MUST USE: Safari, Google Chrome or Firefox 1. Copy and paste http://mercyhurst2.atsusers.com/ into a new tab on your web browser so you can continue to use these directions. Choose the ATS Athlete Portal button from this screen. 2. In the boxes, use the following to log on: Athlete ID: NEW Password: NEW Database: ATSMERCYHURST 3. You will need your student ID number for the next step. Use the last 6 digits of our student ID as your Athlete ID. If you do not know your student ID number, contact your coach and they will supply you with the number.
4. Fill out the information on this page completely. The information in Yellow is mandatory. If you are an international student and do not have a Social Security Number please put the word None. Last 6 digits of your student ID# Year should be the year you are planning on graduating. If you have any medical alerts, allergies, or current medications please use the dropdown box or type it in or put none if this does not apply to you.
5. Fill out the rest of the tabs completely. 6. When you click the insurance tab, you will see this screen. Click the Add button. You will then have to complete this form. The information in Yellow is mandatory. Please bring your insurance card with you to physicals.we will scan it and put it in your file. Feel free to load an image in before you come, but we MUST have a clear copy of your insurance card front and back. 7. Fill out your emergency contacts using the Contacts Tab. You will see a similar Add button as in the last tab. Please provide 2 emergency contacts.
8. Under the Athlete Forms tab, you must fill out the 5 forms: 1. Freshman/Transfer Athlete Pre-Participation Form 2. HIPPA/Assumption of Risk 3. Sickle Cell Acknowledgement 4. MAE/Sports General Release Form 5. MCH Medical Release When you are ready to sign any form, you can do that at the bottom by following these four steps in the listed order. If you are under 18, please have a parent guardian sign as well. 1. Use your cursor to sign in the tan box. 2. Type your name in the Signed By box. 3. Click Sign 4. Click Save When you successfully save a form, the form will show up with a Date/Time at the top of the page. Please verify you have completed the required forms before continuing. (Freshman/Transfer Athlete Pre-Participation Form, HIPPA/Assumption of Risk, and Sickle Cell Acknowledgement, MAE/Sports General Release Form, and MCH Medical Release.)
9. It is the policy of Mercyhurst University that all incoming student athletes provide proof of official sickle cell testing results prior to participation in any intercollegiate event, including, but not limited to: strength and conditioning sessions, practices, competitions etc. IF you are born in the states of Pennsylvania, New York or Ohio, you may proceed to the efiles tab for assistance with obtaining Sickle Cell Trait results. All other student athletes must contact a physician. American primary care physicians should be able to provide you with your newborn screening results. If you cannot obtain newborn screening results, you should be tested for sickle cell trait. Additional sickle cell trait information can be found in the sickle cell acknowledgment form as well as on our website at hurstathletics.com/athletictraining Persons Born in Pennsylvania: You may fill out and sign this form yourself. Form must be faxed to the number provided. Results will be faxed directly to Mercyhurst University. Persons Born in New York: The form found on the efiles must be signed by a physician and submitted to the state to obtain new born screening results. Ohio Residents: The state of Ohio provides regional testing centers. The map will help you to identify the region coordinator. 10. IF you are diagnosed with ADD/ADHD and are medicated, you must proceed to the efiles tab and click the icon under View. Follow the instructions so that we may document your diagnosed medications in order to stay compliant with NCAA drug testing guidelines.
