Meridian Health Phase 1 Quickly integrate its six hospitals and SNF/LTC facilities in two central New Jersey counties. Integrating facilities to improve patient outcomes and reduce costs Initial pilot involved 5 hospitals and 10 physicians in two counties One month into program numerous physicians were regularly using HIE with over 60 more requesting data exchange with their EMRs HIE successfully integrates multiple systems, including Siemens, MEDITECH, Sunquest, MEDHOST, T-Systems, EDIMS, and others, into its broad information platform All six hospitals and 4 long-term facilities are now live.
