11-12 Butler Tech Online Course Purchase Form Notes This form should only be used for courses that are being purchased with a BTOL teacher only. It is recommended for students to discuss courses selection with their guidance counselor to be sure courses will be applied as intended towards graduation requirements. Complete the form in its entirety. Leaving any information blank is a chance for error! Course Selection: Check the appropriate box next to the course(s) the student should be enrolled into. Be sure to pay attention to Foundation vs. College Prep. Foundation Courses: Our foundation courses are written primarily for use as credit recovery and for at-risk students. These courses are more straightforward and less rigorous than the college prep courses. These courses align with the appropriate Ohio state standards. College Prep Courses: Our college prep courses also align with the appropriate Ohio state standards, but, many times, exceed these standards and require more time, more effort, and higher level critical thinking skills than the foundation courses. Other Courses on Pages 2 4: These are courses that Butler Tech Online has licensed from Aventa Learning. Some of these will be taught by a full-time BTOL teacher, while others may be taught by a part-time employed teacher. Some of these courses require additional texts or supplies that must be provided by the student. A list of these requirements is available upon request. **Some courses require a minimum number of enrolled students for the course to be offered. If there is no checkbox available and the selection cell is shaded, that course is not offered at that level. For example, there is not a college prep version of Pre-Algebra A or B, or a foundation version of Physics A or B. In the course name, A refers to first semester content, B refers to second semester content. Any course may be taken at any time. For example, a second semester course (ENG 9 B) can be taken during the first semester of the year and a first semester course could be completed during the second semester. If there is no A or B indicated, the course is only a 1 semester course. All courses listed are worth ½ credit. and End : A Personalized Education Plan (PEP) will be developed for course completion tracking purposes. This PEP is dependant on the start and end dates entered here. Please consider that a student on a normal pace, working 40 minutes per school day, should have 18 weeks to complete a semester course. If you do not enter dates here, we may make an incorrect assumption. To be sure the student is on the appropriate schedule, please enter a start and end date. *Completion Time When courses are purchased, the student will have access to the course for one year from the date of purchase. It is Butler Tech Online s intent to provide year round teacher support, but we cannot guarantee teacher availability over the summer at this time. Submit to Butler Tech Online: You may complete this form by hand and mail or FAX to Butler Tech Online: 3603 Hamilton, Middletown Rd Fairfield TWP, OH 45011 FAX: 513-645-8300 You may also complete electronically and e-mail as an attachment to btonline@butlertech.org Questions? 513-645-8273
