket maximum. For a list of prescription drugs that are considered preventive, go to the Forms & Documents section of the HR website (hr.columbia.edu) and search for Preventive Drug List. When you reach the limit, the plan pays 100%.* Preventive care is covered 100% when you use a network physician. *Note: You may still be responsible for out-of-network services even if you have reached your out-of-pocket maximum. UNITEDHEALTHCARE VISION WITH A UNITEDHEALTHCARE VISION PLAN, THE CHOICES AND SAVINGS ARE EASY TO SEE. Sign up and see the savings. When you sign up for a comprehensive vision plan, you usually get a larger choice of frames and lenses. You have access to a national network of providers. You have access to a national network of providers, including optometrists and ophthalmologists. There are two ways for you to find a vision provider: Visit the vision website. To access the vision website, log into myuhc.com, click on the Benefits & Coverage tab and select Vision from the left menu. You have coverage for an eye exam and eyeglasses/contact lenses. Your plan includes a covered eye exam with eyeglasses or contacts. You have access to an unlimited selection of eyeglass frames.* You also get an allowance to help buy any frame when you use a network provider. You can use your contact lens benefit toward new or replacement contacts, contact lens fittings and follow-up visits. Save an additional 10% off your contact order when you use our online site. FLEXIBLE SPENDING ACCOUNT PAY FOR CARE. KEEP MORE OF YOUR MONEY. A Flexible Spending Account (FSA) is a special account that lets you set aside money, before it is taxed, to help pay for care. You must re-enroll in an FSA each year. Sign up for a health care FSA to pay for eligible health care expenses, which include many medical, prescription drug, dental and vision services, and supplies. Sign up for a dependent care FSA to pay for eligible dependent care expenses. This can include child or adult day care services and programs. AN FSA LETS YOU KEEP MORE OF YOUR MONEY. You keep more of your money because you don t pay taxes on the money you put into your FSA. The amount you save depends on how much you put into your FSA and your income tax rate. IT S EASY TO USE YOUR FSA. Automatic payment: If a service is covered by your medical, dental or vision plan, we can automatically pay the bill (claim) from your FSA so you don t have to. Direct deposit: Don t wait for a check in the mail. We can reimburse your money directly into your personal bank account. Online claim form: Easily submit your claims on myuhc.com to ask for reimbursement from your FSA. You can even sign up for email alerts to track payments. This year s FSA dollar limits The most you can deposit into your FSA this year is: $2,550 for a health care FSA $5,000 for a dependent care FSA Health Care Spending Card MasterCard : This handy debit card will make it easy to pay for services from your FSA. For more details about FSAs, go to hr.columbia.edu/benefits. Child care benefit Eligible officers can elect to receive up to a $2,000 contribution from Columbia University to a dependent care FSA. AFTER YOU ENROLL: PREVENTIVE CARE TAKE ADVANTAGE OF PREVENTIVE CARE TO HELP MANAGE YOUR HEALTH. What does preventive care do for me? Regular preventive care visits and health screenings may help to identify potential health risks, and may help you avoid serious health problems. Your physician will determine what preventive care services or health screenings may be right for you. What is covered as a preventive care service? When a service is provided specifically for a preventive screening, and there are no known symptoms, illnesses or history, the service will usually be considered preventive care. Preventive care services will be based on many factors such as your age, gender, overall health status, personal health history and your current health condition. Preventive care services may include many types of services, including: Physician office services: Lab, X-ray or other preventive Routine physical examinations screening tests: Well-baby and well-child care Screening mammography Immunizations Colorectal cancer screening Cervical cancer screening Osteoporosis screening How do I know if a service is preventive care or not? When you visit your physician, the services you receive will be considered either preventive care or non-preventive care depending on many factors. See if you can determine whether the care received in the following scenarios would be considered preventive or non-preventive. Situation 1 Situation 2 A woman visits her network physician for her screening mammogram. Answer: This is considered preventive care because her visit is part of a routine annual exam and has not been prompted by any sort of previous diagnosis. A man visits his network physician for his preventive care exam and based on his age, his physician recommends a screening colonoscopy. Answer: This is considered preventive care because this service is part of a routine annual exam and has not been prompted by any sort of previous diagnosis. Will my medical plan consider medications a preventive service? Through your pharmacy benefit you may be eligible for certain medications covered as preventive care, including: Certain FDA-approved women s contraceptives Folic acid supplements for women who may become pregnant Generally, over-the-counter medications are excluded from coverage.
